Sounds like there may be something interfering with normal functionality. If you have a URL where the form is displayed, I would be glad to take a closer look.
No it is not displayed, check the plugin setting to display form only to logged in users is enabled.

It looks like there was a problem with plugin installation. Try uninstalling/deleting the plugin and try again with a fresh download. After installing and activating, do not make any changes to plugin settings, just reload your page and try again to see if it works.

Hey thanks. Yes I think it was working but I stopped using it later as I installed phpbb forums and we are using mchat plugin in that one
