Some web hosts disable the ability for you to set a specific reply-to address. In fact some web hosts prevent you from changing the “From” email all together. Bluehost is one of the known offenders of this.

The best thing you can do is setup your WordPress site to NOT send emails using your web host. This will by pass your webhost and usually fix most email related issues.

The {user_ip} smart tag should show the IP address of the vistor who submitted the form. Can you let me know where and how you are trying to use the smart tag? This will let me investigate further.
