I cannot add new images to an existing gallery block from the media library, I can only add images over direct upload works.
What happens is that all images are gone, except the newly added one.
I have the issue on several sites, and default theme 2021 activated.

It appears the gallery UI could use some developer work. When the media library first opens, if you’re immediately in add to gallery mode, the existing images are not selected and they get dropped if you commit the edit.

If before adding, you switch to edit gallery mode first (link in left sidebar), then the existing images are selected and you can then add to the gallery, then commit the edits and all images will remain.

Sorry for the confusion, it shouldn’t be that way, but at least there’s a workaround.

Never thought of going into editing mode first, because you obviously arrive in the wrong place (add to gallery instead of edit gallery) if you click the button “media library” under the gallery. Thank you very much for pointing this out bcworkz.

You shouldn’t have needed to think of that! I stumbled upon the workaround by accident. Turns out it’s a known issue.
