Bug in include_once

In hammy.php inside the plugins/hammy directory, the below line fails to include the options.php file:

include_once( ‘includes/options.php’ );

Change that to:
include_once( “{$hammyPath}/includes/options.php” );

You will obviously have to define $hammyPath, which you can by the following:
$hammyPath = dirname( __FILE__ );
