    Checkout? WP has no checkout page. Would you be using the WooCommerce plugin by any chance?

    If so, find the appropriate template in the woocommerce folder structure and copy it to a similar woocommerce structure in your child theme (skipping the templates folder IIRC). Edit the child theme version as desired. This is better documented on the WooCommerce website.

    You’ll also need to use wp_enqueue_script() to load some javascript to do a comparison when the form is submitted.
