Forminator does not support shortcodes in email notifications. If you would like to call the page title and the page URL where the form is located, please use Form Data, which can help in that matter:
https://wpmudev.com/docs/wpmu-dev-plugins/forminator/#form-data
In your case those will be:
- {embed_title}
- {embed_url}

OK, I think the problem comes from an automatic import from Contact Form 7. It seems that the Form Datas are not correctly transformed.
