Objective: This article aims to discuss the multidisciplinary approach required in the treatment ... more Objective: This article aims to discuss the multidisciplinary approach required in the treatment of cases of impaction and ankylosis of permanent teeth, associated with a history of trauma, considering the psychological state of the child and family when faced with a traumatic case of bullying, by reporting the complex treatment of a central incisor needing to be orthodontically moved across the midline. Conclusion: This clinical case was a major challenge, which included complex multidisciplinary procedures. Results and stability after 26 months of retention indicated successful orthodontic space closure of two maxillary teeth, without the use of implants or prostheses, in an adolescent patient who had a history of dental trauma, alveolar bone loss, and an uncertain initial prognosis.

INTRODUÇÃO: Ocasionada, geralmente, por hábitos deletérios, a mordida aberta anterior pode persis... more INTRODUÇÃO: Ocasionada, geralmente, por hábitos deletérios, a mordida aberta anterior pode persistir mesmo após a remoção do fator etiológico primario, devido ao posicionamento atípico da língua, o que torna essa má oclusão muito recidivante. CASO CLÍNICO: O presente artigo relata o caso clínico de uma paciente de oito anos e dez meses de idade que apresentava má oclusão de Classe II de Angle, mordida aberta anterior de 4,4mm, postura atípica da língua, respiração buconasal e discrepância negativa limítrofe na arcada superior. RESULTADOS: O tratamento interdisciplinar envolveu Otorrinolaringologia, Fonoaudiologia, Cirurgia e Ortodontia. O tratamento interceptativo, que alterou o padrão de hábitos e comportamentos, e o posterior tratamento ortodôntico, que envolveu exodontia de pré-molares, levaram a resultados estéticos e funcionais satisfatórios e estáveis, podendo-se observar um sutil aprofundamento da sobremordida vinte e oito meses após o término do tratamento. It was with great joy that I accepted the invitation from the Dental Press Journal of Orthodontic... more It was with great joy that I accepted the invitation from the Dental Press Journal of Orthodontics to coordinate an interview with Dr. Jeffrey Okeson. I have known Dr. Okeson for over 14 years. All started when I had the fantastic opportunity to be one of his residents for 3 years, and ever since, he has been my mentor and my friend. Dr. Jeffrey Okeson, despite of being a very famous and tremendously talented professional, continues to be an extremely nice and simple person. Due to these and many other reasons, I call him "my American Father"!!! He is married for 48 years to Mrs. Barbara Okeson, and they have two sons and one granddaughter. Dr. Okeson has a twin brother who is also a dentist. He enjoys jogging and has continued to jog for over 40 years. He also enjoys traveling and meeting people. Dr. Okeson has more than 240 professional publications. One of his textbooks, Management of Temporomandibular Disorders and Occlusion, is used in most of the United States dental schools and has been translated into eleven different foreign languages. However, it is important to highlight that the patient who has vertical bone loss has unique needs, arising from the frequent related pathological migrations. Therefore, it requires an individualized orthodontic treatment in terms of anchorage, biomechanics, and multidisciplinary planning, which raises doubts in the hierarchy of priorities and organization of the treatment plan. The Impact on the Quality of Life Caused by the Early Loss of Primary Molars

INTRODUCTION Evidence regarding the impact of early loss of primary molars (ELPM) on Oral Health-... more INTRODUCTION Evidence regarding the impact of early loss of primary molars (ELPM) on Oral Health-Related Quality of Life (OHRQoL) is lacking. The aim of the present study was evaluating the impact of ELPM on OHRQoL of Brazilian schoolchildren aged 6-10 years. METHODS This observational prospective cohort study was conducted with 163 schoolchildren, assigned in the primary tooth loss group (PTLG), in which all tooth loss was caused by carious lesions, and in a control group without tooth loss (CG). Two calibrated examiners conducted the clinical oral assessments to determine caries experience and tooth loss. OHRQoL was assessed using the Child Perception Questionnaire 8-10 years (CPQ8-10) instrument administered at baseline and at 10-12 months of follow-up. RESULTS Children in PTLG exhibited significantly more negative impact on OHRQoL at baseline (p <0.01) than CG, and also after the follow-up period (p <0.01). It was also possible to observe a significant reduction in the negative impact on OHRQoL in both CG and PTLG in the longitudinal analysis (p <0.01). Nevertheless, there was an even more substantial reduction in the negative impact on OHRQoL in children in PTLG. CONCLUSIONS This study provides evidence that early tooth loss is associated with negative impact on OHRQoL. Moreover, it indicates that access to dental treatment can have a positive impact on the OHRQoL of children with dental caries and ELPM.

OBJETIVOS: o objetivo desse estudo foi buscar na literatura informações consistentes sobre a qual... more OBJETIVOS: o objetivo desse estudo foi buscar na literatura informações consistentes sobre a qualidade de vida relacionada à saúde bucal, permitindo aos clínicos acessar e compreender sua influência no processo de busca e tratamento de seus pacientes. MÉTODOS: foram pesquisadas as bases de dados eletrônicas MEDLINE, LILACS, BBO e Cochrane Controlled Trials, entre 1980 e 2010. Foram encontrados 158 estudos que discutiam a qualidade de vida relacionada à saúde bucal. RESULTADOS: foram selecionados 30 estudos, sendo dois prospectivos longitudinais, duas revisões sistemáticas, cinco casos-controle, doze estudos epidemiológicos, cinco estudos transversais e três revisões de literatura, além da declaração da Organização Mundial da Saúde. Objetivos: Determinar la prevalencia de maloclusiones, su severidad y necesidad de tratamiento en... more Objetivos: Determinar la prevalencia de maloclusiones, su severidad y necesidad de tratamiento en adolescentes y adultos jóvenes entre 15 y 24 años del interior del Uruguay, así como su asociación con variables sociodemográficas, de calidad de vida e indicadores de riesgo. Método: Se realizó un estudio transversal utilizando datos originados del "Primer Relevamiento Nacional de Salud Bucal en población joven y adulta uruguaya", realizado entre 2010 y 2012. Análisis estadístico: Las diferencias de prevalencias entre categorías de covariables fueron testadas con test chiquadrado para heterogeneidad con linearización de Taylor. Resultados: La prevalencia de maloclusiones en la población de estudio fue de 33,8% en el periodo 2010-2012. Las maloclusiones presentaron asociación con la calidad de vida. Conclusiones: Estos resultados son los primeros con una muestra representativa en esta población, siendo de interés para gestores de salud pública y útiles para otros investigadores del área en futuros estudios

Orthodontic treatment needs of Brazilian 12-year-old school children

One experienced examiner evaluated the IOTN's dental health component (DHC) and its esthetic component (EC) in 1,182 individuals from 50 randomly selected state schools of Rio de Janeiro. He also requested that these students score their own EC (self-perception). The DHC showed that 51.1% of the children had no/little need, while 26.7% had a need/high need/priority for orthodontic treatment. There was no significant difference between the two sexes (P=.156). According to the EC, esthetics were good in 59.1% of the children, borderline in 32.7%, and unattractive in 8.1%. The self-perception of the EC showed that 89.0% judged themselves as having good esthetics, with girls significantly more critical than boys (P=.035). Background: Terminology that comes from the anthropometric index, which is indicative of the form... more Background: Terminology that comes from the anthropometric index, which is indicative of the form of the skullcap, is widely used to describe the face in orthodontics. Using this terminology assumes that the face and skull always follow the same pattern of morphology. The aim of this study was to test the hypothesis that the morphology of the face corresponds with the morphology of the skull. Methods: Measures related to the cranial and facial indexes were taken randomly from 51 dry human skulls with neutral occlusion, selected from a larger collection. Skulls were classified, according to the cranial index, as dolichocephalic (<76.0), mesocephalic (76.0e<81.0), or brachycephalic (!81.0) and according to the facial index as leptoprosopic (!90.0), mesoprosopic (85.0e<90.0), or euryprosopic (<85.0). The McNemar-Bowker test and linear regression models were used to assess the relationship between the cranial and facial morphologies. Results: The correspondence values between the cephalic and facial index ratings in expected cephalofacial pairs (brachycephalic and euryprosopic, mesocephalic and mesoprosopic, and dolichocephalic and leptoprosopic) were observed to be only 3.9%, 7.8%, and 13.7%, respectively. Coefficients of linear regression confirmed a weak influence of facial measurements on the morphology of the skull. Conclusions: Facial morphology exerts little influence on skull shape and, therefore, does not support the widespread use of a terminology derived from the cranial index. The development of a standardized diagnostic terminology is timely given the increasing use of electronic records in health research and facilitating data sharing across different areas.

These questionnaires assess not only the esthetic factor, but also the functional conditions that may be affected as well as the psychological issues related to self-esteem and sociability, all of which must be assessed in order to enable the development of an individual treatment plan that meets patient's expectations. MÉTODOS: a fotografia de uma mulher jovem sorrindo foi digitalmente modificada, produzindo alterações simétricas na altura gengival dos incisivos centrais e incisivos laterais, tornando o plano gengival progressivamente ascendente. Foi solicitado que os indivíduos escolhessem a foto mais agradável e, depois disso, o entrevistador questionava o indivíduo para verificar se o mesmo sabia o que estava sendo alterado na sequência de fotos, ou seja, se identificava as alterações feitas no plano gengival. The aim of the present work was to systematically review the literature and identify all peer-rev... more The aim of the present work was to systematically review the literature and identify all peer-reviewed papers dealing with etiological and risk factors associated with bruxism. Data sources: Data extraction was carried out according to the standard Cochrane systematic review methodology. The following databases were searched for randomized clinical trials (RCT), controlled clinical trials (CCT) or cohort studies: Cochrane Library, Medline, and Embase from 1980 to 2011. Unpublished literature was searched electronically using ClinicalTrials.gov. Data selection: The primary outcome was bruxism etiology. Studies should have a standardized method to assess bruxism. Data extraction: Screening of eligible studies, assessment of the methodological quality and data extraction were conducted independently and in duplicate. Two reviewers inspected the references using the same search strategy and then applied the same inclusion criteria to the selected studies. They used criteria for methodological quality that was previously described in the Cochrane Handbook. Among the 1247 related articles that were critically assessed, one randomized clinical trial, one controlled clinical trial and seven longitudinal studies were included in the critical appraisal. Of these studies, five were selected, but reported different outcomes. Data synthesis: There is convincing evidence that (sleep-related) bruxism can be induced by esophageal acidification and also that it has an important relationship with smoking in a dose-dependent manner. Disturbances in the central dopaminergic system are also implicated in the etiology of bruxism.

In this nonrandomized prospective study, we compared the effects of the surgery-first approach wi... more In this nonrandomized prospective study, we compared the effects of the surgery-first approach with conventional 2-jaw orthognathic surgery on skeletal Class III patients' oral health-related quality of life (OHRQoL), quality of the orthodontic outcome, and average treatment duration. The sample consisted of 16 patients with severe skeletal Class III malocclusion, who needed 2-jaw orthognathic surgery: 8 were treated with the surgery-first approach, and 8 were treated with the traditional orthodontic-surgical approach. OHRQoL was assessed by using the Orthognathic Quality of Life Questionnaire (OQLQ) and the Oral Health Impact Profile-short version (OHIP-14). OBJETIVOS: o objetivo desse estudo foi buscar na literatura informações consistentes sobre a qual... more OBJETIVOS: o objetivo desse estudo foi buscar na literatura informações consistentes sobre a qualidade de vida relacionada à saúde bucal, permitindo aos clínicos acessar e compreender sua influência no processo de busca e tratamento de seus pacientes. MÉTODOS: foram pesquisadas as bases de dados eletrônicas MEDLINE, LILACS, BBO e Cochrane Controlled Trials, entre 1980 e 2010. Foram encontrados 158 estudos que discutiam a qualidade de vida relacionada à saúde bucal. RESULTADOS: foram selecionados 30 estudos, sendo dois prospectivos longitudinais, duas revisões sistemáticas, cinco casos-controle, doze estudos epidemiológicos, cinco estudos transversais e três revisões de literatura, além da declaração da Organização Mundial da Saúde. Objetivos: Determinar la prevalencia de maloclusiones, su severidad y necesidad de tratamiento en... more Objetivos: Determinar la prevalencia de maloclusiones, su severidad y necesidad de tratamiento en adolescentes y adultos jóvenes entre 15 y 24 años del interior del Uruguay, así como su asociación con variables sociodemográficas, de calidad de vida e indicadores de riesgo. Método: Se realizó un estudio transversal utilizando datos originados del "Primer Relevamiento Nacional de Salud Bucal en población joven y adulta uruguaya", realizado entre 2010 y 2012. Análisis estadístico: Las diferencias de prevalencias entre categorías de covariables fueron testadas con test chiquadrado para heterogeneidad con linearización de Taylor. Resultados: La prevalencia de maloclusiones en la población de estudio fue de 33,8% en el periodo 2010-2012. Las maloclusiones presentaron asociación con la calidad de vida. Conclusiones: Estos resultados son los primeros con una muestra representativa en esta población, siendo de interés para gestores de salud pública y útiles para otros investigadores del área en futuros estudios

One experienced examiner evaluated the IOTN's dental health component (DHC) and its esthetic component (EC) in 1,182 individuals from 50 randomly selected state schools of Rio de Janeiro. He also requested that these students score their own EC (self-perception). The DHC showed that 51.1% of the children had no/little need, while 26.7% had a need/high need/priority for orthodontic treatment. There was no significant difference between the two sexes (P=.156). According to the EC, esthetics were good in 59.1% of the children, borderline in 32.7%, and unattractive in 8.1%. The self-perception of the EC showed that 89.0% judged themselves as having good esthetics, with girls significantly more critical than boys (P=.035). Cranial morphology and facial type: Is it appropriate to describe the face using skull terminology?

Background: Terminology that comes from the anthropometric index, which is indicative of the form... more Background: Terminology that comes from the anthropometric index, which is indicative of the form of the skullcap, is widely used to describe the face in orthodontics. Using this terminology assumes that the face and skull always follow the same pattern of morphology. The aim of this study was to test the hypothesis that the morphology of the face corresponds with the morphology of the skull. Methods: Measures related to the cranial and facial indexes were taken randomly from 51 dry human skulls with neutral occlusion, selected from a larger collection. Skulls were classified, according to the cranial index, as dolichocephalic (<76.0), mesocephalic (76.0e<81.0), or brachycephalic (!81.0) and according to the facial index as leptoprosopic (!90.0), mesoprosopic (85.0e<90.0), or euryprosopic (<85.0). The McNemar-Bowker test and linear regression models were used to assess the relationship between the cranial and facial morphologies. Results: The correspondence values between the cephalic and facial index ratings in expected cephalofacial pairs (brachycephalic and euryprosopic, mesocephalic and mesoprosopic, and dolichocephalic and leptoprosopic) were observed to be only 3.9%, 7.8%, and 13.7%, respectively. Coefficients of linear regression confirmed a weak influence of facial measurements on the morphology of the skull. Conclusions: Facial morphology exerts little influence on skull shape and, therefore, does not support the widespread use of a terminology derived from the cranial index. The development of a standardized diagnostic terminology is timely given the increasing use of electronic records in health research and facilitating data sharing across different areas.

The objective of this article is to demonstrate the importance of assessing the quality of life of these individuals by means of applying specific questionnaires before treatment onset in order to increase the success rate of orthosurgical treatment. These questionnaires assess not only the esthetic factor, but also the functional conditions that may be affected as well as the psychological issues related to self-esteem and sociability, all of which must be assessed in order to enable the development of an individual treatment plan that meets patient's expectations. MÉTODOS: a fotografia de uma mulher jovem sorrindo foi digitalmente modificada, produzindo alterações simétricas na altura gengival dos incisivos centrais e incisivos laterais, tornando o plano gengival progressivamente ascendente. Foi solicitado que os indivíduos escolhessem a foto mais agradável e, depois disso, o entrevistador questionava o indivíduo para verificar se o mesmo sabia o que estava sendo alterado na sequência de fotos, ou seja, se identificava as alterações feitas no plano gengival. The aim of the present work was to systematically review the literature and identify all peer-rev... more The aim of the present work was to systematically review the literature and identify all peer-reviewed papers dealing with etiological and risk factors associated with bruxism. Data sources: Data extraction was carried out according to the standard Cochrane systematic review methodology. The following databases were searched for randomized clinical trials (RCT), controlled clinical trials (CCT) or cohort studies: Cochrane Library, Medline, and Embase from 1980 to 2011. Unpublished literature was searched electronically using ClinicalTrials.gov. Data selection: The primary outcome was bruxism etiology. Studies should have a standardized method to assess bruxism. Data extraction: Screening of eligible studies, assessment of the methodological quality and data extraction were conducted independently and in duplicate. Two reviewers inspected the references using the same search strategy and then applied the same inclusion criteria to the selected studies. They used criteria for methodological quality that was previously described in the Cochrane Handbook. Among the 1247 related articles that were critically assessed, one randomized clinical trial, one controlled clinical trial and seven longitudinal studies were included in the critical appraisal. Of these studies, five were selected, but reported different outcomes. Data synthesis: There is convincing evidence that (sleep-related) bruxism can be induced by esophageal acidification and also that it has an important relationship with smoking in a dose-dependent manner. Disturbances in the central dopaminergic system are also implicated in the etiology of bruxism.
