The hearing aids are one of the options used in auditory rehabilitation with the objective to imp... more The hearing aids are one of the options used in auditory rehabilitation with the objective to improve communication and to minimize the limitations caused by hearing loss. Despite the development of technology, the selection and fitting processes demand specific procedures which goal is to maximize performance, benefit and user' satisfaction. To describe the hearing aid selecting and fitting protocol for adults and the elderly in an audiological center, which development was based on previously described and validated protocols from the specific literature and information based on clinical practice. The Hearing Aid Selecting and Fitting Protocol for adults and elder subjects showed that a greater amount of collected information provides better expectations for the patients' real life and greater effectiveness in counseling about hearing aids use, promoting maximum auditory performance, satisfaction and benefit to users.

Purpose: 1) To measure speech understanding in noise with the Naída Q70 in the omnidirectional mi... more Purpose: 1) To measure speech understanding in noise with the Naída Q70 in the omnidirectional microphone mode (T-Mic) and adaptive directional microphone mode (UltraZoom) in reverberating acoustics and noisy conditions. 2) To measure improvement in speech understanding with use of the Advanced Bionics (AB) Naída Q70 sound processor for existing Harmony users. Methods: Seven adult unilateral cochlear implant (CI) recipients, who were experienced users of the Harmony sound processor, participated in the study. Sentence recognition was evaluated in quiet in a reverberating room, with Harmony and Naída CI Q70 processors. Effectiveness of Naída CI Q70's UltraZoom directional microphone was evaluated in noise. Target stimuli were recorded Portuguese sentences presented from 0° azimuth. Twenty-talker babble was presented at +5dB SNR from ±90° azimuth. In addition to sentence recognition, the participants also rated the clarity of sound and difficulty of listening in the various test conditions. In order to evaluate the outcomes under more realistic acoustic conditions, tests were conducted in a non-sound treated reverberant room (RT60 of 553 ms and noise floor of 42.7 dBA (Leq). Results: The average sentence recognition in quiet in the reverberant non-sound treated room was 38.5% with the Harmony and 66.5% with Naída CI Q70. The average sentence recognition score in noise was 40.5% with Naída CI Q70 without UltraZoom and 64.5% with UltraZoom. For subjective ratings of sound clarity and listening ease in noise no difference were identified between the test conditions. Conclusion: For experienced users of the Harmony sound processor, speech understanding in quiet in a reverberating room was significantly improved with the Naída CI Q70. The use of an adaptive directional microphone technology (UltraZoom) enhanced speech perception in noise.

For the research, 19 users of an Auditory Brainstem Implant answered the following questionnaires... more For the research, 19 users of an Auditory Brainstem Implant answered the following questionnaires: KINDLR (Questionnaire for Measuring Health-Related Quality of Life in Children and Adolescents), for children and adolescents, their parents and/or caregivers; WHOQOL-BREF questionnaire, for adult participants; and the Satisfaction with Amplification in Daily Life (SADL) questionnaire culturally adapted to Brazilian Portuguese. Results: The quality of life of children using Auditory Brainstem Implant from the perspective of their parents showed global results above average, as for most domains, except for the emotional well-being domain. Adults showed results above average for all domains. Regarding satisfaction with the device, the adult users of auditory brainstem implant were satisfied in general, except with regard to personal image. The parents of the children showed dissatisfaction in all subscales, except for the subscale of services and cost. Conclusion: The results indicated that although patients are dissatisfied with the device in some aspects, overall the quality of life was rated as good for most of the aspects assessed. Objetivo: Avaliar a satisfação e qualidade de vida em usuários de Implante Auditivo de Tronco Cerebral.

A prótese auditiva é uma das alternativas utilizadas no processo de reabilitação auditiva que vis... more A prótese auditiva é uma das alternativas utilizadas no processo de reabilitação auditiva que visa auxiliar a comunicação e minimizar as limitações causadas pela deficiência auditiva. Mesmo com o avanço da tecnologia, a seleção e adaptação da prótese auditiva exigem um processo criterioso para maximizar o desempenho, benefício e satisfação do usuário. Descrever o protocolo de seleção e adaptação de prótese auditiva para candidatos adultos e idosos utilizado em um centro de audiologia, cuja elaboração foi embasada em protocolos validados e descritos previamente na literatura científica específica e complementada por informações de evidências clínicas do serviço. Comentários Finais: O protocolo fonoaudiológico de seleção e adaptação da prótese auditiva para adultos e idosos demonstra que um maior número de informações coletadas propicia melhor orientação da expectativa real e maior efetividade no aconselhamento ao uso da prótese auditiva, favorecendo, desta maneira, o desempenho auditivo, a satisfação e o benefício do indivíduo. Palavras-chave: audição, perda auditiva, auxiliares de audição, adulto, protocolos.

A Europa, mergulhada na carnificina da l' Guerra Mundial, sofre os horrores da grande batalha, travada com o fim de acabar com todas as guerras. Enquanto isso, nas terras distantes deste continente austral, Arnaldo Vieira de Carvalho prossegue em seu intuito de consolidar a nova Faculdade de Medicina. Para atingir este objetivo, tem feito contatos com renomados mestres das diferentes áreas médicas, sempre buscando aglutiná-Ios a fim de atingir o nobre ideal. Nesse sentido, chama sua atenção um jovem médico maranhense de nome João Britto. Este, formado na Faculdade de Medicina do Rio de Janeiro, havia feito seus estudos de especialização em Oftalmologia junto aos grandes Mestres europeus, nas Clínicas de Viena e Berlim. J. Britto, como ficaria conhecido, aceita o convite de Arnaldo e assume como primeiro professor da Cadeira de Oftalmologia.

Residual Hearing Preservation with the Evo® Cochlear Implant Electrode Array: Preliminary Results

Introduction The preservation of residual hearing is currently an important challenge for cochlea... more Introduction The preservation of residual hearing is currently an important challenge for cochlear implant surgeries. Indeed, if patients exhibit functional hearing after cochlear implantation, they can benefit from the combination of acoustical stimulation, usually in the low-frequencies and electrical stimulation in the high-frequencies. This combined mode of stimulation has proven to be beneficial both in terms of speech perception and of sound quality. Finding the right procedures for conducting softsurgeries and designing electrode arrays dedicated to hearing preservation is an open issue. Objective The objective of this study is to evaluate the combination of a soft-surgery procedure implicating round-window insertion and the use of dexamethasone and hyaluronic acid during surgery, with the use of a specifically designed straight soft electrode array, on hearing preservation in patients with functional hearing in the low frequencies. Methods This pre-clinical trial was conducted on seven patients with residual hearing in the low frequencies. The surgical method used employed a round window insertion and the use of topical dexamethasone. Results The soft-surgery protocol could be successfully followed in five patients. In this group, the average hearing threshold shift compared with pre-operative values was of 18.7 þ/À 16.1 dB HL up to 500 Hz and 15.7 þ/À 15.1 up to 1 kHz, demonstrating satisfying levels of hearing preservation. Conclusion We were able to demonstrate the possibility of preserving residual hearing in most of the patients using the EVO electrode. Significant residual hearing preservation levels were was obtained when a soft surgical approach involving round window insertion, dexamethasone and hyaluronic use during the surgery.

One ofthese involves the precise nature ofsurgical intervention once the decision to operate has been made. Between Jun e 1, 1984, and June 30, 1993, we surgically treated 220 cases of traumatic facial paralysis with good cochlear reserve by decompressing the tympanic and mastoid segments via a transmastoid approach fo llowed by decompression ofthe geniculate ganglion and the distal halfofthe labyrin thine segment via a middlef ossa approach. We discuss the results of surgery via the middleJossa approach, and we review the literature.

The use of transcutaneous device has increased, given its aesthetic appeal, the complications and limitations of percutaneous devices, and patient's treatment adherence, focusing mainly on efficacy. However, various complications are associated with the use of transcutaneous prosthesis, which can often be serious. Objective To approach the literature on complications involving transcutaneous bone-anchored prostheses through a systematic review of articles published in the past 10 years (2011–2021). Data Synthesis The MEDLINE, EMBASE, Scopus, and Cochrane Library databases were searched. The use of transcutaneous device has increased, given its aesthetic appeal, the complications and limitations of percutaneous devices, and patient's treatment adherence, focusing mainly on efficacy. However, various complications are associated with the use of transcutaneous prosthesis, which can often be serious. Objective To approach the literature on complications involving transcutaneous bone-anchored prostheses through a systematic review of articles published in the past 10 years (2011–2021). Data Synthesis The MEDLINE, EMBASE, Scopus, and Cochrane Library databases were searched.
