Susan Simpson is a Clinical Psychologist and researcher based in Scotland. She has conducted extensive research into patient and therapist experience of videoconferencing-based psychotherapy. In addition, her research has focused on the treatment of complex eating disorders with Schema Therapy. She is adjunct academic at the University of South Australia and is now Professional Lead for Applied Psychology Service in Regional Eating Disorders Service, Livingston, Scotland.

The use of videoconferencing in the provision of psychotherapy is a relatively new one, although ... more The use of videoconferencing in the provision of psychotherapy is a relatively new one, although it dates back to before developments in email and Internet chat. Historically, however, videoconferencing has been used for commercial, educational and medicinal purposes over the past 40 years or so. Early developments in videoconferencing grew out of a history of rapid growth in electronic forms of communication, which were originally analogue, and most recently digital. In the initial stages, the growth of videoconferencing communications was driven by commercial organizations, such as NASA (National Aeronautics and Space Administration) in the USA. It only became used for telemedicine when individual practitioners began to utilize commercial-based equipment that was already available.

However, empathy that is delivered in ways that are misaligned with the client’s clinical presentation and emotional states or ‘modes’ can have the opposite effect, reinforcing unhelpful coping patterns, and hindering recovery. In this theoretical development paper, it is proposed that ‘pseudo vulnerability’ is an important yet overlooked source of therapeutic impasses, often resulting in unremitting clinical problems, and poor clinical outcomes. A range of commonly occurring pseudo vulnerable presentations are described, including Complaining Protector, Attention/Recognition Seeking, and Self-Pity/Victim, with the addition of a new mode Helpless Surrenderer. There is an urgent need for research into transdiagnostic treatment models that address comorbidity, particularly rigid personality traits and disorders, alongside eating disorder pathology. Schema therapy may be ideally suited due to its growing evidence base both for eating disorders and the comorbidities that often interfere with outcomes with this population.

The current roll-out of broadband across Australia provides new opportunities for psychologists t... more The current roll-out of broadband across Australia provides new opportunities for psychologists to reach out to residents in remote and rural areas. With the advent of internet protocol (IP)-based videoconferencing, telepsychology is becoming more accessible and affordable, and a potential solution to those in remote areas who require access to specialist assessment and psychotherapy services. Telepsychology represents a highquality/low-cost model of service delivery. Findings to date suggest that outcome might be equivalent to in-person therapy across a range of client groups both using standardised assessments and evidence-based therapies. Studies have consistently shown that clients rate high levels of satisfaction and therapeutic rapport. However, uptake among psychologists has been slow. This article provides a synthesis of the gradual growth of telepsychology in Australia. It identifies factors that hinder the development of telepsychology and outlines the main arguments for developing this as a model for the provision of psychology across remote and rural Australia. The authors draw on their own recent experiences in developing collaborative telepsychology services to make recommendations for the future of telepsychology in Australia.

Therapy via videoconferencing: a route to client empowerment?

This study examines the clinical utility of cognitive behavioural therapy delivered via videoconf... more This study examines the clinical utility of cognitive behavioural therapy delivered via videoconferencing for bulimic disorders, and factors associated with adjustment to this mode of treatment delivery. A single-case-series design was used with six participants with bulimic disorders. Most were living in remote areas in northeast Scotland or Shetland. Therapy sessions were conducted weekly at a bandwidth of ISDN 6 (384 kbytes/sec) via videoconferencing links between local community hospital sites and the eating disorders service in Aberdeen, Scotland. Almost all participants rated high levels of therapeutic alliance and satisfaction with video therapy. Some participants preferred video therapy due to feeling less selfconscious and intimidated, whereas others felt it was less personal than face-to-face sessions would be. It is proposed that video therapy may be particularly suited to the treatment of eating disorders, especially for those with high levels of shame and body-related selfconsciousness and those who require greater levels of control in therapy.
