Mitochondrial DNA (mtDNA) gene defects may play a role in the development of maternally inherited... more Mitochondrial DNA (mtDNA) gene defects may play a role in the development of maternally inherited diabetes mellitus and deafness (MIDD). Mutation in the mitochondrial gene at position 3243 was recently identified in several pedigrees of diabetes mellitus and deafness. As the mitochondria play an important role in glucose-stimulated insulin secretion in pancreatic beta-cells, we therefore searched for this mutation in a non-consanguineous family with MIDD from Southern Italy. The mitochondrial 3243 bp mutation of the tRNALEU (UUR) gene was identified in one subject with deafness and Type 1B diabetes mellitus and in his mother. The mother was affected by Type 2 diabetes mellitus, deafness and cardiomyopathy. Our study points out the variable phenotypic expression of this mitochondrial mutation. This may suggest the presence of other mitochondrial or nuclear mutations required to modulate the phenotype.

In the present work the sialic acid content of the red blood cell ghost membranes was measured in... more In the present work the sialic acid content of the red blood cell ghost membranes was measured in blood specimens obtained from 18 patients with beta-thalassemia major and 10 controls. The sialic acid-protein weight ratio was reduced in comparison with normal mean value only in four patients who had undergone splenectomy several years ago. The remaining 14 unsplenectomized patients showed a sialic acid/protein ratio that was 50% higher than normal (P less than 0.001). The titratable sulphydryl groups were also measured in red blood cell membrane preparations. SH groups content was reduced in patients with beta-thalassemia major, and a dramatic decrease (-60%) was observed in patients after splenectomy.

Introduction: Systemic fungal disease, by Candida species, are a much feared complication in NICU... more Introduction: Systemic fungal disease, by Candida species, are a much feared complication in NICU. Objectives: We evaluated the higher incidence of Candida A. infection and not particularly in VLBW and ELBW infants. Methods: Infection cases by Candida albicans and not recorded in NICU of Messina. The cases were reported 42/1839 (2%) 21 premature infants , G. A. 30.5± 3.78 , weight 1550.4 ± 906.9, 14 term infants , G.A. 38.42 ± 1:45,weight 3128.9 ± 290.8 , 7 pediatric patients. The fungus most frequently isolated was Candida parapsilosis (19/42-46%), followed by C. albicans (13/42 -32%), by C. lambica (5/42 -12%), C. tropicalis (2/42 -4%), C.zeylanoides (1/42 -2%), C.krusei (1/42 -2%), C.guillermondi (1/42-2%). Of the 15 preterm with GA ≤ 32, in 8 / 15 (53%) was isolated C.parapsilosis, in 4 / 15 (27%) C. albicans ,in 2 / 15 (13%) C.lambica, in one case (1 / 15 -7%) C.zeylanoides. About patients studied 7 / 42 died (17%) which 6 / 21 preterm infants ( ), 1 / 7 pediatric patients (14%), no term baby died. We have registered two cases of mycetoma in a preterm baby. The increased incidence of Candida parapsilosis is a main cause of infections, linked to fungal adherence ability to prosthetic materials and to proliferate in presence of high glucose concentrations , especially in preterm infants in parenteral nutrition. Our study shown the problem of colonization,the selection of resistant strains of Candida in NICU and its new therapeutic options.

Data comparing the effectiveness of high-frequency oscillatory ventilation and of conventional me... more Data comparing the effectiveness of high-frequency oscillatory ventilation and of conventional mechanical ventilation in the treatment of respiratory distress syndrome of very low birth weight infants are, to date, still matter of debate. We investigated the effects of first intention high-frequency oscillatory ventilation or conventional mechanical ventilation support on selected primary and secondary outcomes in very low birth weight infants complicated by respiratory distress syndrome in which antenatal glucocorticoid prophylaxis was not performed. Design: Multicenter randomized control trial. Setting: Three tertiary centers of neonatal intensive care units from December 2004 to December 2007. Population: Eighty-eight very low birth weight infants complicated by respiratory distress syndrome, without antenatal glucocorticoids, supported by first intention high-frequency oscillatory ventilation (n ؍ 44) or conventional mechanical ventilation (n ؍ 44). Interventions: All newborns were monitored by standard monitoring procedure, including routine laboratory variables, neurologic patterns, and ultrasound imaging. Primary outcomes were: the length of ventilatory support, the need of reintubation, and the length of nasal continuous positive airway pressure support in the postextubation period. Secondary outcomes were: the length of stay in neonatal intensive care unit and in hospital, death before discharge, adverse short-and long-term pulmonary and neonatal outcomes, and the need for a second dose of surfactant and of postnatal glucocorticoid treatment. Results: High-frequency oscillatory ventilation infants showed a significant lower duration (p < .001 for all) of ventilator dependency, lower need of reintubation and of duration of nasal continuous positive airway pressure support in the postextubation period. Among secondary outcomes in the high-frequency oscillatory ventilation infants, the need of a second dose of surfactant administration, and the length of stay in the neonatal intensive care unit and in hospital were significantly lower (p < .05 for all). Conclusions: We found that high-frequency oscillatory ventilation in very low birth weight infants without antenatal glucocorticoid prophylaxis reduced the need of ventilatory support, surfactant therapy, and reintubation, and shortened neonatal intensive care unit and hospital stay, thus reducing unit and hospital costs. These data would support the usefulness of first intention high-frequency oscillatory ventilation strategy in managing in a selected population, such as very low birth weight newborns complicated by severe respiratory distress syndrome not antenatally treated with glucocorticoids.

We studied the tolerance and pharmacokinetic parameters of ciprofloxacin in (2 mg/kg, i.v.) in 45... more We studied the tolerance and pharmacokinetic parameters of ciprofloxacin in (2 mg/kg, i.v.) in 45 chicks. Serum concentrations of ciprofloxacin at various times of drawing (15 min, 30 min, 1 hr, 3 hr, 6 hr, 8 hr and 12 hr) peaked in the first 15 min with a trough after 6 hr, and were completely absent after 8 hr. We found no neurotoxic effects or behavioral changes. Encouraged by a good tolerance for this quinolone, we conducted a preliminary study concerning the therapeutic efficacy of ciprofloxacin in 8 subjects of pediatric age (8-14 years) with urinary tract infections (UTI) that are resistant to classic antibiotics. The results and normality of hematologic parameters, checked before and after therapy, allow us to recommend the monitored use of ciprofloxacin in children with UTI resulting from bacteria resistant to other antibiotics.

A two-cohort vaccination strategy was adopted to minimise the shift of the mean age of varicella occurrence to older age groups, with the goal of vaccinating with one dose at least 80% of children in their second year of life and 50% of susceptible adolescents in their 12th year of life. Two studies were implemented in parallel to closely monitor vaccination coverage as well as varicella incidence. Overall, the programme achieved its target, with 87.5% vaccine coverage for the birth cohort 2005 and 90.2% for adolescents born in 1995 and 1996. In fact, major improvements in prenatal evaluation and neonatal management techniques have greatly increased the possibility of survival for infants at risk, especially those born from multiple pregnancies. Our study reports the results of a retrospective survey for the years 1990 to 1993, on the outcome of 76 twin infants (ie. 11,4% of the 661 newborns admitted to our unit over that period) correlating the clincial symptoms observed with the modality of birth.
