The worldwide shortage of liver grafts causes medical, social and ethical problems. Although this... more The worldwide shortage of liver grafts causes medical, social and ethical problems. Although this situation requires intensive efforts to develop alternative grafts using regenerative biotechnology, they are still far away from the practical application. Now, we have the strategy to develop regenerative chimeric liver grafts containing human hepatocytes and the scaffold of xenogeneic animals such as transgenic pigs. Using our established rodent model that the chimeric livers with rat hepatocytes and mouse scaffold are transplanted to rat recipients, we evaluated their long-term function after transplantation and outcomes in graft and recipient survivals under immunosuppressive treatments. We produced chimeric livers in urokinase-type plasminogen activator/severe combined immunodeficiency (uPA/SCID) mouse by cell transplantation with hepatocyte isolated from luciferase transgenic Lewis (LEW) rat (MHC haplotype: RT1 l), and transplanted them to wild-type LEW rats (RT1 l) and Nagase analbuminemia rats (NAR) (RT1 a) in the auxiliary fashion, followed by daily tacrolimus administration. We evaluated the intensity of luminescence derived from the transplanted chimeric liver as a marker of its viability and examined serum rat albumin level in NAR recipients using ELISA. Furthermore, we evaluated blood circulation in chimeric livers using Doppler ultrasonography. In vivo bioluminescent imaging showed that tacrolimus monotherapy improved graft survivals for four weeks after transplantation, suggesting the controllability of the rejection toward transplanted chimeric livers. In NAR recipients, chimeric livers produced rat albumin after transplantation, and we could detect it even 170 days after transplantation. Doppler ultrasonography indicated the arterial inflow and venous outflow in chimeric livers even six months after transplantation. These results suggest that, with appropriate immunosuppression, chimeric livers may work as auxiliary livers for a long-period and our strategy can contribute to solve the critical graft shortage in the world.

A qualitative thematic discourse analysis has been achieved. The experience of falls and the fear of falling are very variable. The fear of falling is not always explicit. It is linked to the subjective interpretation of the fall, to its long-term impact (social, functional, and psychological changes), to the identity modifications, to the awareness of aging, to the fear of dependency and possible placing in an home for the aged. The data of this rsearch contribute to a better understanding of the fear of falling and open new perspectives for its evaluation for planning preventive interventions.

Semi-structured interviews were conducted in 30 patients (mean age = 53±10) after their inclusion on the waiting-list (Gr. A). Follow-up interviews were performed 6 months later in 15 patients still awaiting KT (Gr. B6), and in 15 patients 6 months (Gr. C6) and 12 months (Gr. C12) after KT. Qualitative analysis was performed. Gr: A:All patients underlined loss of freedom, 87% devoted much energy to maintain normality, 57% modified medical directives. All reported emotional fragility related to dialysis and loss of quality of life (QOL), negative (43%) or suicidal thoughts (20%). Professional stigma was underlined (26%). Gr: B6:40% reported no change, 60% mentioned increase of illness intrusiveness, 46% dialysis side-effects, 40% communication problems, 33% tension with medical staff and waiting list handling. It is not always clear how these are to be defined but the authors do acknowledge this difficulty. This is a useful book for references, particularly for the borderland areas. Reflecting its encyclopaedic style there are many useful tables and especially for the lexicologist there is a table of compulsions and phobias which include such delights as 'apeirophobia' and 'catagelphobia'. Authors must be prone to kakorraphobia but apart from minor criticisms they need have no fear!

BACKGROUND: With the increase of the organ shortage, several authors assume that financial incent... more BACKGROUND: With the increase of the organ shortage, several authors assume that financial incentives would improve organ donation rates and fill the unbalance between the demand and the organs available for transplant medicine. This line of argument has been criticised with people arguing that an exchange of money for organs would violate the legal and the ethical principle of gratuity, decrease voluntarism and increase the body parts commodification phenomena. PURPOSE: Switzerland is often highlighted as having under-average organ donation rates compared to other European countries. In this paper we investigate the opinions of the Vaud French-speaking population concerning direct, indirect and non financial incentives in order to assess their opinions and anticipate the further debate. METHODS: As part of a broad survey about the organ donation decision-making process, questions about incentives for both living and deceased organ donation were addressed to Vaud inhabitants and physicians. RESULTS: The data collected showed that respondents were opposed to rewarding both living and deceased organ donation. The analysis of positive answers showed that indirect and non financial incentives were the most likely choice to acknowledge the donor's act. People in critical financial situations preferred direct financial incentives. CONCLUSIONS: These results showed that altruism and gratuity were key-values in the organ donation and transplantation social perceptions of the respondents. Thus, we assume that introducing financial incentives could tarnish the image of transplant medicine. Nevertheless, further studies are needed to evaluate if their introduction would improve the organ donation rates or decrease voluntarism.

Background: This study explores significant ones' implication before and after transplantatio... more Background: This study explores significant ones' implication before and after transplantation. Methods: Longitudinal semi-structured interviews were conducted in 64 patients awaiting all-organ transplantation. Among them, 58 patients spontaneously discussed the importance of their significant other in their daily support. Discourse analysis was applied. Findings: During the pre-transplantation period renal patients reported that significant others took part in dialysis treatment and participated to regimen adherence. After transplantation, quality of life improved and the couple dynamics returned to normal. Patients awaiting lung or heart transplantation were more heavily impaired. Significant others had to take over abandoned roles. After transplantation resuming normal life became gradually possible, but after one year either transplantation health benefits relieved physical, emotional and social loads, or complications maintained the level of stress on significant others. Discussion: Patients reported that significant others had to take over various responsibilities and were concerned about long-term stress that should be adequately supported.
