De nos jours, la notion de réseau occupe une place cruciale dans le système de santé québécois et... more De nos jours, la notion de réseau occupe une place cruciale dans le système de santé québécois et dans les recherches sociales en santé. Il s’agit à la fois d’un mode d’organisation de services sanitaires et communautaires, d’une méthode d’investigation de ces services et d’une théorie plus générale d’appréhension du monde social. Le présent ouvrage fournit des exemples pratiques de la manière dont la notion de réseau est mobilisée, au Québec, pour mieux comprendre les processus à l’œuvre dans le domaine de la santé. Pour ce faire, il rassemble des auteurs d’horizons disciplinaires variés. Sociologues, psychologues, anthropologues, chercheurs en travail social, en administration publique, spécialistes en évaluation de programme y présentent leur usage spécifique du concept de réseau comme méthode de recherche, inspiration théorique ou objet d’étude. L’ensemble de ces contributions offre ainsi le premier ouvrage francophone en la matière. Il sera utile aussi bien aux étudiants cherchant à se familiariser avec « l’approche réseau » qu’aux chercheurs plus expérimentés souhaitant se renseigner sur les recherches contemporaines dans ce domaine, aux professionnels de la santé, du travail social et du communautaire à l’affût de pistes d’innovations sociales, ainsi qu’aux citoyens s’interrogeant sur le monde de la santé aujourd’hui.

Comparison of Midwifery Care to Medical Care in Hospitals in the Quebec Pilot Projects Study: Clinical Indicators

ABSTRACT The purpose of this study was to compare indicators of process and outcome of midwifery ... more ABSTRACT The purpose of this study was to compare indicators of process and outcome of midwifery services provided in the Quebec pilot projects to those associated with standard hospital-based medical services. Women receiving each type of care (961 per group) were matched on the basis of socio-demographic characteristics and level of obstetrical risk. We found midwifery care to be associated with less obstetrical intervention and a reduction in selected indicators of maternal morbidity (caesarean section and severe perineal injury). For neonatal outcome indicators, midwifery care was associated with a mixture of benefits and risks: fewer babies with preterm birth and low birthweight, but a trend toward a higher stillbirth ratio and more frequent requirement for neonatal resuscitation. The study design does not permit to conclude that the associations were causal in nature. However, the high stillbirth rate observed in the group of women who were selected for midwife care raises concerns both regarding the appropriateness of the screening procedures for admission to such care and regarding the quality of care itself.

Whereas group prenatal education is the most common educational model, some health institutions have opted to implement online prenatal education to address accessibility issues as well as the evolving needs of future parents. Various studies have shown that prenatal education can be effective in acquisition of knowledge on labour and delivery, reducing psychological distress and maximising father's involvement. However, these results may depend on educational material, organization, format and content. The objective of this paper is to raise questions about appropriateness of the concept of continu... more The objective of this paper is to raise questions about appropriateness of the concept of continuity of care for guiding the development of helping strategies for people with mental health problems in the era of non-institutionalization. Research on continuity of care is reviewed to show the strong legacy of institutional thinking. A leading problem addressed in this literature is that of client drop-out from programs designed to ensure continuity. The research results clearly suggest that this “problem” lies with the agency of clients: the fact that, in the natural milieu, they make decisions according to their own definitions of the situation and in a social context of their own. This perspective, however, seemingly cannot be incorporated into the continuity of care paradigm, which remains firmly grounded in the world of service providers.
