Skin test gave an overall positivity of 85% and determination of specific IgE of 42%. Skin tests have a greater sensitivity but "in vitro" tests are an useful associated diagnostic method.

In the last few years the consensus on asthma, guidelines and GINA have introduced a classificati... more In the last few years the consensus on asthma, guidelines and GINA have introduced a classification based on clinical criteria, PFR or FEV1 values and reversibility. However the relation of clinical classification with the functional data is not well defined. To correlate the clinical evaluation of asthma with the most usual parameters of larger and smaller airways bronchial obstruction, PFR and FEV1.n 153 patients have been studied by spirometry on absence of bronchodilator or anti inflammatory therapy in the last few days. Clinically, they were classified following the 2002 revised strategy for asthma management (NIH), 66 as mild intermitent, 61 mild persistent and 26 moderate persistent asthmas. In mild intermitent asthma PFR was decreased in 53 patients (80.3%) and FEV1 in 51 (77.2%). In mild persistent PFR and FEV1 were decreased in 59 (96.7%). Delayed hypersensitivity to Candida antigens was observed only in 17 of the patients. 12 patients had also chronic urticaria. A defect in NK cells was found in 18 out of 23 cases (78%). Treatment with Candida vaccines had obtained good clinical results in 11 of the first 13 patients treated.

Beta-adrenergic agents have been used for a long time in the treatment of asthma. For the purpose... more Beta-adrenergic agents have been used for a long time in the treatment of asthma. For the purpose of bronchodilation the better results would be attained with the increase in Beta-2-selectivity. From the newer Beta-agonists the mot currently used are TERBUTALINE, FENOTEROL, SALBUTAMOL, CLEMBUTEROL, TOLBUTEROL, CARBUTEROL, PROCATEROL, RIMITEROL and REPROTEROL, this last combining in its molecule the structure of a beta-agonist with a Xanthine group. These agents could be used in different ways, by mouth, injection and inhalation (with a exception of Clembuterol which is effective only by oral route). The authors have, some years ago, comparatively studied the bronchodilating effect of Salbutamol and Fenoterol including 18 patients. The main increase of PFR was slightly higher after FENOTEROL but this difference was not significant. The authors have studied REPROTEROL by inhalation and oral routes in 11 asthmatic patients. For others NSAIDS including the newer coxibs positivity around 50%. Nimesulid and acetaminophen with a lower of positivity. Skin tests can be used in NSAIDS hypersensitivity to confirm clinical history but also to choose, case by case, a drug less prone to give hypersentivity reactions.

RATIONALE: Antihelmintics are widely used in tropical areas, but also prescribed in Western count... more RATIONALE: Antihelmintics are widely used in tropical areas, but also prescribed in Western countries. Allergic reactions to these drugs are rare. METHODS: A 52 year-old woman suffering from acute parasitic infection took the first dose of mebendazole (100 mg) and 2 hours later developed acute generalized urticaria. Five months later the patient was treated with albendazole (400 mg) and 10 hours later developed a few small, localized urticarial lesions in thorax. After obtaining written informed consent, we performed skin test (SPT) and intradermal tests (IDT) and simple-blind oral challenge test with mebendazole and albendazole. RESULTS: SPT and IDT (0.1 -0.01 mg/ml) were negative with mebendazole. Oral challenge with mebendazole gave a positive reaction after 90 minutes of taking 75 mg; the patient developed itching and wheal flares located on her forearms. She was treated with intramuscular methylprednisolone and dexchlorpheniramine improving in 1-2 hours. SPT and IDT (0.1 -0.01 mg/ml) were negative with albendazole. Seven hours after taking 400 mg, the patient developed identical lesions in trunk. The patient was treated as previously described and recovered within 1hour. We wanted to confirm the absence of cross-reactivity between mebendazole, albenzole and other benzimidazole derivatives, but the patient refused to continue the study. CONCLUSIONS: To our knowledge, this is the first case of immediate hypersensitivity to mebendazole and albendazole documented up to date. In our case, the clinical history, physical examination and provocation tests allow us to establish the diagnosis of hypersensitivity to mebendazole with cross reactivity with albendazole. We could not exclude cross reactivity with other benzimidazole derivatives.
