Many smaller lesions may even go unnoticed due to minimal clinical symptoms. Clinical suspicion and judicious use of imaging may reveal lesions at an uncommon location. Histological confirmation, however, is the basis of the appropriate diagnosis. We describe two cases of lipoma at uncommon sites with the relevant description of both cases managed by surgical excision and histological confirmation. One case of a mid-palmar lipoma in a 42-year-old female and a lipoma adjacent to the right thumb base near the first web space in a 29-year-old male patient are described. The diagnosis was confirmed on histopathology in both cases. The excision biopsy resulted in complete removal; no recurrence or complication was noted in the follow-up of 6 and 8 months, respectively. Our case report highlights the importance of the early diagnosis and excision before involving adjacent vital structures or growing larger. Clinical suspicion and judicious use of imaging are important for the appropriate diagnosis at an uncommon location.

Despite being called tropical pyomyositis, these cases are frequently reported from both tropical and non-tropical regions. The most common causative organism is Staphylococcus aureus with anecdotal reports of a variety of muscles involved throughout the human body. The isolated and focal affliction of muscles is uncommon and more so in children. Case Report: We report a case of isolated pyomyositis involving inter-osseous muscles of the first web space of the left hand in a 5-year-old otherwise healthy male child. Conclusion: The good delineation of the lesion on magnetic resonance imaging was followed by aspiration of the abscess, leading to uneventful healing and good clinical outcome.

Background: The fracture of iliac bone or blade is uncommon injury and usually a part of pelvic r... more Background: The fracture of iliac bone or blade is uncommon injury and usually a part of pelvic ring disruption with varied clinical presentation. Isolated iliac bone fractures are rare, with literature limited to few reports or small series. Materials and Methods: Retrospective data of consecutive cases of isolated iliac bone fractures were analyzed during the period of July 2014 to August 2019. Relevant patient demographics, mode of injury, pattern of fracture, associated adjacent regional or remote injuries were noted for each case. Details of previous treatment and that of treatment given by us were also noted for each case. Results: A total of 18 cases with isolated iliac bone fracture of the pelvic bone were noted. The mean age was 31.2 years (range: 22–54 years). All the cases were male, and fall from height was etiology in 12 cases, followed by road traffic accidents in the rest. Left- and right-side involvement was noted in 11 and 7 cases each. The fracture pattern was minimally displaced simple in 13 and comminuted in 5 cases. The conservative management was done in all cases. The radiological union and clinical stability of fractures were assessed as end points. The fracture united in all cases within 8–9 weeks and without any immediate or remote complication. Level of evidence - Level 5, Observational Study. Conclusion: The uncommon isolated iliac wing fractures require better recognition and documentation for its management. Further studies should provide a better understanding of these injuries and treatment guidelines.

Undescribed fracture patterns are rare presentations in fracture clinics and are mostly limited t... more Undescribed fracture patterns are rare presentations in fracture clinics and are mostly limited to a few reports in the literature if any. The pediatric diaphyseal fractures of the tibia usually follow common described patterns, such as transverse, spiral, oblique, or comminuted configuration. Depression fractures, though well described at the articular region such as tibial plateau, are not found at diaphyseal sites. The localized saucer-shaped depression fracture in the diaphysis thus is an undescribed injury. We present a rare report of an isolated, unicortical, and localized depression fracture of the tibial diaphysis in a 10-year-old child following hit by a cricket ball during the match. He was managed conservatively with good functional outcome. Knowledge of this rare pattern of injury is helpful to acknowledge its occurrence and its listing in scientific literature.

Trauma or diseases like various forms of arthritis may alter normal joint architecture and physiology leading to secondary arthritis. Infection within the joint leading to septic arthritis is another catastrophic disorder that may irreversibly damage the human articular cartilage. The resultant bony ankylosis in certain cases is a serious complication of septic arthritis. There has been limited data regarding the burden of people living with post-infective ankylosis of one or more major joint and its other variables. Materials and Methods: Details of all consecutive cases coming to hospital out-patient department for the related or other complaints with one or more major joint found completely ankylosed in the radiographs were noted in separate file. The clinical complaints for which they present either related with the condition or other were also noted. Key demographic details like age, sex, laterality, history of causative factor, time living with the deformity, other co-morbidities were also recorded. The relevant details of the ankylosed joint following confirmation on radiographs were clinically confirmed with presence of no apparent movement of the affected joint. Results: A total of 21 (17 males and 3 females) patients with mean age of 32.1 years were part of the study. The ankle joint was most commonly involved followed by hip, knee and elbow in 13, 6, 2 and 1 case respectively. Our study attempts to know the burden of people living with ankylosed joint as a result of open injuries in the past. Conclusion: Relevant demographic data and the type of joint involvement in adults living in rural areas with one or more major ankylosed joints shall help draft preventive and treatment strategy in this regard.

Hip dislocation is a serious injury which most commonly presents as posterior dislocation. Inferi... more Hip dislocation is a serious injury which most commonly presents as posterior dislocation. Inferior dislocation is a rare event with a few anecdotal case reports or series described in the literature. This has also been called luxatio erecta of the hip borrowing from similar affliction at shoulder. We report five cases of luxatio erecta of the hip managed by reduction and conservative care in all but one. All five cases were males (mean age 31.8 years, range 18–52 year) with three cases being isolated injuries, whereas associated fracture of ipsilateral superior ramus and shaft femur was found in two separate cases. All were managed conservatively following closed reduction, except the case with shaft femur that was managed by additional operative fixation following the reduction of hip. The results were excellent in all cases without radiological evidence of avascular necrosis during the mean follow-up of 7.6 months (range 4–10 months).

The basic principles of diagnosis and management of snake-bite injuries should be known to rural practitioners and prompt referral should be done in selected cases. The extremity bite may occasionally result in localized cellulitis and soft tissue infection that may complicate later into underlying bone invilement. Dislocation following the open wound in these setting is rare occurrence that needs proper knowledge for its diagnosis and management.

Lateral elbow dislocation is a rare injury with few reported cases. Most elbow dislocations are p... more Lateral elbow dislocation is a rare injury with few reported cases. Most elbow dislocations are posterior and many have associated fractures of either humeral condyles. The medial epicondyle fracture in the setting of lateral elbow dislocation is a rare injury pattern. We report a case of lateral elbow dislocation with medial epicondyle displaced fracture in a 15-year-old male patient following a contact-sports-related injury. The elbow was reduced in the emergency and the medial fracture was fixed later with a pin and screw. There was no ulnar nerve injury associated primarily with the injury and was also not observed after surgery or during follow-up. The functional outcome was good and fracture united uneventfully. The patient had stable elbow with good range of motion and no clinical instability noted in the follow-up of 13 months. There was, however, gradual loosening of the screws noted in the follow-up and implants were removed at 9 months. There was 10° of cubitus valgus noted at last follow-up and the Mayo Elbow Performance Score was 90 depicting excellent outcome. The lateral elbow dislocation requires careful identification and exclusion of any associated nerve injury, soft tissue injury or bony fracture or their incarceration. The standard management and early rehabilitation leads to the optimum outcome.
