Andropause is characterized by the decrease of testosteron level which is accompanied by hypogonadism symptoms. Many studies mentioned that decreased testosterone level will contribute to inflammation state, one of the cytokine is TNF-α, which then lead to many chronic inflammation disorders and influence the quality of life. Understanding the significant correlation between androgen and inflammation allows accurate consideration making in using androgen replacement therapy, therefore the quality of life will be improved significantly. This research was an analytical cross-sectional study. Subjects were recruited through random sampling at Denpasar in 2019. TNF-α serum level was measured by human immunoassay method. Andropause was measured by ADAM questionnaire and confirmed by total decreased testosterone serum level. Quality of life was measured by WHOQOL-BREF questionnaire which was then continued by bivariate and multivariate analysis to control the confounding variables. It was considered statistically significant if P value is more than 0.05. There were 60 elderly involved as research subjects consisting of 43 andropause cases and 17 non andropause cases. There was significant correlation between TNF-α serum and quality of life. This study also found that there was significant difference in quality of life between andropause and non andropause group. This study shows that there was significant correlation between TNF-α serum level and andropause with quality of life in elderly.

Changes in the formation of this enzyme will cause the melanin pigment to be disrupted, resulting in the appearance of skin pigmentation disorders (hyperpigmentation). The purpose of this study was to prove the effectiveness of 15% sensitive plant leaf extract cream on guinea pig skin exposed to ultraviolet B (UVB). Methods: This is true experimental research using randomized post-test only control group design. The subjects were 30 male guinea pigs randomly divided into 3 groups, namely the control group (K), treatment group 1 (P1), and treatment group 2 (P2). Control group was not given any treatment, P1 was smeared with basic cream and exposed to UVB and P2 was smeared with 15% sensitive plant leaf extract cream and exposed to UVB for 2 weeks. After 2 weeks, the tyrosinase enzyme level were examined using ELISA and melanin amount were examined using Masson Fontana staining. The result was analyzed using comparative test between groups. Results: The tyrosinase enzyme in P2 group was 5,440,63 ng/ml, which is lower than K group (8,600,16 ng/ml (p<0,001)) and P1 (12,96±2,21 ng/ml (p<0,001)). The mean melanin amount in P2 was 0,090,07 %, which was also lower than K (0,310,24 % (p=0,049)) and P1 (0,53±0,47 % (p=0,043)). Conclusion: Administration of sensitive plant leaf extract cream inhibited the increase in tyrosinase enzyme and melanin amount in guinea pig exposed to UVB and normal subjects.

Hyperpigmentation is one of aging sign that happens due to melanin buildup in the skin. Hyperpigmentation can be caused by many factors, and one of the most frequent cause is ultraviolet (UV) ray exposure. Robusta coffee silverskin is a thin layer on a green coffee bean that is detached from the bean through roasting process, and is a waste product of coffee production. However, coffee silverskin contains high amount of phenolic and flavonoid compounds, and is categorized as strong antioxidant. The purpose of this study was to prove that administration of 30% Robusta coffee silverskin ethyl acetate fraction cream inhibited the increase of MITF expression and melanin amount in the skin of male adult guinea pig exposed to UVB ray. Methods: This study used randomized post-test only control group design. Subjects used in this study were healthy male guinea pigs with age ranging from 3-4 months and average weight 300-350g. Total samples used in this study were 36 guinea pigs divided evenly into two groups, Group K that were only exposed to UVB ray, and Group P that were exposed to UVB ray and 30% Robusta coffee silverskin ethyl acetate fraction cream. Total UV ray used during two weeks were 390mJ/cm 2. 30% Robusta coffee silverskin ethyl acetate fraction cream was applied twice a day on guinea pigs at the days they were not exposed to UVB ray, and at the days they were exposed to UVB ray, the cream was applied 20 minutes before and 4 hours after the exposure. MITF expression was examined under the microscope with MITF antibody kit, and melanin amount was examined with Masson-Fontana staining. Results: Result of the experiment showed higher MITF expression and melanin amount in the Group K.

Methods: Twenty erectile dysfunction patients aged between 32-74 years old were recruited in this study. The inclusion criteria were as follows: 1) adult males 18 years or older with a subjective complaint or erectile dysfunction, 2) to provide written informed consent, 3) to agree not to use other forms of treatment for erectile dysfunction, 4) fulfill the screening laboratory values. Part 1, eligible patients were titrated in the clinic starting with a dose of 250 microg and proceed in a stepwise manner to 500 microg and 1,000 microg on separate clinic visits until they identified a dose that produced a satisfactory response. The interval between each in-clinic titration was 2-3 days. Each in-clinic titration dose was evaluated at 15 min intervals over a one hour period for erection assessment, blood pressure and pulse. Part 2, patients used MUSE at home for three months at the dose identified during the in-clinic titration. Monthly interim visits were required for patient follow-up and drug distribution. At the end of the study, patients had another laboratory (except testosterone, only assayed in screening procedure) and physical examination. Results: The etiology of erectile dysfunction was psychological in 5 patients and organic in 15 patients. The 65% of the patients achieved the erection scale of 4 or 5 either in the clinic or at home, 10% achieved the scale of 4 at home, but not in the clinic, and 25% only achieved the scale of 2 or 3 with the highest dose of 1,000 microg either in the clinic or at home. No significant differences were found in biochemical examination before and after the study. The 60% of the patients who achieved erection scale 4 or 5 continued to use MUSE until the end of the study, while 40% of them complained of pain at the time of MUSE application, during erection and/or during intercourse. They withdrew from the study. Conclusion: Transurethral application of alprostadil (MUSE) is effective and safe to produce erection sufficient for intercourse in erectile dysfunction of various etiologies. Pain during application, erection and intercourse is a common side effect and a cause of withdrawal.

Twenty healthy, fertile volunteers were allocated randomly to either of two treatments each consisting of four intramuscular injections at monthly intervals. Group I ( n =10 men) received 100 mg DMPA plus 100 mg TE monthly while group I1 ( n = 10 men) received 200 mg DMPA plus 250 mg TE monthly. Sperm concentration was suppressed markedly, with all men attaining azoospermia between the third and fourth month after the start of treatment. There was no significant difference in the suppression of spermatogenesis between the two dosage regimens. The median time to reaching azoospermia was 2.5 months from the onset of injections and the median time to recovery of sperm in the ejaculate was 2.0 months after cessation of treatment. Both steroid regimens were equally effective in suppressing LH, FSH and testosterone levels. Testosterone levels returned to baseline by the fourth post-treatment month while LH and FSH demonstrated significant rebound above baseline levels from 3 to 5 months after cessation of treatment. No serious clinical side effects were observed. Weight gain and increases in libido were reported during treatment by most volunteers. A transient decrease in libido was noted in 5/20 (25%) men between 1-2 months after cessation of injections, presumably due to the prolonged effects of DMPA relative to TE. These results indicate that uniform induction of reversible azoospermia with minimal side effects can be achieved in a non-Causasian population. This study therefore supports the feasibility of an effective and reversible hormonal malecontraceptive in some populations even if this requires the consistent induction of azoospermia. The explanation for the higher frequency of azoospermia induced in Indonesian compared with Caucasian men remains to be clarified and is strategically critical to the future development of male contraception.

Introduction: The decrease of estrogen in the aging process causes various complaints in women. T... more Introduction: The decrease of estrogen in the aging process causes various complaints in women. This causes limitations in physical activity which ultimately reduces the quality of life. The use of phytopharmaca that has phytoestrogen activity is expected to help reduce the symptoms and complaints caused by a decrease in estrogen. This is due to its ability to bind to estrogen receptors. Basil leaves have phytoestrogen activity so that they can be an alternative in dealing with premenopausal symptoms. This study aims to prove that the ethanol extract of basil leaves can inhibit the decrease in the number of ovarian follicles and increase estradiol levels in premenopausal female Wistar rats. Methods: This research was an experimental post-test only control group design using female rats as the object of research, aged 10-18 months (human equivalent aged 40-45 years). The sample was divided into 2 groups, the treatment group which was given ethanol extract of basil leaf at a dosage of 0.50 g/kg BW and the control group which was given aquabidest every day orally for 21 days. After 21 days, blood was taken through the medial canthus of the orbital sinus to check estradiol levels using the ELISA method and the ovarian organs were taken for histological follicle counting. Results: The mean of follicles in the control group was 5.39 ± 1.14/LPB while the mean in the treatment group was 13.88 ± 2.87/LBP. These data indicate that the mean of the treatment group is higher than the control group.
