In a multicenter study, 905 patients with newly diagnosed advanced prostate cancer treated with f... more In a multicenter study, 905 patients with newly diagnosed advanced prostate cancer treated with flutamide were followed for safety and side effects. Flutamide was administered in monotherapy (75 patients (8%)) or as part of total androgen blockade (TAB groups): the latter group had orchiectomy (196 patients (22%)) or an LHRH (634 patients (70%)). The incidence of gastrointestinal complaints such as loss of appetite, vomiting/nausea and diarrhea (+/- 15%) was similar in the TAB and the monotherapy groups. Eighteen patients (2%) were withdrawn because of severe diarrhea. Hot flushes was the most frequently reported adverse event in TAB groups (40% of patients), leading to withdrawal in 4 patients (0.4%). Breast tenderness occurred most frequently in the monotherapy group (49%) and was reported as severe in 7% of the patients. Only 0.8% of the patients were withdrawn because of liver function changes. No unexpected potentially dangerous adverse events were reported. Flutamide alone or in combination therapy appears to be safe and well tolerated.

Conservative treatment, EWSL, ureteroscopy (URS), percutaneous litholapaxy, and (laparoscopic) ureterolithotomy, all have there place in the treatment of ureteric lithiasis. In order to evaluate if ureteroscopy is a valuable alternative to ESWL in the treatment of ureteric lithiasis, an analysis of all the ureteroscopies performed in our institution was made and these results were compared with results (both ESWL and URS) described in literature. During the period 1990 till 1997, 292 patients underwent in our institution in total 354 ureteroscopies for ureteric lithiasis. In all procedures a rigid 9 Fr. ureteroscope was used, together with laser lithotripsy or pneumatic lithotripsy as fragmentation device. Five hundred forty-six patients with advanced loco-regional (M0) or distant metastatic (M1) prostatic carcinoma treated with flutamide combined with either orchiectomy or LHRH analogues were included in this analysis. Initial patients characteristics and the PSA response were evaluated in relation to progression-free survival using a univariate and multivariate (Cox regression) analysis. The following prognostic factors were indicative of a decrease in progression-free survival: the absence of PSA normalization (< 4 ng/ml) after 3 or 6 months, M1 stage, high G grade, ECOG performance status > 1, presence of pain and absence of dysuria. Prognosis of patients who have been treated by radical prostatectomy, because of prostatic carcin... more Prognosis of patients who have been treated by radical prostatectomy, because of prostatic carcinoma, is dependent on a number of factors. In this study we try to determine these factors, their relationship with each other, their relationship with progression of disease and the prognostic value of this relationship. "Progression" (or relapse) of disease is defined as clinical evidence of local or general recurrence and/or isolated elevation of postoperative PSA level more than 0.2 ng/ml (Hybritech) at two consecutive outpatient visits. In our institution, 62 radical retropubic prostatectomies were performed over a period of 66 months. Without any exclusion, these patients were included in the analysis (with respect to "progression"), which consisted of bi- and multivariate analyses, Kaplan-Meier estimations and multivariate survival analyses using the "Cox proportional hazards model". We did a randomised phase III trial comparing external irradiation alone and external irradiation... more We did a randomised phase III trial comparing external irradiation alone and external irradiation combined with an analogue of luteinising-hormone releasing hormone (LHRH) to investigate the added value of long-term androgen suppression in locally advanced prostate cancer. Between 1987 and 1995, 415 patients were randomly assigned radiotherapy alone or radiotherapy plus immediate androgen suppression. Eligible patients had T1-2 tumours of WHO grade 3 or T3-4 N0-1 M0 tumours; the median age of participants was 71 years (range 51-80). In both treatment groups, 50 Gy radiation was delivered to the pelvis over 5 weeks, and 20 Gy over 2 weeks as a prostatic boost. Goserelin (3.6 mg subcutaneously every 4 weeks) was started on the first day of irradiation and continued for 3 years; cyproterone acetate (150 mg orally) was given for 1 month starting 1 week before the first goserelin injection. The primary endpoint was clinical disease-free survival. Analyses were by intention to treat. 412 patients had evaluable data, with median follow-up of 66 months (range 1-126). 5-year clinical disease-free survival was 40% (95% CI 32-48) in the radiotherapy-alone group and 74% (67-81) in the combined-treatment group (p=0.0001). 5-year overall survival was 62% (52-72) and 78% (72-84), respectively (p=0.0002) and 5-year specific survival 79% (72-86) and 94% (90-98). Immediate androgen suppression with an LHRH analogue given during and for 3 years after external irradiation improves disease-free and overall survival of patients with locally advanced prostate cancer.

Two generations in the Western countries are equal to three generations in Africa, where life is shortened by early marriage, early childbirth and early death. Today, already more than 70% of the population lives in urban areas. A growing city becomes dirtier, uglier and more dangerous as a consequence of lack of planning and money, poor health and crime. But in spite of the fact of poverty, AIDS and low economic development, the population in Africa is still increasing. The average life expectancy at birth for men in Africa is 49 years (worldwide 63), but will increase to 60 years by 2050. The major problem for Africa is that the Western countries became rich before they became old, whereas the African countries will become old before they are rich!

King Leopold II was the driving force behind these undertakings. In China, for instance, a railway between Beijing and Hankou and a tramway network in Tianjin were constructed. 'A Belgian Passage to China (1870-1930)' puts these two projects in the spotlight with a strong focus on the personal histories of some of the Belgian citizens involved in these large-scale projects. François Nuyens left Ghent in 1905 for Tianjin, where he built a power station and constructed a tram network. In a well-documented diary, Nuyens writes down his impressions of his stay in China between 1905 and 1908. Around the same time the Ghent born brothers Philippe and Adolphe Spruyt, both doctors, travelled to China to take care of the medical service at the railway construction sites between Beijing and Hankou. Both Philippe and Adolphe were avid collectors of Chinese antiquities, which are currently preserved in various Belgian musea. The brothers' lively correspondence and their more than 1,200 stereoscopic photographs on glass plate offer a unique glimpse of everyday life in China in the early twentieth century

Although Gersuny injected glycerine around the perineal urethra to treat incontinent males it was Politano [1] who proposed teflon to treat incontinence in men after transurethral resection of the prostate and extended the technique to treat women patients. Heer [2] injected teflon into the wall of the urethra to increase its length and resistance but the technique of transurethral and periurethral injection of teflon paste was really developed in Western Europe by Sparwasser and Lampante [3,4] in 1978 with the fabrication of specialised instrumentation by Wolf-Knittlingen and Storz-Tuttlingen. Marketing by these manufacturers certainly diffused this new technique to most urological departments but was also responsable for a subjective overevaluation!
