Sunnybrook opening new CONNECT Clinic space in B-Wing
Introduction to the Care Program
At Sunnybrook, your care doesn’t stop when you leave the hospital. The Integrated Community Program is designed to seamlessly connect you with essential clinical and care services to care for you while you are in the hospital and bridge your care with community and primary healthcare. This program includes inpatient care, many specialized outpatient clinics, as well as programs that help you move from hospital to home. Some of the services include family and community medicine, palliative care, reactivation care, specialized geriatric services and connections to homecare. We work closely with family doctors and community care teams to make sure our community gets the right care at the right time, in a way that meets their needs.
The New Peter Cipriano Centre for Senior’s Health
The Peter Cipriano Centre for Seniors Health will create a first-ever patient-centred model of care to keep seniors healthy and in their community.
