This is the I-94W Nonimmigrant Visa Waiver Arrival / Departure Form that is required to be completed by non-Americans from certain countries (e.g. Australia, the UK, France, Germany, New Zealand) on arrival to the USA. I've had to fill this in on a number of occasions when arriving / transiting through Los Angeles, and the questions on the back asking whether you're a spy, drug-dealer or a Nazi have always amused me - I mean, is anyone /really/ going to think "Why yes, I'm bringing in 20kg of cocaine to finance my terrorist activities, I'd better check that box"?

If you're from the USA, be glad you don't have to fill out anything like this for getting home :-)
