Aim: To identify the characteristics of rhinolithiasis in terms of its clinical, radiological, an... more Aim: To identify the characteristics of rhinolithiasis in terms of its clinical, radiological, and surgical features in the largest case series in the literature. Material and Methods: A rhinolithiasis series comprising 32 cases that were treated at a single tertiary care center between 2014 and 2019 was reviewed retrospectively. The patients' demographic characteristics, clinical features, rhinolith localization, accompanying sinonasal pathologies, and surgical data were noted. All patients underwent surgery via an endoscopic approach under general anesthesia, and any concomitant sinonasal pathology was treated during the same session. The patients were followed up with endoscopic examinations at the postoperative first and sixth months. Results: The sample consisted of 19 female (59.4%) and 13 male (40.6%) patients, with a mean age of 35.53±15.06 years. The most common symptoms were nasal malodor (65.6%), nasal congestion (53.1%), and purulent nasal discharge (15.6%). Rhinoliths were located in the right nasal cavity in 17 cases and the left nasal cavity in 15 cases. The most common localization of rhinoliths was between the septum and the inferior turbinate (65.6%). A nidus was detected in only four cases (12.5%), of which three belonged to an ectopic tooth. The most common concomitant nasal pathology was chronic rhinosinusitis (34.4%), followed by septal deviation (25%) and allergic rhinitis (9.4%). Endoscopic rhinolith excision was performed in 17 patients (53.1%), while surgical intervention was required for additional pathologies in 15 patients (46.9%). The most common simultaneous surgical intervention was septoplasty (15.6%). Conclusion: Rhinolithiasis should be considered in cases presenting with unilateral nasal symptoms that persist or worsen after initial treatment. Since the treatment of rhinolithiasis is surgical excision, the appropriate approach is the preoperative detection of pathologies which require additional surgery that can be simultaneously undertaken with rhinolith excision.

The right vocal cord was fixed. A biopsy obtained from the lesion showed a low-grade mucoepidermoid carcinoma. He underwent total laryngectomy and right functional neck dissection.

Methods: Thirty-four patients with nasal polyposis were divided into two groups receiving either intranasal steroid or intranasal steroid plus per-oral vitamins A, C and E and selenium. Paranasal sinus computed tomography, endoscopy, and polyp tissue and serum sampling were conducted pre-and post-therapy. Serum levels of malondialdehyde, superoxide dismutase, nitrite and myeloperoxidase and tissue levels of malondialdehyde and superoxide dismutase were measured. Group results were compared using the Mann-Whitney U test and Wilcoxon signed-rank test. Results: Both groups had significantly lower tissue parameters, computed tomography scores and serum malondialdehyde levels, comparing pre-versus post-treatment results. Post-treatment, the steroid plus antioxidant group had significantly lower tissue malondialdehyde levels and a greater fall in tissue and serum malondialdehyde, compared with the steroid group. Conclusion: Serum and tissue levels of malondialdehyde (an oxidative marker) were significantly decreased by adding antioxidants to standard therapy. This is the first report of the positive effects of adding antioxidants to steroid therapy for nasal polyposis.

In this study, we evaluated the CT scans of 1193 patients and detected cases with sphenoid sinus agenesis despite the absence of craniofacial anomalies or syndromes. Methods: The CT scans of 1193 patients who had undergone endoscopic sinus surgery in our clinic between 1999 and 2008 were retrospectively evaluated. Of the patients, 652 (54.6%) were female and 541 (45.3%) were male. The mean age of the patients was 32 years (range: 13-75 yrs.). Results: In this study, we identified eight patients (0.67%) with sphenoid agenesis. Three patients (37.5%) had bilateral, 3 patients (37.5%) had left, and 2 patients (25%) had right sphenoid agenesis. None of the patients had craniofacial anomalies or syndromes. Conclusion: Sphenoid sinus agenesis is rarely seen in the absence of craniofacial anomalies or syndromes. In our study, its incidence was 0.67%. Awareness of sphenoid sinus agenesis and/or hypoplasia is very important, particularly when transphenoidal hypophysectomy is planned.

A 35-year-old male patient presented with nasal obstruction and snoring. Nasal endoscopic examination with a rigid endoscope revealed a large mass on the posterior wall and roof of the nasopharynx. A computed tomography scan of the nasopharynx revealed a polypoid mass in the posterior nasopharyngeal wall. Histopathological examination of a biopsy taken from the posterior nasopharyngeal wall revealed granulomatous lesions with caseation necrosis. Treatment was decided after histopathological examination and intradermal positive reaction to tuberculin test, and started a week later. The patient was treated with antituberculosis drugs including isoniazid, rifampicin, pyrazinamide, and ethambutol. After six months, the nasopharyngeal examination was absolutely normal, and the patient was also symptom free. A 17-year-old male patient presented with nasal obstruction. Anterior rhinoscopy and nasal endoscopic examination revealed left middle turbinate and bilateral inferior turbinate hypertrophy, septal deviation to the right. A coronal computed tomography (CT) scan of the paranasal sinuses showed pneumatisation of both superior and inferior turbinates and a left middle turbinate, and also showed pneumatisation of the right middle turbinate. The patient underwent endoscopic sinus surgery of the left side with lateral lamellar resection of the middle turbinate under local anaesthesia. We present the first case in the world literature of the pneumatisation of all six turbinates.

Yöntem ve gereçler: Mart 2004 ve Mart 2007 tarihleri arasında takip edilen horlama ve hafif obstrüktif uyku apne sendromu olan 103 hasta çalışmaya dahil edildi. Apne indeksi 0-15,3 arasındaydı (ortalama 5,08±3,84). Uygulamalar lokal anestezi altında yumuşak damağın üç bölgesine yapıldı. Değerlendirmeler uygulama öncesinde, altıncı ve on ikinci aylarda yapıldı. Hastalar Epworth Uykululuk Skalası (EUS) ve Görsel Analog Skala (GAS) kullanılarak değerlendirildi. Twenty-six consecutive patients with Behçet's disease and a control group consisting of 25 age-matched healthy subjects were prospectively included in this study. Pure tone and speech audiometry, tympanometry, distortion product otoacoustic emission testing, and auditory brainstem-evoked response assessment were performed in the patients and controls. The pure tone audiograms and the results of distortion product otoacoustic emission testing showed statistically significant hearing loss in the Behçet's disease patients (p < 0.05). Auditory brainstem-evoked response results were not significantly different between the patients and controls (p > 0.05). The aim of this presentation is to show that the sublabial transseptal transsphenoidal approach t... more The aim of this presentation is to show that the sublabial transseptal transsphenoidal approach to pituitary adenomas is the least invasive anatomic route with the lowest postoperative complication rates. PATIENTS and METHOD: This study was based on a retrospective analysis of 276 patients with a diagnosis of pituitary adenoma who were surgically treated via the sublabial transsphenoidal route. RESULTS: The overall complication rate of the presented series was 14.8%. The recurrence rate was 14.1%; there were no deaths or intranasal complications in this series. Postoperative rhinological complications, such as septal perforation, synechiae, and mucosal tear which impaired nasal function, occur rarely with this approach compared to other approaches. CONCLUSION: Besides the technical advantages of this approach, performance of the initial phase of the operation by an otorhinolaryngologic surgeon is the basis of the low intranasal complication rate in our series.

The study included 116 patients (4 females, 112 males; mean age 55+/-9 years; range 35 to 75 years) who underwent surgery for squamous cell carcinoma of the larynx. Risk factors for survival and recurrence were analyzed using univariate analysis and Cox regression analysis. The mean follow-up period was 43+/-25 months. Mortality occurred in 14 patients, nine of which were due to tumoral causes. In univariate analyses, lymphovascular invasion, cartilage invasion, positive surgical margins, postoperative deglutition time, and complications had a significant effect on survival, while recurrence was influenced by age, postoperative deglutition time, postoperative radiotherapy, and positive surgical margins (p<0.05). Eighty-three patients (55 (66.3%) men, mean age: 38.2±14.5 years) were included in the study. Patients were divided into three groups according to the treatment strategy: group 1 received only antibiotic treatment, group 2 underwent abscess drainage with needle puncture in addition to antibiotic treatment, and group 3 underwent surgical drainage with antibiotic treatment. Laboratory outcomes, imaging methods, duration of hospital stay, treatment strategy, and clinical outcomes were analyzed.According to the laboratory results, complete blood count values did not vary among the three groups, but C reactive protein (CRP) and erythrocyte sedimentation rate (ESR) values were higher in group 3 (p<0.01). ÖZET Amaç: Çalışmanın amacı horlama ve hafif obstrüktif uyku apne sendromlu kişilerde yumuşak dam... more ÖZET Amaç: Çalışmanın amacı horlama ve hafif obstrüktif uyku apne sendromlu kişilerde yumuşak damağa ısı kontrollü radyofrekans ablasyonun etkilerinin değerlendirilmesidir. Yöntem ve gereçler: Mart 2004 ve Mart 2007 tarihleri arasında takip edilen horlama ve hafif obstrüktif uyku apne sendromu olan 103 hasta çalışmaya dahil edildi. Apne indeksi 0-15,3 arasındaydı (ortalama 5,08±3,84). Uygulamalar lokal anestezi altında yumuşak damağın üç bölgesine yapıldı. Değerlendirmeler uygulama öncesinde, altıncı ve on ikinci aylarda yapıldı. Hastalar Epworth Uykululuk Skalası (EUS) ve Görsel Analog Skala (GAS) kullanılarak değerlendirildi. Hastaların şikayetlerini (konuşma bozukluğu, yutma güçlüğü gibi) de subjektif yöntemlerle değerlendirdik. Takip süresi 6-14 (ortalama 9,40±2,10) ay arasındaydı. GAS, EUS seviyesi ve horlama skorları paired-samples T test ile analiz edildi. Twenty-six consecutive patients with Behçet's disease and a control group consisting of 25 age-matched healthy subjects were prospectively included in this study. Pure tone and speech audiometry, tympanometry, distortion product otoacoustic emission testing, and auditory brainstem-evoked response assessment were performed in the patients and controls. The pure tone audiograms and the results of distortion product otoacoustic emission testing showed statistically significant hearing loss in the Behçet's disease patients (p < 0.05). Auditory brainstem-evoked response results were not significantly different between the patients and controls (p > 0.05). Medikal tedaviye rağmen 6 aydır baş ağrısı yakınması olan 52 yaşında bayan hasta kliniğimize başvurdu. Muayene ve görüntüleme yöntemleri sonucunda süperior konkada aşırı pnömatizasyon saptanan hastaya endoskopik sinüs cerrahisi uygulandı. Postoperatif dönemde hastanın şikayetlerinin gerilediği izlendi.

The aim of this presentation is to show that the sublabial transseptal transsphenoidal approach t... more The aim of this presentation is to show that the sublabial transseptal transsphenoidal approach to pituitary adenomas is the least invasive anatomic route with the lowest postoperative complication rates. PATIENTS and METHOD: This study was based on a retrospective analysis of 276 patients with a diagnosis of pituitary adenoma who were surgically treated via the sublabial transsphenoidal route. RESULTS: The overall complication rate of the presented series was 14.8%. The recurrence rate was 14.1%; there were no deaths or intranasal complications in this series. Postoperative rhinological complications, such as septal perforation, synechiae, and mucosal tear which impaired nasal function, occur rarely with this approach compared to other approaches. CONCLUSION: Besides the technical advantages of this approach, performance of the initial phase of the operation by an otorhinolaryngologic surgeon is the basis of the low intranasal complication rate in our series.

It is mostly located in the supraglottis. It frequently occurs in middle-aged and elderly men. The treatment is primarily surgical. A 42-year-old man presented with a complaint of hoarseness of six-month history. Examination revealed a swelling involving the right arytenoid, false cord, and vocal cords. The right vocal cord was fixed. A biopsy obtained from the lesion showed a low-grade mucoepidermoid carcinoma. He underwent total laryngectomy and right functional neck dissection.
We investigated the prognostic factors affecting survival and recurrence in laryngeal cancer. The... more We investigated the prognostic factors affecting survival and recurrence in laryngeal cancer. The study included 116 patients (4 females, 112 males; mean age 55+/-9 years; range 35 to 75 years) who underwent surgery for squamous cell carcinoma of the larynx. Risk factors for survival and recurrence were analyzed using univariate analysis and Cox regression analysis. The mean follow-up period was 43+/-25 months. Mortality occurred in 14 patients, nine of which were due to tumoral causes. In univariate analyses, lymphovascular invasion, cartilage invasion, positive surgical margins, postoperative deglutition time, and complications had a significant effect on survival, while recurrence was influenced by age, postoperative deglutition time, postoperative radiotherapy, and positive surgical margins (p<0.05).
