1) Threads that should be logs. Now, threading has its place. Back-and-forth, live interaction, that's best handled by threads. But plots, things with structure, things that happen outside the context of an IC journalling system? Logs, dammit. We're writers, how about we try actually writing?

2) If you're going to force me to thread, try to actually keep up, mmkay? I understand people get busy and all that, but forcing me to wait fifteen-minutes plus (or some ridiculous amount of time) in response to one ping is just unacceptable. End of story. If you're that busy that's fine - your business - but maybe you shouldn't be trying to RP while you're being pulled in that many different directions, mmmkay? It really takes a lot of the pleasure of play out of things. I should not have time to read a chapter of a book in between pings.
3) Lack of structure when I'm not invested in the plot to begin with. You want me to participate in your Cool Thing? Excellent! I'll be happy to. But don't just say "OK, go for it!" without telling me how precisely what I'm going for slots into things. Or providing a starter. Or anything at all. Bonus ugh when the RP in question takes place in a word I'm not tremendously vested in or comfortable trying to write around. Like Xander said, "Give me an axe and tell me where to stick it."
More as I think of them, but those were the biggies off the top of my head.

It's along the lines of "Well, let's take this batshit insane premise and play it absolutely straight, extrapolate it out, and see where it leads us." Which is, apparently, where the Machine Empire…
