Risk Indicator Checklist

The risk factors included in the Risk Indicator Checklist (RIC) connect to the items in the Saskatchewan Safety Planning Guide. Completion of the RIC will produce a Risk Report and a Safety Planning Guide. The Risk Report includes the completed checklist, including risk factors present along with the details provided, risk factor totals, and the IPVA professionals’ assessment of risk.  The Safety Planning Guide will produce a corresponding safety plan with strategies linked to relevant risk factors. Some suggested strategies and resources are pre-filled on the Safety Planning Guide; however, these must be revised and added to, depending on the client’s geographic location and unique situation.

When completing the RIC, the client will be assigned a confidential ID number. Clients can use this number to return to their completed RIC to update information, download, or print for the next two months. Do not enter the client’s name or any other identifying information.

 

PATHS will periodically publish updates on the usage of the RIC—for example, how many people have used the tool or identified the presence of particular risk factors.
