Pathologic 3 has been released! Thank you for your patience as we begin to document this incredible game!
Pathologic is a 2005 psychological horror survival role-playing game developed by Russian game studio Ice-Pick Lodge. On May 23, 2019, Pathologic 2 was released as a re-imagining of the original, and Pathologic 3 released on January 9 2026.
A mysterious and deadly disease breaks out in a remote town in the Russian steppe. It is highly contagious and kills whoever contracts it, giving them only several days left to live. This struggle to survive follows three healers and their individual stories. One is a learned doctor from the country’s capital, named the Bachelor. The second is a local medicine man who has returned at the request of his father, named the Haruspex. The third and final healer is a strange young girl who claims to be both a saint and a miracle worker, named the Changeling.
Taking on the role of one of the healers, you’ll explore the Town, its traditions and history, and the complex relationships of its inhabitants. You will only have twelve days to defeat the invisible enemy called the Sand Plague as starvation, exhaustion, sickness, and time itself stands in your way. It will be impossible to save everyone. It may be even be impossible to save yourself. The Pathologic Wiki is a community project run entirely by volunteers to provide information about the games. If you'd like to help, please first create an account.
Not sure where to start?
- Make sure to read first the Community portal's recommendations and guidelines.
- If you are unsure of what to do or how to create a page, search for a few articles on the same topic and see what they look like. You can always view the source code in a wiki and learn from what others have done. For details on the style of articles hosted on this Wiki please visit our Style Guide!
- An edit doesn't have to be massive; if you feel you don't want to create whole articles, then just fixing spelling errors and broken links is enough.
- If you have a question on how a page should be structured or presented, never hesitate to ask. We are all working together to create the best wiki project we can, and we can accomplish that by brainstorming together.
- We are always looking for multilingual individuals who would like to assist with translating the wiki! If you are able to speak an additional language to English and would like to help with translating the wiki, please reach out to admins via Discord or our User pages to learn about translating requirements.
- Those who are interested in modding and debugging the games should check out the debugging category for a basic overview, or refer to the Modding Wiki for detailed explanations.
- All contributors are encouraged to join our Discord to help further the project and aid in communication with one another!

Murky is a fragile, sickly-looking orphan girl who lives in a railway carriage next to the Train Station. She is one of the town's orphans.
She often gets hunches on other people or things going on in the town, and some describe her as a "feral child, improperly raised". The Earth seems to be able to speak to Murky in a way that is not dissimilar to the ability of the Haruspex or some of the Mistresses. Murky often spends time with the Soul-and-a-Halves, though she lives alone in an abandoned train car by the Train Station without any other child. She often dreams that one day the train will unlock from the tracks and take her away to the sea as she sleeps. Her mindset is that people are like safety pins; open or closed, they always have a pointing end. While cryptic, she is still a child, and thus is curious of things shown to her. Read more.
