High numbers of people living with TB/HIV fail treatment and develop resistance because they cannot maintain a high degree of adherence to their medication regimens. This paper illustrates how a simple and inexpensive SMS-based mHealth application can be used to facilitate the TB/HIV treatment.

The risk of progression from exposure to the tuberculosis bacilli to the development of active di... more The risk of progression from exposure to the tuberculosis bacilli to the development of active disease is a two-stage process governed by both exogenous and endogenous risk factors. Exogenous factors play a key role in accentuating the progression from exposure to infection among which the bacillary load in the sputum and the proximity of an individual to an infectious TB case are key factors. Similarly endogenous factors lead in progression from infection to active TB disease. Along with well-established risk factors (such as human immunodeficiency virus (HIV), malnutrition, and young age), emerging variables such as diabetes, indoor air pollution, alcohol, use of immunosuppressive drugs, and tobacco smoke play a significant role at both the individual and population level. Socioeconomic and behavioral factors are also shown to increase the susceptibility to infection. Specific groups such as health care workers and indigenous population are also at an increased risk of TB infection and disease. This paper summarizes these factors along with health system issues such as the effects of delay in diagnosis of TB in the transmission of the bacilli. of Hindawi Publishing Corporation

A few reports exist on comorbidities among HIV-infected IDUs in India. We carried out a retrospective study to analyse data on comorbidities in India and treatment challenges faced when treating HIV-infected IDUs in India. A retrospective chart review of 118 HIV-infected IDUs who accessed care at the YRG Centre for Substance Abuse-Related Research, Chennai, between August 2005 and February 2006 was done. Demographic, laboratory and clinical information was extracted from medical records. Descriptive demographic and clinical characteristics and distributions of comorbidities across CD4 cell count strata were analysed. All IDUs were male with a median age of 35.5 yr. The majority were married with average monthly income less than INR 3000 per month. The prevalence of hepatitis B and C infections were 11.9 and 94.1 per cent, respectively. The controls with the positive or negative results were selected by matching 1:1 with the case for date of QFT-G IT assay. An indeterminate result was considered when the level in both the tuberculosis antigen and mitogen tube was less than 0.35 IU/ml, or the level in the nil tube was higher than 8.0 IU/ml. Through retrospective review of medical records, age, gender, total lymphocyte count at the QFT-G IT assay, underlying diseases, and medical conditions which can influence the immune function were collected as covariates. The predictive factors for the indeterminate results on QFT-G IT were assessed by multivariate logistic regression models.

This online program may be a model for other efforts to provide education to health care providers treating HIV-infected patients in the developing world.
