Record data in your CRM and automatically gather leads with the Jetpack CRM forms.
Jetpack CRM comes with built-in Lead Generation Forms. You can use these forms to capture data in your CRM and automatically capture leads.
Create a CRM form
To set up a form and add it to a post or page, follow these steps:
- Head to the Jetpack CRM Dashboard.
- From the horizontal menu bar, head to Tools → Forms.
- You will see the Form Language Labels box with a list of default fields that will appear in the form.
- Scroll down to the Form Setting box. Click on your preferred style for your form (see below for more details), and save the changes.
- Copy and paste the embedding code into your site’s content page or post, or into a different website.
Submission flow
To check where the submissions go, you can test your form by checking the following steps:
- Head to the page or post where the form has been added.
- Fill out the form. You may want to use a test email address.
- If you have entered a new email address that is not in your CRM contact lists yet, the system creates a contact account.
- If the email address is already present in your CRM contact list, the data included in the form will be added (if new information) or updated to the existing contact account page.
Any content submitted through the form will be added to the contact page in the Activity log area. The entry is called Form filled.
Form Styles
The Jetpack CRM built-in forms come with three style choices:
- Naked style: it includes the first name and the email fields.
- Content grab: you can collect additional information as the form contains multiple fields.
- Simple style: it collects emails only.
The built-in forms also track each form’s conversion rate and log exactly which form the user came from in the customer log.
Add more fields to the form
The CRM forms come with the three styles mentioned above, which include only a few fields by default.
If you need to add additional fields to your lead generation form, you can use a Jetpack form instead.
The Jetpack contact form block allows you to add a contact form to your site and emails you a notification when someone submits the form.
By contrast, the Jetpack CRM form allows you to add an email signup form to the site. This is different from Jetpack’s contact form, which does not email you a notification when someone signs up.
Another alternative is to use one of our premium form integration extensions, along with their native contact forms plugins, such as Gravity Forms and Contact Form 7. You can check our extensions on this page.
is there a way to redirect them to a new page after clicking submit?
Hi Mo,
Good question. In the built in forms it doesn’t redirect after submit I’m afraid.
You could use Gravity Forms with our Gravity Forms Extension (https://jetpackcrm.com/product/gravity-forms/)
We also have a Contact Form 7 extension (https://jetpackcrm.com/product/contact-form-7/).
Gravity Forms is a premium plugin whereas Contact form 7 isn’t so test out with Contact Form 7 + our extension first.

I need to store the data of a Contact Form to a new Quote entry with custom fields. Is this possible?
Form to Quote is not something currently supported (as of 26th September)
If this is still important to you please contact us: http://kb.jetpackcrm.com/submit-a-ticket/ to discuss further
Can I receive an email when someone fills out a form? I see that the message goes into the activity log on the contact record.
Hi Lou
With Automations you can set it up so when you have a new lead (via a Form) it can email you as admin.
Hi,

Hi Peha,
This was a bug which was fixed in 3.0.14, please do update your plugin and you should see this resolved.
Hi,
I’m using Version 3.0.15 but I still don’t know how to translate the headings and sentences of the form?
thx
Hi Roberto,
You can enter any headings and sentences you’d like in the front-end form (via the edit page) – do you mean change the labels in the form editor?
Hello Woody,
I edited the page with my phrases and it didn’t work. 🙁
Thank you
