Quality Tools and Processes

Across our organisation, we use Nourish. This digital tool allows us to plan care in a way that works for our colleagues and the people we support.

It brings everything together in one system, including referrals, assessments, transition planning, risk management, support planning and quality monitoring.

This means we can see a person’s journey, we can celebrate their achievements and quickly find ways to improve. We can make all our plans for their future in real time.

The people we support act as ‘experts by experience’ to help us drive continuous improvement.
