The Prevention of Diabetic Ketoacidosis in Patients with Type 2 Diabetes on Sodium-Glucose Transport Protein 2 Inhibitors

Diabetic ketoacidosis (DKA) has been reported as a life-threatening adverse effect due to SGLT2Is use. Aim: This study aims to review the current evidence of incidence, predisposing factors and the prevention of DKA in T2DM patients on SGLT2Is use. Methods and Materials: Two reviewers have conducted a search strategy of studies published in English between August 2012 and November 2020, in EBSCOhost, Google Scholar, PubMed, Science Direct and Wiley. Two reviewers independently assessed the eligibility and quality of the studies and extracted the data. Results: 85 studies were identified in the initial search; 75 records were removed and finally, 10 studies were included. Only studies discussing the prevention of DKA in T2DM patients on SGLT2Is were selected, extracted and categorized into main domains that included SGLT2Is use inT2DM patients and DKA (50%), SGLT2Is use in T2DM patients (20%), the clinical presentation of DKA (20%) and DKA prevention (10%). Six studies showed SGLT2Is increased the risk of DKA with very low rates in two studies. The precipitating factors of DKA in all included studies were revealed as stopping or reducing insulin, trauma, infection, surgery, severe acute illness, vigorous exercise, dehydration, low carbohydrate intake and excessive alcohol intake. In two studies DKA can be prevented by wakefulness and education, in one study by closed follow-up, in one study by regular monitoring and adjustment of medications and in two studies by recognition of patients at risk. Conclusions: This review summarized the prevention of DKA in T2DM patients on SGLT2Is use with consideration of incidence, a summary of evidence and predisposing factors. Physicians, health care providers and patients should be aware of SGLT2Is use, regular follow up, precipitating factors, symptoms, signs and prevention of DKA.

The adverse effects and interactions are numerous so, physicians' knowledge including accurate indications after confirmations of diagnosis, duration of use and followup are an essential part of the treatment process. Aim: The purpose of this review tis o help health care professionals to prescribe the PPIs wisely and appropriately. Objective: To undertake a review of the literature relating to the long-term use of PPIs Data Sources: United States, Food and Drug Administration [FDA], PubMed, Scopus, Science Direct, Wiley, Google Scholar, major clinical guidelines, and governmental agencies. Data Extraction: The 2 reviewers Musa Basheer Mansour and Sara Ahmedana [MBM and SEA] independently assessed the qualities of the extracted studies and summarized data of the studies for outcomes of interest and performed quality assessments based on review of published articles and the data sources. Results: We reviewed the available evidence and summarized the long-term use of PPIs as indicated by major clinical guidelines, governmental agencies, and published studies.

Methods: It was a descriptive-analytical study in OFTH August 2010. (241) participants by simple random sampling. All patients attend the OFTH for their appointment and agreed to be included in this study. Whereas the emergent and urgent were excluded. Data were collected using small groups discussion and pre-tested questionnaires. Results: Many doctors know some information about Doctor-Patient Communication Skills but, they didn't perform it because of, the high frequency of patients in referral clinics. Doctors spent 6 to 10 minutes with 179 patients (74.3%), 11to 15 minutes with 46 patients (19.1%), 1 to 5 minutes with 12 patients (5%), 16 to 20 minutes with 3 patients (1.2%) and more than 26 minutes with one patient (O.4%). Patients' satisfaction; 205 patients are satisfied represent (85.1%), 36 patients aren't satisfied represent (14.9%). Those who aren't satisfied; 25 patients (69%) because of a short duration of the medical interview, 8 patients (22%) because doctors didn't mention the adverse effects of the drugs and 3 patients (9%) because doctors didn't mention the possible complications of the operation. Conclusion: Doctor-patient communication skills were done by medical officers, registrars, physicians, and consultants. They took a short time 6-10 minutes for the medical interview (74.3%), they didn't share information with their colleagues in the same unit by (46.5%) in 112 patients and didn't involve other health care professionals in patients' care by (62.7%) for 151 patients.
