Objective: We examined a group of patients awaiting interferon treatment for hepatitis C to estim... more Objective: We examined a group of patients awaiting interferon treatment for hepatitis C to estimate the prevalence and detection rates of and risk factors for mood disorders. Methods: The Structured Clinical Interview for DSM-IV Axis I Disorders: Clinician Version was used to detect psychiatric disorder. Selfcompletion instruments were used to rate symptom severity, subjective cognitive function, work and social adjustment, stigma, acceptance of illness and treatment satisfaction. Results: The 90 participants included 23 women (26%); 33 (37%) had contracted hepatitis C iatrogenically, 42 (47%) through injecting drug use and the remainder (17%) were of unknown origin. There was a 28% 1-month prevalence of depressive disorders, 72% of whom were previously undiagnosed, and a 24% prevalence of anxiety disorders, 86% previously undiagnosed. Current methadone maintenance was strongly associated with risk of depression (odds ratio, 5.0; 95% CI, 1.08-23.0). After adjustment for age and sex, depression was associated with poorer work and social adjustment, lower acceptance of illness, higher illness stigma, poorer reported thinking and concentration, and higher levels of subjective physical symptoms (all P b.05). Anxiety disorders were uncorrelated with any risk factor. Conclusions: Depression and anxiety have high prevalences in hepatitis C, and are largely undetected and treated. Depression, but not anxiety, is associated with adverse experiences of illness.

Studies conducted in many countries the world has shown that fatigued driving contributes to appr... more Studies conducted in many countries the world has shown that fatigued driving contributes to approximately 3%-30% of road accidents. We conducted this study to estimate the prevalence of fatigue among taxi drivers and to assess the determinant of fatigue among taxi drivers. In this cross-sectional study, 300 urban and rural taxi drivers from Fez, Morocco were enrolled from January to May 2013. The mean score of Pichot scale was 13.59 ± 8.23. The mean duration of sleep was 6.78 ± 1.47 h per day. The mean rate of accident was 2.78 ± 1.12 per year. At the final model of the multiple linear regression we found an association statistically significant between Pichot scale and income, cardiovascular disorders, tobacco use, problems with colleagues, regularity of sleep and sleeping at the wheel. This study opened the door to many discussions about the professional drivers' conditions of work and we can argue that we should ameliorate the professional drivers conditions of work in Morocco.

The goal of this study was to investigate the prevalence of MBS and, in particular, the main factors that determine this syndrome in Moroccan patients with psoriasis. A case-control study has included 150 patients with psoriasis and 300 controls matched for age and sex, the MBS was defined according to the International Diabetes Foundation, and the severity of psoriasis was assessed by body surface area. Mild psoriasis was seen in 10.7%, 40.3% had moderate psoriasis, and 49% had severe psoriasis. MBS was higher in cases than in controls with statistical differences (44.7 vs. 2.7%, odds ratio [OR]: 26 CI: [12.4-54.3]; P = 0.000). Abdominal obesity and dyslipidemia were the only metabolic factors significantly related to psoriasis whereas diabetes, hypertension, smoking, alcohol consumption, and cardiovascular diseases were not significant. MBS increases with age in our patients with psoriasis, whereas there was no relationship between MBS and gender. Hypertension (P = 0.007), diabetes (P = 0.003), and increased level of triglycerides (P = 0.05) and high-density lipoprotein cholesterol (P = 0.003) were associated with the severity of psoriasis. Metabolic syndrome is an important comorbidity in patients with psoriasis, and vigilance and enhanced screening may be important in this population, especially patients with severe disease.

Resume Introduction Les recherches abordant la perception de la psychiatrie de liaison par les so... more Resume Introduction Les recherches abordant la perception de la psychiatrie de liaison par les soignants sont encore peu nombreuses au Maroc, la collaboration entre praticiens en psychiatrie de liaison s’avere necessaire et la perception des medecins influence leurs implications en psychiatrie de liaison. Objectifs Evaluer la perception de cette activite aupres des differents services medico-chirurgicaux, determiner l’implication, le niveau de collaboration et les attentes des praticiens, ainsi que les difficultes et les contraintes rencontrees. Methodologie Il s’agit d’une enquete realisee aupres du personnel medical au niveau du Centre hospitalier universitaire Hassan II de Fes en 2016, a travers un auto-questionnaire determinant la perception et la position des soignants a l’egard de la psychiatrie de liaison. Les donnees sont recueillies a l’aide d’un auto-questionnaire anonyme, comprenant 27 items. Nous avons inclus dans cette etude une population ciblee : les internes, les residents et les professeurs. Resultats La population totale des soignants participant a cette enquete est de 615 medecins, dont 111 professeurs, 436 residents, et 68 internes. Le taux de participation a ete de 80,32 %, l’âge moyen etait de 30,25 ans, avec 47,6 % (236) de femmes et 52,4 % (258) d’hommes. Le pourcentage des medecins ayant obtenu leurs doctorats en medecine au Maroc est de 89,3 % dont 85,9 % a la faculte de medecine de Fes et 92,3 % avaient beneficie d’un stage unique au service de psychiatrie au cours de leurs cursus universitaires, alors que 98,7 % n’ont jamais recu une formation continue dans le domaine de la psychiatrie. La moitie des medecins repondeurs jugent avoir des difficultes a prendre en charge les troubles psychiatriques sur le plan diagnostique, et 59,5 % (292) trouvent des difficultes pour traiter les patients avec des troubles psychiatriques, alors que 90 % (444) jugent ne pas pouvoir faire le suivi therapeutique. Seulement 35,6 % des medecins enquetes sont informes sur la mise en place de la psychiatrie de liaison mais seulement 10 % de ces medecins sont renseignes sur les modalites de deroulement de la demande de l’avis psychiatrique. Devant un trouble psychiatrique, 98 % des medecins gerent la situation en demandant un avis psychiatrique, cette demande est estimee urgente dans 87,1 % des cas. Les raisons de la non-sollicitation du psychiatre sont partagees entre les difficultes a reperer un trouble psychiatrique (51 %) et les difficultes liees a la demande de l’avis (49 %). Conclusion Notre etude a montre plusieurs difficultes percues par les praticiens, que ca soit pour le diagnostic, pour le traitement ou pour le suivi des patients qui presentent des manifestations psychiatriques. La collaboration entre differents praticiens constitue une obligation dans la prise en charge des patients. L’amelioration des connaissances a travers la sensibilisation et l’organisation de journees scientifiques regroupant les differentes specialites aidera a ameliorer la perception des medecins.

Results: The average age of the participants was 42, 2±14, 66 years. Introduction : l'objectif principal de ce travail est de décrire les connaissances et les pratiqu... more Introduction : l'objectif principal de ce travail est de décrire les connaissances et les pratiques vis-à-vis du cancer du col de l'utérus (CCU) et de l'infection à l'Human Papilloma Virus (HPV) des médecins généralistes exerçant aux centres de santé du réseau de soins de santé de base (RSSB). Méthodologie : étude transversale menée auprès des médecins généralistes exerçant dans tous les centres de santé de la Wilaya de Fès pendant le mois de mai 2010. Le recueil de l'information a été réalisé par un questionnaire auto-administré. Résultats : le taux de réponse était de 77,7 %. Seulement 25,6 % des médecins rapportent connaître la technique du frottis cervico-vaginal (FCV) et 7,9 %, toutes des femmes, pratiquent eux-mêmes les FCV. Les raisons de la non pratique du FCV se répartissent principalement entre l'absence de formation (69,0 %) et le manque de moyen de diagnostic (24,0 %). 89,0 % pensent que les antécédents familiaux de CCU représentent un facteur de risque, tous les médecins rapportent que les infections sexuellement transmissibles et la multitude des partenaires sexuels représentent des facteurs de risque du CCU, 94,9 % pensent que l'âge précoce du premier rapport sexuel est un facteur de risque et 97,4 % rapportent que les condylomes génitaux représentent un facteur de risque.76,4 % déclarent connaître l'existence d'un vaccin anti-HPV. Conclusion : le niveau de connaissances épidémiologiques des médecins exerçant dans le RSSB, sur le CCU et l'HPV, est relativement modeste et leur participation aux actions de dépistage et de diagnostic du CCU est assez faible. Des actions de formation et de sensibilisation sont très recommandées.

The aim of this study was to assess their current clinical practices concerning risk factors such as smoking, alcohol, diet and cancer screening. A self-administered questionnaire was sent to GPs working at the Fez Prefecture health centre (Morocco).Questions concerned primary prevention (smoking, alcohol, diet, sun exposure) and cancer screening (breast, uterine cervix, colon-rectum, prostate, skin cancers). The participation rate was 75.8%. The average age of GPs was 45.6 ± 6.8 years and 53.8% were female. Monitoring training on cancer prevention was reported by 25.6% of GPs. We noted a great diversity for screening, even for cancers with a consensus concerning the role of screening, such as breast, and uterine cervix cancers. Treatment Default is a major obstacle in the fight against the disease. The purpose of this study was to describe the impact of knowledge and attitudes toward TB on treatment adherence. Case-control study of 290 TB patients (85 defaulters and 205 controls). A defaulter was defined as a TB patient who interrupted treatment for two months or longer. Socio-demographic measurements, knowledge and attitude were collected by face to face anonymous questionnaire. Khi-square test was conducted to examine differences in TB attitudes and knowledge according to treatment adherence. The mean age of participants was 31.7 ± 12.0 years.
