In the present work we studied the modulation of the effect of urea on the renal (Na þ + K þ )ATP... more In the present work we studied the modulation of the effect of urea on the renal (Na þ + K þ )ATPase by cAMP. We observed that urea inhibits the ðNA þ þ K þ ÞATPase activity in a dose-dependent manner, reaching 60% of inhibition at the concentration of 1 M. This effect was completely reversed by dibutyryl-cAMP (dBcAMP) at 5 Â 10 À4 M. The effect of dBcAMP was mimicked by 50 units of the catalytic subunit of protein kinase A and completely abolished by 5 Â 10 À7 M H89, an inhibitor of protein kinase A. Addition of 1 M urea decreases basal phosphorylation of the immunoprecipitated ðNA þ þ K þ ÞATPase in 50%, with this effect completely reversed by 5 Â 10 À4 M dBcAMP. Furthermore, 5 Â 10 À4 M dBcAMP by itself induced ðNA þ þ K þ ÞATPase phosphorylation. Taken together these data indicate that cAMP could be, in addition to the organic solutes already known, an important physiological modulator of the deleterious effect of urea on enzyme activity. Aortoesophageal fistula induced by atherosclerotic thoracic aortic aneurysm is rare, but is usual... more Aortoesophageal fistula induced by atherosclerotic thoracic aortic aneurysm is rare, but is usually a fatal disorder, with few survivors reported. We report the case of a 72-yearold man with aortoesophageal fistula successfully treated in a two-stage operation. In the first stage, we performed resection and replacement of the aortic aneurysm with a prosthetic graft in situ, esophagectomy, cervical esophagostomy, and jejunostomy. After the patient recovered well postoperatively, a transmediastinal retrosternal interposition of the stomach was performed, with esophagogastroanastomosis in the cervical area, to re-establish the gastrointestinal tract. We include a discussion of the causes, diagnostic approach, management of the aorta and esophagus, and review of the literature. (J Vasc Surg 1999;30:1150-7.) 1150

PURPOSE: To report a series of patients with symptomatic internal carotid artery (ICA) occlusion ... more PURPOSE: To report a series of patients with symptomatic internal carotid artery (ICA) occlusion treated with angioplasty and stents. MATERIALS AND METHODS: From a consecutive series of 50 patients experiencing neurologic ischemic symptoms and shown by conventional ultrasonography (US) to have a total ICA occlusion, 16 patients (ages 45-79 years; mean, 63 y; 10 men) were selected between August 2006 to September 2008 to be treated with angioplasty based on discovery of an open ICA distal to the occlusion through contrast-enhanced echo Doppler imaging and/or multislice contrast computed tomography (CT). Angioplasty and stent placement were performed under cerebral protection. Follow-up duplex imaging was performed at 14 days and 3 months and every 6 months thereafter and CT follow-up was performed at 2-9 months; the mean follow-up period was 9.9 months.
