Our aim is to perform a retrospective cohort study to determine the incidence of symptomatic GERD following different types of bariatric surgery and which operations are considered a contraindication of GERD. In addition, we are attempting to identify the risk factors of GERD after bariatric surgery. Materials and Methods: Medical records of 729 patients hospitalized for bariatric surgery between January 2010 and June 2019 in Shamir (Assaf Harofeh) Medical Center were reviewed. Results: There were significant difference between the type of the bariatric procedure and the incidence of GERD symptoms after the operation. The incidence of symptomatic GERD in patients who underwent SG was 39.9% (p =0.0131). This was significantly higher comparing to 16.4% following roux en y gastric bypass, 23.4% following LAGB, and 11% following OAGB. 113 patients out of 718 had a positive swallow test and of these patient 71 developed GERD symptoms post-operatively without correlation to the degree of reflux at the swallow test but with statistically significant correlation to the type of operation especially for SG (P value <0.001) and to our knowledge this was never reported in the literature. Conclusion: SG is a good bariatric procedure option but should be contraindicated in asymptomatic reflux contrast swallow study and symptomatic GERD patients preoperatively because of high levels of symptomatic GERD post-operatively. Asymptomatic reflux at contrast swallow study pre-operatively should be considered a risk factor for GERD after the operation.

Inomhusmiljön är en viktig del av vårt liv, med tanke på att vi spenderar ungefär 90 % av vår tid... more Inomhusmiljön är en viktig del av vårt liv, med tanke på att vi spenderar ungefär 90 % av vår tid inomhus dagligen. Olika typer av föroreningar förekommer i inomhusmiljön, som kan bidra till hälsoeffekter och upplevd luftkvalité. På sistone har det diskuterats kring hur bra inomhusmiljön är i svenska skolor, som är en förutsättning för hälsa och inlärningskapacitet. Syfte Lärarna i träslöjd på Tunaskolan i Lund har under en längre tid känt besvär med luftkvalitén i just träslöjdsalen. De kontaktade genom Lunds kommun, institutionen för Ergonomi och Aerosolteknologi, och undrade om det fanns en möjlighet till att undersöka egenskaperna hos de luftburna partiklarna i träslöjdsalen. Syftet var att karaktärisera de luftburna partiklarna, mäta den personliga exponeringen för lärarna samt undersöka hur lärare och elever upplevde den fysiska arbetsmiljön och vilka symptom de hade. Effektiviteten av de nyinstallerade luftrenarna undersöktes också. Utifrån hela studien, utfärdades rekommendationer på hur träslöjdsalen bör utformas, och vilka åtgärder som kan göras för att reducera exponeringen för luftburna partiklar, och säkerställa en god arbetsmiljö för lärare och elever. Metod Mätningar genomfördes för att bestämma mass-och antalkoncentrationen och storleksfördelningen av partiklar i träslöjdsalen. För att kunna göra en jämförelsestudie gjorde vi mätningar i andra typer av klassrum, d.v.s. i syslöjdsalar och i vanliga klassrum. Vi genomförde även en spårgasmätning för att kunna bestämma luftomsättningen och se om allmänventilationen är tillräcklig med hänsyn till dagens krav. Genom vårt frågeformulär undersökte vi hur lärare och elever upplever inomhusmiljön (fysiska faktorer), samt undersökte vilka symptom de upplever. En okulär besiktning av ventilation, utformning av träslöjdsal samt städning genomfördes i kombination med intervjuer med lärarna och de anställda på Tunaskolan. Resultat och slutsatser Koncentrationen av luftburna partiklar i träslöjdsalen, visade sig var högre i jämförelse med resterande klassrum, d.v.s. i genomsnitt 151 µg/m³ och 19700 pt/cm³ i träslöjdsalen till skillnad mot 51 µg/m³ och 6700 pt/cm³ i syslöjdsalen. Personburna mätningarna visade att träslöjdslärarna exponerades för högre koncentration av luftburna partiklar än andra lärare, i genomsnitt 796 µg/m³ för träslöjdlärarna och 191 µg/m³ för vanliga lärare. Det observerades att städare exponeras för jämförbara koncentrationer med träslöjdsläraren. Montering av nya luftrenare, gav en minskning i antalkoncentrationen (pt/cm³) som domineras av partiklar mindre än 2,5 µm. Dock så blev masskoncentrationen (mg/m³) oförändrad vid montering av nya luftrenare, som domineras av de större partiklarna som kan bidra med problem i ögon, näsa, mun och hals. När det gäller allmänventilationen, visade resultaten att träslöjdsalen klarar dagens krav. Som ett resultat av denna studie rekommenderas följande åtgärder, för att minska exponeringen av luftburna partiklar i träslöjdsalen. Processventilation bör undersökas och dess effektivitet borde kontrolleras och säkerställas, eftersom det visade sig att de nyinstallerade luftrenarna inte reducerade de grövre partiklarna. För att underlätta städningen, skall all material, snickerier och lådor som för närvarande är placerade på golvet, fönsterkarmar och hyllor, placeras i separata skåp. Eleverna skall uppmuntras till att använda de mobila utsugen mer regelbundet och ofta, och för att förhindra större träbitar och verktyg från att sugas in, kan ett sorts galler placeras vid utsuget. Om möjligt bör maskinerna placeras i ett avskiljt rum, genom att bygga en innervägg, detta för att minska på mass-och antalkoncentrationen där eleverna arbetar. För framtida studier rekommenderas att en studie görs på bullerproblemet, med tanke på att det gav ett högt utslag bland de tillfrågade i enkätundersökningen.

INTRODUCTION: Endothelial shear stress (ESS) is a local hemodynamic factor that is dependent on v... more INTRODUCTION: Endothelial shear stress (ESS) is a local hemodynamic factor that is dependent on vessel geometry and influences the process of atherogenesis. As in vivo measurements of ESS are not possible, it must be calculated using computational fluid dynamics (CFD). In this feasibility study we explore CFD-models generated from coronary CT-angiography (CCTA) using an individualised blood viscosity and a pulsatile flow profile derived from in vivo measurements. MATERIALS AND METHODS: We retrospectively recruited 25 consecutive patients who received a CCTA followed by a coronary angiography including intravascular ultrasound (IVUS) and generated 3D models of the coronary arteries from the CT-datasets. We then performed CFD-simulations on these models. Hemodynamically non-relevant stenosis were identified in IVUS. They were isolated in the CFD-model and separated longitudinally into a half with atherosclerotic lesion (AL) and one without (NAL). ESS was measured and compared for both halves. RESULTS: After excluding vessels with no IVUS data or relevant stenosis we isolated 31 hemodynamically non-relevant excentric AL from a total of 14 vessels. AL segments showed consistently significantly lower ESS when compared to their corresponding NAL segments when regarding minimum (0.9 Pa, CI [0.6, 1.2] vs. 1.3 Pa, CI [0.9, 1.8]; p = 0.004), mean (5.0 Pa, CI [3.4, 6.0] vs. 6.7 Pa, CI [5.5, 8.4]; p = 0.008) and maximum ESS values (12.4 Pa, CI [8.6, 14.6] vs. 19.6 Pa, CI [12.4, 21.0]; p = 0.005). Qualitatively ESS was lower on the inside of bifurcations and curvatures. CONCLUSION: CFD simulations of coronary arteries from CCTA with an individualised flow profile and blood viscosity are feasible and could provide further prognostic information and a better risk stratification in coronary artery disease. Further prospective studies are needed to investigate this claim.

Methods A total number of 7 consecutive patients with suspected coronary artery disease who received CTA and invasive angiography with IVUS analysis were included in this study. CTA examinations were performed using a dual-source scanner. These datasets were used to build a 3D mesh model. CFD calculations were performed using a validated CFD solver. The presence of plaque was assumed if the thickness of the intima-media complex exceeded 0.3 mm in IVUS. Plaque composition was derived by IVUS radiofrequency data analysis. Results Plaque was present in 32.1% of all analyzed cross-sections. Plaque prevalence was highest in areas of low ESS (49.6%) and high ESS (34.8%). In parts exposed to intermediatelow and intermediate-high ESS few plaques were found (20.0% and 24.0%) (p<0.001). Wall thickness was closely associated with local ESS. Endto-end anastomosis of the portal trunk to the left portal branch is the conventional portal reconstruction in cases of right extended hepatectomy requiring resection of the portal vein bifurcation. This mandatory reconstruction may be challenging due to (1) vessel incongruence, (2) fragility of the left portal branch wall, and more importantly, and (3) the divergent orientation of the two vessels exposing to vascular twisting/kinking. We report here the first two cases of porto-Rex shunt, between the portal vein trunk and the left portal vein in the umbilical fissure during right extended hepatectomy for advanced extrahepatic biliary cancer: one following failed conventional portal reconstruction and one to achieve macroscopically complete resection.

The study dealt with the concept of measuring the quality of financial reports and the determinants and methods of measurement, according to a scientific theoretical framework based on a group of Arab and foreign scientific research. The study reached a set of conclusions, the most important of which is that the quality of financial reports is low in Iraqi companies. Therefore, the analysis of battery thermal management (BTM) of this type of batteries is of a great interest. This work aims at evaluating the effectiveness of different types of passive and active BTM systems. The electro-thermal behavior of the battery pack is characterized using 3D finite element method (FEM) modeling. The heat generation rate of battery cells during utilization is determined, so that the thermal behavior of the battery pack can be evaluated. The different BTM solutions are evaluated to determine the improvements in the pack thermal behavior. For further improvements, a new mixture of phase-change materials (PCM) is proposed and simulated. The mixture contains 75% of hydrogel and 25% of paraffin wax (PW). Endto-end anastomosis of the portal trunk to the left portal branch is the conventional portal reconstruction in cases of right extended hepatectomy requiring resection of the portal vein bifurcation. This mandatory reconstruction may be challenging due to (1) vessel incongruence, (2) fragility of the left portal branch wall, and more importantly, and (3) the divergent orientation of the two vessels exposing to vascular twisting/kinking. We report here the first two cases of porto-Rex shunt, between the portal vein trunk and the left portal vein in the umbilical fissure during right extended hepatectomy for advanced extrahepatic biliary cancer: one following failed conventional portal reconstruction and one to achieve macroscopically complete resection.

The study includes a sample of (13) Iraqi companies. These companies belong to an important group of different sectors represented by (banks, industry, insurance, agriculture, and tourism). The study dealt with the concept of measuring the quality of financial reports and the determinants and methods of measurement, according to a scientific theoretical framework based on a group of Arab and foreign scientific research. The study reached a set of conclusions, the most important of which is that the quality of financial reports is low in Iraqi companies. ESS assessment so far depended on invasive procedures. The aim of this study was to demonstrate the relationship between ESS and coronary atherosclerosis by using non-invasive coronary computed tomography angiography (CTA) for computation-al fluid dynamics (CFD) simulations. Methods A total number of 7 consecutive patients with suspected coronary artery disease who re-ceived CTA and invasive angiography with IVUS analysis were included in this study. CTA examinations were performed using a dual-source scanner. These datasets were used to build a 3D mesh model. CFD calculations were performed using a validated CFD solver. Therefore, the analysis of battery thermal management (BTM) of this type of batteries is of a great interest. This work aims at evaluating the effectiveness of different types of passive and active BTM systems. The electro-thermal behavior of the battery pack is characterized using 3D finite element method (FEM) modeling. The heat generation rate of battery cells during utilization is determined, so that the thermal behavior of the battery pack can be evaluated. The different BTM solutions are evaluated to determine the improvements in the pack thermal behavior. For further improvements, a new mixture of phase-change materials (PCM) is proposed and simulated. The mixture contains 75% of hydrogel and 25% of paraffin wax (PW).
