Results: Commonest mediastinal mass was of thymic origin followed by neurogenic, germ cells and o... more Results: Commonest mediastinal mass was of thymic origin followed by neurogenic, germ cells and others. Tissue or origin of the masses are : Thymic 32 cases, Lymphatics 13 cases, Mesenchymal (Dermoid) 17 cases, Bronchial Cyst 2 cases and Neurogenic 23 cases. There were 4 mortaity in our series 3 were of thymic origin and one was of huge dermoid. Conclusions: Mediastinal mass was not as rare as it is thought to be. Majority are asymtomatic. Commonest mediastinal mass was of thymic origin and an aggressive surgical approach gives gratifying results.

Interventional therapies and surgical procedures improve or normalize coronary blood flow greatly. Experimental data suggests that bone marrow derived Mesenchymal Stem Cells (MSCs) may contribute to the healing of Myocardial infarction. The change of a bacterium from a motile planktonic to a nonmotile long chain of growing cells is a complex, regulated process that depends on several factors. The probability of a biofilm-related infection occurrence is between 65 and 80%. This review critically evaluates the mode of biofilm formation on native tissues and orthopedics, dental, cardiac, and urological implants and vascular grafts. The combination of biochemical advancements with conventional microbiological techniques and the use of radio-labeled monoclonal antibodies in imaging techniques, with recent developments in the detection of these biofilm in vivo, would help in designing biomaterials that prevent bacterial adhesion and biofilm formation, and dislodge the formed biofilm, thereby accelerating the product development phase.

In addition, he also had absent right superior vena cava and a persistent left superior vena cava entering the coronary sinus.

The abstract is typically a short summary of the contents of the document.] Rajan Modi et al Pectoral and omental flap plasty 2 ABSTRACT Objective Mediastinal infection is a rare but life-threatening complication after cardiac surgery. Flap reconstructive procedures have made possible early closure and healing of sternal wounds. This study evaluates the outcome of post-sternotomy mediastinitis managed with pectoralis muscle mobilization and omental transposition. Methods 47 patients with a diagnosis of post-operative mediastinitis underwent either pectoral and/or omental plasty during the last 7 years. 20 patients had both omentum and pectoral muscle plasty, 17 had bilateral pectoral muscle plasty, 3 had pectoral muscle flap turnover and 6 had omentoplasty alone. The greater omentum is dissected based on the right gastro-epiploic artery. Pectoral flaps are raised by releasing the sternocostal origin and dissecting upto the posterior axillary line. All the unhealthy tissues are excised till they bleed freely. Omentum Rajan Modi et al Pectoral and omental flap plasty 3 is placed in the mediastinum and fixed at a few points. If the sternal margins are healthy, it is approximated with delayed absorbable sutures. The pectorals are sutured and the skin is closed with interrupted sutures. Continuous redivac suction drains are placed. A pressure dressing is applied using elasto-crepe bandage. The negative suction drains are removed when the drainage becomes less than 20 ml for 3 consecutive days Results 26 patients underwent procedure in the early post-operative period in the early post-operative period and 21 patients had plasty procedure after discharge (late). 75% were culture positive and 34% of them tested positive for Staphylococcus aureus which was the single largest organism to be cultured. The next largest group was coagulase negative staphylococci (10.6 %).The overall mortality was 15 %. Conclusion Post-operative mediastinitis is a potentially lethal complication following open heart surgery.
