Background: Cannabis is the most commonly consumed illicit drug around the world; in Germany, abo... more Background: Cannabis is the most commonly consumed illicit drug around the world; in Germany, about 4.5% of all adults use it each year. Intense cannabis use is associated with health risks. Evidence-based treatments are available for health problems caused by cannabis use. Methods: Selective literature review based on a search of the PubMed database, with special emphasis on systematic reviews, meta-analyses, cohort studies, randomized controlled trials (RCTs), case-control studies, and treatment guidelines. Results: The delta-9-tetrahydrocannabinol content of cannabis products is rising around the world as a result of plant breeding, while cannabidiol, in contrast, is often no longer detectable. Various medical conditions can arise acutely after cannabis use, depending on the user's age, dose, frequency, mode and situation of use, and individual disposition; these include panic attacks, psychotic symptoms, deficient attention, impaired concentration, motor incoordination, and nausea. In particular, intense use of high doses of cannabis over many years, and the initiation of cannabis use in adolescence, can be associated with substance dependence (DSM-5; ICD-10), specific withdrawal symptoms, cognitive impairment, affective disorders, psychosis, anxiety disorders, and physical disease outside the brain (mainly respiratory and cardiovascular conditions). At present, the most effective way to treat cannabis dependence involves a combination of motivational encouragement, cognitive behavioral therapy, and contingency management (level 1a evidence). For adolescents, family therapy is also recommended (level 1a evidence). No pharmacological treatments can be recommended to date, as evidence for their efficacy is lacking. Conclusion: Further research is needed to elucidate the causal relationships between intense cannabis use and potential damage to physical and mental health. Health problems due to cannabis use can be effectively treated.

Accordingly, regular cannabis consumption was associated with only minor effects on thyroid function tests in serum. While the TSH-levels remained stable, total T4- and total T3-levels were found to be significantly lower in chronic cannabis smokers, when compared with nonsmoking controls, but the levels remained mainly within normal limits. More importantly, the free T4-levels, which correlate more highly with the thyroid status than total T4-levels, were normal in cannabis addicts with a pattern of sustained and heavy consumption. Also in this cohort, thyroid function tests (TSH, total T3, free T4) were not significantly influenced by serum levels of Δ9-tetrahydrocannabinol (THC), the major psychoactive ingredient of cannabis. Altogether, these results argue against a relevant interplay between thyroid function and chronic, and even heavier, cannabis abuse.

An otherwise healthy 25-year-old patient with heavy cannabis-abuse suffered from an undiagnosed c... more An otherwise healthy 25-year-old patient with heavy cannabis-abuse suffered from an undiagnosed cannabis hyperemesis syndrome (CHS) over years, which characteristically was resistant to usual antiemetics. In an apparently last attempt at healing, opiates (morphine, methadone) were administrated and improved the CHS, however, this led to an at least as equally distressing and painful opiate withdrawal syndrome. In the controlled cannabis abstinence during the 2-week inpatient treatment of opiate addiction syndrome the CHS has not recurred. Opiates are not suited for the treatment of CHS because they are addictive and lead to respiratory depression in overdose.

Epidemiological studies show that the prevalence of cannabis use in Germany is approaching that o... more Epidemiological studies show that the prevalence of cannabis use in Germany is approaching that of countries known for their high consumption rate, such as Australia and some parts of the USA. In these countries, the resulting cannabis-related psychosocial damage has led to mainly psychotherapeutic treatment studies (conducted in the past ten years) that will be introduced here. According to these studies, the optimal treatment of cannabis-dependent adults would be a short intervention which consists of a combination of motivational-enhancement and cognitive-behavioural elements as well as individual case-counselling (c. f. "Marihuana Treatment Project"). In parallel to a clear consume-reduction over time, these interventions were associated with abstinence rates between 15 % and 22 % also in longer follow-ups (greater than 12 months). Alcohol use disorders (e.g. abuse and dependence) account for a plethora of consequences for affe... more Alcohol use disorders (e.g. abuse and dependence) account for a plethora of consequences for affected individuals and for a substantial proportion of the overall burden of disease for the community. To date, existing treatment options are either poorly known by doctors or they are not fully applied and only approximately 15 % of potential patients are treated with a mean latent period of 10 years between early symptoms and the first intervention. So-called S3 treatment guidelines were recently developed to close this gap. Representatives of more than 50 learned societies, families and patients were involved. A systematic literature search from 2005 to 2012 was performed and more than 120 recommendations were made. Financing came exclusively from those societies and the academic and treatment institutes involved. This article summarizes the recommendations pertinent for psychiatrists and include early detection and intervention, acute withdrawal and long-term psychotherapy and pharmacotherapy. Classical and new treatment goals are discussed. If the new guidelines were properly applied an increase in patients receiving treatment to 30-40 % could be expected, which would improve the quality of lives of affected persons and their families and in Germany would save several thousand lives per year

Cannabis use, abuse and dependence during the COVID-19 pandemic: a scoping review

As of 08/01/2022 among 57 evaluated epidemiological/clinical studies found in Pubmed-database, most evidence for how cannabis use patterns were influenced by the pandemic was given by two systematic reviews and 17 prospective studies, mostly involving adolescents. In this age group, cannabis use patterns have not changed markedly. For adults, several cross-sectional studies reported mixed results with cannabis use having increased, decreased or remained unchanged. Two cross-sectional studies demonstrated that the severity of adults´ cannabis dependence was either increased as a consequence of increasing cannabis use during the pandemic or not changed. Regarding the effect of cannabis use on COVID-19 outcomes, we found only five retrospective/cross-sectional studies.
