ArteFill is the first permanent injectable approved in 2006 by the FDA for nasolabial folds. It consists of cleaned microspheres of polymethylmethacrylate (PMMA) suspended in bovine collagen. Over the development period of 20 years most of its side effects have been eliminated to achieve the same safety standard as today's hyaluronic acid products. A 5-year follow-up study in U.S. clinical trial patients has shown the same wrinkle improvement as seen at 6 months. Long-term follow-up in European Artecoll patients has shown successful wrinkle correction lasting up to 15 years. A wide variety of off-label indications and applications have been developed that help the physician meet the individual needs of his/her patients. Serious complications after ArteFill injections, such as granuloma formation, have not been reported due to the reduction of PMMA microspheres smaller than 20 lm to less than 1% ''by the number.'' Minor technique-related side effects, however, may occur during the initial learning curve. Patient and physician satisfaction with ArteFill has been shown to be greater than 90%.

Although well established in the face and abdomen, these folding lines are often disputable on other parts of the body. Striae distensae always develop perpendicular to lines of tension, and their direction can be used to determine optimal incision lines. The authors examined photographs of 213 individuals with striae, and a composite diagram was created. This composite along with descriptions of Langer lines, Pinkus main folding lines, and Kraissl lines were compared with a clinical scar revision database and 276 images of incisions and scars from the Internet. Pinkus described the main folding lines in 1927 and Kraissl in 1951 recommended that incision lines be placed perpendicular to the direction of underlying muscles. Urethral Bulking Agents (UBAs) are injected locally as a minimally invasive procedure for Stress ... more Urethral Bulking Agents (UBAs) are injected locally as a minimally invasive procedure for Stress Urinary Incontinence (SUI) and are beneficial for properly selected patients. Many different materials have been developed and are available, although none so far meet all the requirements of an ideal agent. The first UBA was cross-linked bovine collagen (Contigen ®), followed by autologous fat injections, solid silicone particles (Macroplastique ®), microspheres covered with pyrolytic carbon (Durasphere ®), calcium hydroxyapatite (Coaptite ®), polyacrylamide hydrogel (Bulkamid ®), and dextran/HA copolymer (Zuidex ®). The latest product development is PDMS-U, a silicone gel bulking agent that polymerizes in situ (Urolastic ®). The ideal urinary bulking agent could consist of permanent microspheres, which immediately elicit a modest foreign body reaction along with the production of fibro-vascular tissue, which encapsulate every single microsphere individually and prevent their migration from the injection site. Polymethylmethacrylate microspheres have a successful history as dermal fillers used worldwide and can be safely injected submucosally at the urinary sphincter under direct vision, rather than peri-urethrally into the muscle like most of the present agents. Overall, short-term clinical results with most of the currently used urinary bulking agents are encouraging; however, longer follow-up results are often disappointing and retreatment is required. Proper patient selection and a safe, biocompatible and non-migrating bulking agent that elicits permanent fibro-vascular tissue formation at the injection site are paramount to successful treatment of stress urinary incontinence.

Patient satisfaction is the goal, but proof of the value of a particular product requires objective measurement. A wrinkle assessment scale was developed as a simple tool for use by plastic surgeons, dermatologists, and aesthetic surgeons who want to assess the changes resulting from injecting filler materials in their patients. By correlating the grade of the wrinkle in the reference photographs with the wrinkle in a patient's face, a classification of 0 to 5 is assigned. Reliability of the scale was assessed by "live" judgment of 76 wrinkles by nine observers. The same rating was given to 92.7 percent of all wrinkles. In a second trial, photographs from 130 wrinkles were presented to eight observers who rated 89.4 percent of all wrinkles equally. By applying mechanical protection of bony defects with, e.g. membranes or titanium mesh, soft tissue prolapse as well as pressure on bone transplants which contributes to partial resorption can be avoided. The use of a pressure-resistant, resorbable, macroporous sheet combines the advantage of protected bone regeneration and complete resorption of the implanted sheet. The macroporous structure facilitates capillary ingrowth from the surrounding soft tissue. The sheet is made of 70:30 Poly(L-co-DL)-lactate with thermoplastic character and can be used as a container for autologous spongiosa or other osteoinductive and -conductive bone graft substitutes.
