Uncomplicated pregnant women (n = 58) were divided into two groups according to State-Trait Anxiety Inventory grade (high trait [group I, n = 21] and low trait [group II, n = 37]) or Edinburgh Postnatal Depression Scale score (high score [group III, n = 15] and low score [group IV, n = 43]). An exploratory analysis of the textual data from the Maternal and Child Handbook was conducted using the text-mining method with the Word Miner software program. A comparison of the 'structure elements' was made between the two groups. The project of Paliadelis et al. supports novice writers to publish papers in rural medical university settings, including funded ‘workshops for writing’ and ‘off-campus overnight retreats for paper completion’. The number of ‘under review’ papers doubled, an amazing outcome. We commend the authors for their impressive results. Herein we summarise our 5-year project as a slightly different publication support system. The ‘Right for Publication’ (RFP) project and ours have similarities and differences. Jichi Medical University (JMU) graduates doctors who work in rural Japan. Many JMU graduates working in rural areas with little opportunity to receive advice on publishing, reported frequently experiencing ‘wrote a paper, rejection and trashcan’. VASH1 is expressed mainly in endothelial cells to inhibit angiogenesis, whereas VASH2 is expressed mainly in cancer cells to stimulate tumor growth. The aim of the present study was to establish neutralizing monoclonal antibody (mAb) against human VASH2 and apply it as an anti-cancer treatment. We previously raised mAbs against several synthetic peptides of hVASH1, and found that one of them exhibited neutralizing activity against hVASH1. Because of the similarity in the amino acid sequences between VASH1 and VASH2, we hypothesized that they shared the bioactive center. When we mutated 4 amino acids within the region, the mutant VASH2 lost its pro-angiogenic activity. Objectives: Invasive placentation (placenta accreta, increta, or percreta) presents significant c... more Objectives: Invasive placentation (placenta accreta, increta, or percreta) presents significant challenges at Caesarean section. Caesarean hysterectomy in such circumstances may result in massive blood loss despite surgical expertise. We reviewed two divergent surgical approaches: planned Caesarean hysterectomy versus a "conserving surgery" in which the placenta is left in situ after Caesarean section. Methods: We conducted a single-centre retrospective review of all patients who delivered with invasive placentation between 2000 and 2009. We included only patients with antenatally diagnosed invasive placentation and planned mode of delivery. Results: Twenty-six patients met the inclusion criteria. Caesarean hysterectomy was planned in 16 patients and conserving surgery in 10. Intraoperative and postoperative complications were comparable in the two groups. Four of 10 patients initially treated by conservative surgery required a subsequent hysterectomy for severe vaginal bleeding, coagulopathy, or sepsis. No pregnancies were subsequently reported in the conserving surgery group. Conclusion: An initial conserving surgical procedure is an option in patients with extensive invasive placentation, but it requires further monitoring for potential complications and carries a high subsequent hysterectomy rate. Résumé Objectifs : La placentation invasive (placenta accreta, increta ou percreta) pose des défis considérables au moment de la césarienne. Dans de telles circonstances, l'hystérectomie césarienne peut se solder en une perte sanguine massive, et ce, malgré l'expertise chirurgicale. Nous avons analysé deux approches chirurgicales divergentes : l'hystérectomie césarienne planifiée et la « chirurgie conservatrice » dans le cadre de laquelle le placenta demeure in situ à la suite de la césarienne. Méthodes : Nous avons mené une analyse rétrospective unicentrique de toutes les patientes ayant accouché en présence d'une placentation invasive entre 2000 et 2009. Nous n'avons inclus que les patientes chez lesquelles la placentation invasive avait été diagnostiquée avant l'accouchement et pour lesquelles un mode d'accouchement avait été planifié. Résultats : Vingt-six patientes ont satisfait aux critères d'inclusion. Une hystérectomie césarienne avait été planifiée chez 16 patientes et une chirurgie conservatrice, chez les 10 autres patientes. Les complications peropératoires et postopératoires étaient comparables dans les deux groupes. Quatre des 10 patientes initialement traitées par chirurgie conservatrice ont subséquemment nécessité une hystérectomie en raison de graves saignements vaginaux, d'une coagulopathie ou d'une sepsie. Aucune grossesse n'a par la suite été signalée au sein du groupe « chirurgie conservatrice ». Conclusion : Bien que la tenue initiale d'intervention chirurgicale conservatrice constitue une option chez les patientes qui présentent une importante placentation invasive, elle nécessite un suivi prolongé visant les complications potentielles et s'accompagne d'un taux élevé d'hystérectomie subséquente .

It has been reported that the concentration of free placental growth factor (PlGF) is decreased a... more It has been reported that the concentration of free placental growth factor (PlGF) is decreased and that of soluble fms-like tyrosine kinase 1 (sFlt-1) is increased before the onset of preeclampsia. However, no study has determined the reference values for sFlt-1 and free PlGF during pregnancy using a commercially available enzyme-linked immunosorbent assay (ELISA) kit. This longitudinal cohort study was undertaken to address this issue. Serum samples were collected from 148 women at 10, 18, 28, and 37 weeks of gestation. Preeclampsia occurred in 6 women: 4 women who delivered at <37 weeks of gestation, and 2 women who delivered at ≥37 weeks. The average and 90% confidence interval (90% CI) of the serum concentration of both sFlt-1 and free PlGF were determined in a total of 433 specimens from 148 subjects with 1 to 4 collections at 7 to 39 weeks of gestation, and were represented as quadric curves. The mean values (90% CI) of sFlt-1 (pg/ml) at 10, 18, 28, and 37 weeks of gestation were 413 (174-981), 296 (125-704), 413 (174-982), and 1,130 (477-2,690), respectively. The mean values (90% CI) of free PlGF (pg/ml) were 36 (14-89), 206 (83-515), 518 (207-1,290), and 354 (142-884), respectively. We also established the reference values for the ratio of sFlt-1/PlGF. Although various tissue macrophages possess high glucose-6-phosphate dehydrogenase (G6PD) activit... more Although various tissue macrophages possess high glucose-6-phosphate dehydrogenase (G6PD) activity, which is reported to be closely associated with their phagocytotic/bactericidal function, the fine subcellular localization of this enzyme in liver resident macrophages (Kupffer cells) has not been determined. We have investigated the subcellular localization of G6PD in Kupffer cells in rat liver, using a newly developed enzyme-cytochemical (copper-ferrocyanide) method. Electron-dense precipitates indicating G6PD activity were clearly visible in the cytoplasm and on the cytosolic side of the endoplasmic reticulum of Kupffer cells. Cytochemical controls ensured specific detection of the enzymatic activity. Rat Kupffer cells abundantly possessed enzyme-cytochemically detectable G6PD activity. Kupffer cell G6PD may play a role in liver defense by delivering NADPH to NADPH-dependent enzymes. G6PD enzyme-cytochemistry may be a useful tool for the study of Kupffer cell functions.

Our second aim was to compare the serum levels of sFlt-1, PlGF, and the sFlt-1/PlGF ratio in 81 women with PE which occurred at <32, 32-33, 34-35, and ≥36 weeks. Serum levels of sFlt-1 and PlGF were measured by automated immunoassays (Elecsys sFlt-1 and Elecsys PlGF). We constructed the normal reference ranges of logsFlt-1, logPlGF, and the log(sFlt-1/PlGF) between 19 and 38 weeks using 309 samples, which could be represented by a quadratic curve. The aim of this study was to evaluate the impact of the number of removed lymph nodes on recurrence-free survival (RFS) in stage I ovarian CCC. The subjects were patients with ovarian CCC who underwent surgery between January 1988 and December 2013. Clinicopathological variables were obtained from the medical records retrospectively. Statistical analysis using Kaplan-Meier method, log-rank test, and Cox proportional hazards model was performed. A total of 68 patients were entered into this study. Hysteroscopic transcervical resection (TCR) is often performed as fertility sparing treatment for... more Hysteroscopic transcervical resection (TCR) is often performed as fertility sparing treatment for atypical polypoid adenomyoma (APA) patients. However, TCR has the risk of uterine wall perforation, especially when the tumor extends deeply into the uterine muscle layer. We report an APA patient in whom it was impossible to completely resect the tumor by TCR, but laparotomy tumor resection followed by levonorgestrel-releasing intrauterine system (LNG-IUS) was successful. The patient was a 35-year-old nulligravida woman. We performed laparotomy tumor resection and inserted the LNG-IUS into uterine cavity just after surgery. Microscopic residual tumor was suspected based on histopathological findings. However, the patient has not relapsed for 26 months, even though the LNG-IUS was removed after 6 months. To compare serum levels of angiogenesis-related factors between 14 women with HELLP (hemolysis, e... more To compare serum levels of angiogenesis-related factors between 14 women with HELLP (hemolysis, elevated liver enzymes and low platelet count) syndrome and a woman with acute fatty liver of pregnancy (AFLP). Serum samples were collected in 2004-2008 and 2013-2016. The levels of soluble fms-like tyrosine kinase 1 (sFlt-1) and placental growth factor (PlGF) were measured by an automated electrochemiluminescence immunoassay using Elecsys sFlt-1 and Elecsys PlGF. After logarithmic transformation, levels of sFlt-1, PlGF and the sFlt-1/PlGF ratio in a woman with AFLP were compared with those in women with HELLP syndrome, using the one-sample t-test. At 37 weeks of gestation, a patient was diagnosed with AFLP based on Swansea criteria (showing six features including elevated transaminases), and she also showed a duodenal ulcer with active bleeding, thrombocytopenia and hypertension.
