The purpose of this study was to analyze the functional results after circumferential pharyngolaryngectomy (CPL) and flap reconstruction, in order to offer decisional guidelines for the choice of the most effective reconstructive method. We performed a retrospective analysis on the medical records of patients submitted to reconstructive surgery after CPL from July 1991 to November 2011. 75% of the 94 patients underwent reconstruction with a free flap (group A), while 25% underwent reconstruction with a pedicled flap (group B); 80% of patients in group A and none in group B were discharged with a free diet; 14% of patients in group A and 26% in group B were unable to resume oral feeding and were discharged with NG-tube or PEG. Electrochemotherapy is a tumor ablation modality providing delivery into the cell interior of imp... more Electrochemotherapy is a tumor ablation modality providing delivery into the cell interior of impermeant or poorly permeant chemotherapeutic drugs such as cisplatin and bleomycin. A locally applied electrical field enhances the membrane permeability allowing intracellular accumulation of the chemotherapeutic agent. The aim of the study was to evaluate the effectiveness of ECT for the treatment of a group of patients affected by recurrent of extended primary head and neck cancer and not suitable for standard therapeutic options. From April 2009 to January 2011, we treated with electrochemotherapy a total of 15 patients with head and neck cancers, 13 with squamous cell carcinoma, 1 with basaloid carcinoma and 1 with Merkel cell carcinoma. Electrical pulses were delivered to 33 lesions (3 primaries, 30 recurrences) after an intravenous bolus injection of a dose of 15,000 IU/m2 of bleomycin. Elettrochemioterapia (ECT) nei tumori testa e collo: un'alternativa ai trattamenti convenzion... more Elettrochemioterapia (ECT) nei tumori testa e collo: un'alternativa ai trattamenti convenzionali nelle forme avanzate L'elettrochemioterapia (ECT) è una modalità di trattamento oncologico ottenuta favorendo il trasporto di farma-ci chemioterapici come il cisplatino e la bleomicina ai quali la membrana cellulare normalmente risulta imper-meabile o scarsamente permeabile. Un campo elettrico applicato localmente modifica la permeabilità di membrana permettendo l'accumulo intracellulare dell'agente chemioterapico. Da Aprile 2009 presso la nostra clinica sono stati trattati con ECT un gruppo di pazienti affetti da neoplasie della testa e del collo ricorrenti o da tumori primitivi particolarmente estesi e quindi non suscettibili in prima istanza alle opzioni terapeutiche standard. Gli impulsi elettrici sono stati applicati a 33 lesioni in 15 pazienti dopo iniezione endovenosa in bolo di una dose di 15000 IU/m 2 di bleomicina. In questa sede presentiamo 3 pazienti trattati ...
