Aggressive surgery remains a potentially efficacious anti-tumor therapy but is rarely performed because of its possible morbidity and mortality. The purpose of this work is to describe a case of PNETs who underwent aggressive multivisceral pancreatic resection and the literature's review. Case report: A 52-year old man with no medical history, who presented for abdominal pain, recent constipation and intermittent rectal bleeding. Clinically, we found a tender mass of 10x7 cm in the left upper quadrant. Colonoscopy revealed an irregular lesion of 2 cm in the splenic flexure with histological diagnosis at biopsies suggesting a pancreatic neoplastic origin. Computed tomography (CT) of the abdomen demonstrated a large mass in the left pancreas invading the spleen and its vessels, the adjacent left transverse colon with sign of segmental extrahepatic portal hypertension. There were no liver metastases or suspected adenopathies. He underwent laparotomy with extended resection of the left pancreas, the spleen and the left colon. Histological examination confirmed a well differentiated PNET with Ki67 < 2%. The post operative course was uneventful. After 16 months of follow, the patient remains well. There is no sign of recurrence at CT. Conclusion: Many retrospective studies and the European neuroendocrine Tumors' (ENETs) guidelines claim that debulking and aggressive surgical resection in locally advanced PNETs should be considered in selected patients because it is technically feasible with encouraging disease-specific survival.

A particular trellis-coded modulation without parallel branches was chosen and a special mapping rule developed for the channel signal assignment so that differential coherently equivalent sequences were eliminated. On the receiver side, neither pilot signal nor training sequence is needed for synchronisation purposes. The detection process involves two steps. The first one consists of seeking for the initial carrier phase using a particular noncoherent detection metric. The second one uses the conventional Viterbi decoding algorithm in conjunction with an iterative phase updating. Simulation results show that the performance of this new NCC-32QAM is very close to that of the coherent 32QAM even for low signal-to-noise ratio. Indeed, only 0.3 dB is lost compared with the coherent trellis-coded 32QAM for a bit error rate of 10 À6 .

In this paper, the authors will study link adaptation requirements to allow the requested multimedia quality-based communications. The efficiency of rate control diagrams is linked to the fast channel variation response. A new dynamic time-based link adaptation

Therefore, we derive the analytical expressions of Fisher information matrix entries that assess the optimal variance of any unbiased SNR estimator. Based on statistical Monte Carlo computing method, simulation results are drawn from several constellation densities and observation window sizes. For the linear modulation schemes used here, it is shown that the lower bound is as higher as the modulation order increases. The derived bound provides an efficient standard for evaluating the performance of any unbiased non-data aided (NDA) SNR estimator from linear modulation signals over flat Rayleigh fading channel (FRFC).

The numerical Monte Carlo method is used to compute the true CRLB from general M-PSK modulation signals over FRFC with complex additive white Gaussian noise. The efficiency of the proposed method is made obvious through the assessment of the true CRLBs from both symmetric and asymmetric 8-PSK modulation signals for various observation window sizes. The simulation results show that optimal NDA carrier phase recovery is harder to acquire from symmetric modulation signals for signal-to-noise ratio (SNR) range up to 5 dB, but depends on the constellation density, only for SNR range superior to 5 dB. For both constellations the CRLB decreases as long as the observation window size increases. The derived bound provides an efficient standard for evaluating the performance of any unbiased NDA carrier phase estimator from general M-PSK modulation signals over FRFC. The method presented here stands useful for general M-ary quadrature amplitude modulation signals as well.

The proposed concatenated encoder takes advantage of both the high coding gain from its outer-coder and the ease of the detection process from its inner-coder. Simulation results demonstrate the good performance of the proposed scheme (4D-8-PSK-TCM-DSTBC) against the well-known DSTBC scheme for the same spatial diversity and similar signal constellation. The 3 dB performance gap usually involved by symbol by symbol differential detection is also observed between the presented non-coherent detection method and the coherent detection (4D-8-PSK-TCM-STBC) recently published.

We derive for the first time the expression of the Cramer-Rao lower bound for non-data aided (NDA) CFO estimation from trellis coded modulation (TCM) transmission over flat Rayleigh fading channel (FRFC). Existing NDA carrier frequency synchronizers based on a cyclostationary approach are then studied and generalized for encoded transmission. Behavior and performances of the estimation algorithms are presented through numerical results and are compared to the derived bounds.

We develop here a novel MIMO channel estimation algorithm that exploits a pilot symbol assisted modulation (PSAM) which has been proven to be effective for fading channels. The proposed concatenated scheme takes advantage of both the high coding gain from its outer-coder and the ease of the channel estimation from the use of PSAM technique. Simulation results demonstrate the good performance of the proposed MIMO scheme against the perfect coherent counterpart for the same spatial diversity.

General CRLB expressions for carrier frequency offset, carrier phase, symbol timing and signal to noise ratio (SNR) estimation are analytically developed going through the computation of the Fisher information matrix (FIM) entries. We consider both uncoded and coded transmissions spoiled by complex additive white Gaussian noise (CAWGN). Assessment of the derived bounds is obtained for M-PSK linearly modulated and convolutionally encoded signals using Monte Carlo (MC) integration technique.

This method is based on a special phase metric that performs an absolute minimum for the carrier phase offset. In particular, it does not require any modification of the modulation structure. As a result, the non-coherent detection process is easy even for modulations with large QAM constellations. Simulation results will demonstrate the good performance of the estimator described, which is superior to that of conventional blind phase estimation methods.
