We report the case of a patient treated with femoro-femoral V-A ECMO who developed a sudden, significant asymmetric decrease in cannulated limb regional oxygen saturation, rSO2, signal, despite the presence of a DPC. This finding, noticed within 6 hours after the implementation of extracorporeal cardiopulmonary resuscitation (ECPR) in a patient experiencing refractory intra-hospital cardiac arrest (IHCA), suggested ongoing limb ischemia. After careful monitoring, a clot was detected in the line supplying DPC, which appeared completely kinked at dressing removal. Line patency and reperfusion were promptly re-established. No additional ischemic episodes occurred, and the patient was successfully weaned from extracorporeal support, and experienced full functional recovery, with ...

Trustworthy TDI Clinical Practice Guidelines (CPGs) can assist clinicians in making a proper diagnosis, and guide them to the most appropriate therapy for every case. The aim of this study was to identify and evaluate the quality of CPGs for the diagnosis, emergency management and follow-up of TDIs. A systematic search was carried out in MEDLINE, EMBASE, Epistemonikos, Trip database, CPG’ websites, and dental societies to identify documents providing recommendations for the emergency and sequelae management of TDIs. Reviewers assessed the included guidelines independently and in duplicate, using the AGREE II instrument. T-student or ANOVA tests were used to determine the attributes of CPGs associated with the total score in AGREE II. Ten CPGs published between 2010 and 2020 were included, mostly from Europe (n=6). The overall agreement between reviewers was very good (0.94; ...

Therefore, training and technique reviews became necessary. Objective: this article aims to propose the application of the Rapid Cycle Deliberate Practice (RCDP) strategy for the training of anesthesiologists in orotracheal intubation in people with confirmed or suspected COVID-19 and to present a guide for the application of this strategy in this situation. Method: This is a methodological study presenting the theoretical and operational aspects for the Rapid Cycle Deliberate Practice application and a guide constructed from the search for evidence published in journals and official recommendations published by the institutions linked to the Brazilian and international health area. Results: the main theoretical aspects reported are related to the three principles on which Rapid Cycle Deliberate Practice is based: maximizing time in deliberate practice, targeted feedback, and explicit psychological security. As for the operational aspects, it is highlighted that the training must be carried out with a maximum of six people. An error must be interrupted, prescriptive feedback must be given, and the task must be performed again until the participant reaches mastery. As for the procedure technical specificity, a guide to the application of the rapid cycle deliberate practice is presented with the sequence for the adequate airway management of hypoxemic patients with suspected or confirmed COVID-19 infection. it is concluded that the assessed instructional strategy showed to be promising for the training with mastery learning in all professionals who will perform the procedure of orotracheal intubation while facing the severe forms of COVID-19, minimizing the risk of contamination.

We focus on specific areas predominated by the wine, forestry and salmon industry. Together with the above we present the most important political expressions that relate to each of the conflicts treated at each of the territories. We selected three places as case studies, areas where we do ethnographic work as a research strategy. We conclude identifying and reflecting on the commonalities and differences between the economic and the political in the rural territories.

A total of 100 nonsmoking volunteers from a central region of Mexico provided a urine sample before entering a discotheque and another sample an average of 6 hr after the end of exposure. Concentrations of cotinine and its metabolite, trans-39-hydroxycotinine, were measured in the urine by liquid chromatography-mass spectrometry. In females the average preexposure level of urinary cotinine was 2.2 ng/ml, and the average postexposure level was significantly higher, at 15.7 ng/ml. In males the average preexposure level of cotinine was 3.7 ng/ml, compared with 49.1 ng/ml in the postexposure assessment. The highest postexposure values were found in men younger than 22 years old with a value of 469.5 ng/ ml. Exposure to secondhand smoke is a serious health risk. Our findings are important given that many of our subjects were exposed to substantial amounts of secondhand smoke in discotheques, as evidenced by the high urinary cotinine and 39-hydroxycotinine concentrations. These findings support the need to prohibit smoking in discotheques to protect nonsmokers' health.

The analysis includes different drive topologies, shovel duty cycles for a typical truck pass loading and energy regeneration capabilities. Special attention is given to the interaction between the high power shovel drives and the grid: harmonics injection, reactive power requirements, and voltage regulation. This paper presents operational experiences obtained in copper mine industries. The main characteristics of the newest shovels with Active Front End (AFE) at the line side are also included. Finally, a critical evaluation of the different topologies is presented, showing that the shovel drive operation strongly affects the behavior of the power distribution system and that the introduction of the AFE technology produces an important performance improvement.

The defoliating leaßet-roller Episimus utilis Zimmerman was selected as potential biocontrol agent of this invasive species. The objectives of this study were to determine development rate and survival of E. utilis at seven constant temperatures (10, 15, 20, 25, 30, 33, and 35ЊC) and generate prediction maps of the number of generations per year this species may exhibit in the United States. The rate of development of E. utilis as a function of temperature was modeled using linear regression to estimate a lower developmental threshold of 9.6ЊC and the degree-day requirement of 588. The Logan nonlinear regression model was used to estimate an upper developmental threshold of 33ЊC. Cold tolerance of E. utilis was examined using all insect stages, and each stage was exposed to three constant temperatures (10, 5, 0ЊC) for 0.5, 1, 2, 4, and 8 d (or until all insects died). The pupal stage was the most cold tolerant with 100% mortality after 12 d at 0ЊC. The pupal lethal times at 5 (Ltime 50 ϭ 10 d, Ltime 90 ϭ 28 d) and 0ЊC (Ltime 50 ϭ 5 d, Ltime 90 ϭ 9 d) were used to generate isothermal lines to predict favorable regions for E. utilis establishment. A GIS map was generated to predict the number of generations of E. utilis (range, 0.5Ð9.8) across all Brazilian peppertree range in the United States. The potential for establishment of E. utilis and its probable distribution in the continental United States was examined.
