Neuropathic pain has traditionally been associated with C fibres; however, as Valleix suggested i... more Neuropathic pain has traditionally been associated with C fibres; however, as Valleix suggested in 1841, followed by Tinel in 1916, among others, neuropathic pain is also conveyed by the large diameter myelinated Aβ fibres, to such an extent that, in 2009, Marshall Devor offered the concept of Aβ pain in Exp Brain Res. The previously indisputable « gate theory » of pain added to the confusion: Aβ fibers will now be considered exclusively as inhibitory fibers of pain and will lose their former connotation of etiology of pain. The term pain fibre frequently used in Transcutaneous Electric Nerve Stimulation (TENS) adds to the confusion (Houghton et al., 2010) : while C fibres are involved in transmitting painful stimuli, they are not the fibres solely responsible for generating pain.

The first well-known aetiology of neuropathic pain is small fiber C lesion. The second one has been recently reproposed: aβ pain (Devor, 2009). Aβ fibers have been considered as a pain inhibitor, since the Gate Control Theory (Melzack & Wall, 1965). But at the beginning of the twentieth century the boundaries of the altered skin sensibility were already carefully determined (Trotter & Davies, 1907); at the time, these fibers were considered as a pain inducer. In 1935, Titus von Lanz established a concept of aesthesiology: the largest territory of cutaneous distribution of the nerves. So could we dream of a life without pain ? Certainly not, when it comes to its function as a necessary protection mechanisms that accompanies us through our development. But when it comes to pain that becomes a conqueror who invades our bodies we dare to dream of a life without it.
