Intervienen en él tres actores en constante interacción: la madre, el niño y la placenta. Actualmente, su comprensión está limitada en el plano metodológico tanto por los límites justificados de la investigación en la mujer embarazada como por la inadecuación de los estudios realizados en los modelos animales. Parece estar basado en mecanismos endocrinológicos, como el balance progesterona/estrógenos, la hormona liberadora de corticotropina, e inmunológicos, como el NF-B y la familia de los receptores tipo Toll. Existen numerosas moléculas efectoras, desde las prostaglandinas hasta las proteínas de membrana acopladas a proteínas G con el fin de conectar la señal en las células. El conjunto de estas acciones actúa, entre otras formas, por contigüidad entre los diferentes microentornos que son el miometrio, el cuello uterino, las membranas fetales y la placenta. Parece ser que diferentes elementos exógenos, infecciosos pero también ambientales, pueden interferir con estos mecanismos y provocar el inicio del trabajo del parto. Es indispensable conocer mejor estos mecanismos para prevenir partos prematuros o para manejar mejor el parto.

Endometriosis is a chronic inflammatory disorder with a prevalence of six to ten percent in women... more Endometriosis is a chronic inflammatory disorder with a prevalence of six to ten percent in women of childbearing age. As long as the aetiology of endometriosis is not fully understood and the disease has no definitive treatment, an examination of the environmental factors or interventions that could modify or cure endometriosis would greatly benefit women suffering from this chronic condition. This literature review utilized the electronic databases PubMed, EMBASE, and MEDLINE until February 2021. Studies indicate that fish oil may have a positive effect on reducing endometriosis-related pain due to the effects of pro-inflammatory prostaglandins derived from omega-3 fatty acids. The same effect was seen with the introduction of antioxidant vitamins C, D, and E. There is clinical viability of a low fermentable oligo-, di-, and monosaccharides and polyols diet to successfully reduce the symptoms of patients who suffer from both endometriosis and irritable bowel syndrome. Despite the low level of evidence, there are frequent associations between endometriosis and gastrointestinal conditions in addition to the influence of various nutritional factors on the disease. The management of endometriosis requires a holistic approach focused on reducing overall inflammation, increasing detoxification, and attenuating troublesome symptoms. A dietician may provide great benefit in the management of these patients, especially at younger ages and in early stages. High-level evidence and welldesigned randomized studies are lacking when it comes to studying the effect of lifestyle and dietary intake on endometriosis. Inarguably, further research with a more extensive focus is needed.

We report the design and the evaluation of a three dimensions (3D) video training for LP procedure.MethodsWe recorded a stereoscopic 180-degrees 3D video from two LPs performed in clinical setting in Fernand Widal Lariboisière University Hospital, Paris, France. Video was administrated to third-year medical students as well as to a residents and attendings group during LP simulation-based training sessions. ResultsOn 168 participants (108 novice third-year medical students, and 60 residents and attendings with prior LP experience), satisfaction after video exposure was high (rated 4.7 ± 0.6 on a 5-point scale). Uterine arteriovenous malformations (UAVMs) are not common and are not well known. They are mainl... more Uterine arteriovenous malformations (UAVMs) are not common and are not well known. They are mainly manifested by hemorrhages that can be life-threatening. Their diagnosis is based mainly on ultrasound coupled with Doppler flow, as well as pelvic magnetic resonance imaging and angiography. In the absence of a consensus, and based on our experience, we discuss the place of hysteroscopy in the therapeutic strategy of UAVM. We reported 3 cases of UAVM, and we performed a review of the literature concerning the diagnosis and treatment of this pathology. We finally collected the advantages of the embolization-hysteroscopy sequence. In fact, the data suggest that hysteroscopy is a feasible and safe alternative treatment modality for UAVM. We believe that the resection of UAVM can be done under optimal conditions with a reduced risk of bleeding. Hysteroscopy would allow for optimal evaluation of the uterine cavity after treatment as well as the possibility of applying an anti-adhesive gel. In addition, the combination of hysteroscopy and embolization would require less healing time and shorter follow-up than embolization alone. It could also reduce the number of patients lost to follow-up.
