We report a series of 107 patients who underwent mandibulotomy at the Gustave-Roussy Institute, F... more We report a series of 107 patients who underwent mandibulotomy at the Gustave-Roussy Institute, France for cancer of the oral cavity and orophaynx between 1998 and 1996. The most common tumor site was the base of the tongue. Most cases were stage T2 or T3. Postoperative radiotherapy was employed except in those who had previous irradiation, either for the same tumor (12 cases) or another cancer (16 cases). The different types of mandibulotomies and osteosynthesis methods were analyzed in relation to postoperative course and functional and oncological outcome at mean follow-up of 6 years. Osteitis was the most frequent complication (14%) requiring hemimandibulectomy in 5 cases. Complications occurred particularly when osteosynthesis was in the pre-or postoperative irradiation field. Osteosynthesis was definitive in 79.4% with good functional results in 76.5%. Previous radiotherapy or use of flaps for closure gave poor function results. Il s'agit d'une etude retrospective portant sur la preservation laryngee dans les cancers... more Il s'agit d'une etude retrospective portant sur la preservation laryngee dans les cancers de l'endolarynx, par chimiotherapie d'induction et radiotherapie en cas de bonne reponse. Entre 1985 et 1995, 104 patients sont entres dans ce protocole therapeutique, a l'institut Gustave Roussy (87 patients) et au CHU de Limoges (17 patients). Pour l'ensemble de la population la survie a ete de 76 % et 69 %, a 3 ans et 5 ans, avec un taux de preservation laryngee de 36 %. Dans cette serie retrospective, le seul facteur pronostic retrouve a ete la fixite arytenoidienne avant chimiotherapie (66 % et 55 % a 3 et 5 ans vs 85 % et 82 %, p< 0,004). Les echecs loco-regionaux ont ete plus frequents (33 % vs 15 %, p<0,03) et la preservation de la fonction laryngee plus faible (18 % vs 51 %, p<0,01) pour les patients dont l'arytenoide etait initialement fixe (49 patients), par rapport aux autres patients (55 patients). The free jejunal autograft (FJA) has become a common procedure for pharyngeal reconstruction afte... more The free jejunal autograft (FJA) has become a common procedure for pharyngeal reconstruction after circumferential pharyngolaryngectomy. In order to evaluate the postoperative outcome and the functional and carcinologic results, we retrospectively reviewed 83 cases of reconstruction with FJA. Fifty-one patients had received no prior radiotherapy, and 25 had received prior radiotherapy for their hypopharyngeal tumor or for another previous primary. Seven patients underwent a secondary reconstruction. In the postoperative course, there were 2 postoperative deaths, 4 graft failures (5%), and 11 salivary fistulas. The median time to removal of the nasogastric tube was 16 days, and to discharge, 23 days. Forty-eight patients received postoperative radiotherapy, with good tolerance. At 1 year postoperatively, 98% of the patients were able to eat a solid or soft diet. The postoperative radiotherapy did not impair the quality of the functional results in a long-term assessment. They then describe a case of a malignant tumour of the parotid which was shown by histological examination to be a metastasis of a carcinoma of the kidney, this being the first manifestation of the primary tumour. Finally, the authors review the literature concerning metastases of carcinoma of the kidney to the ENT area. They feel that the great rarity of parotid localisations is due to the fact that they may be confused with an acinous carcinoma of the parotid if histochemical stains are not used.

The authors report the results of a series, between 1969 and 1978, of 42 patients who had receive... more The authors report the results of a series, between 1969 and 1978, of 42 patients who had received ligation to the carotid arterial trunk. In 36 patients, the ligation was performed as an emergency measure, often under extemporary circumstances, because of massive hemorrhage in individuals who had received surgery after previous radiotherapy. The complications after applying these ligatures have been catastrophic for many reasons. Fifteen rapid recurrent hemorrhages occurred, leading to the deaths of 13 patients. Twenty-two patients developed major neurological problems and 12 of these subsequently died. The third complication observed was the onset of extensive necrosis. This necrosis was the direct result of applying a ligature, as a last resort, to the carotid trunk supplying a vascular territory already compromised by previous radiotherapy and surgery. The authors make a report on a series of 173 supraglottal horizontal laryngectomies carried out f... more The authors make a report on a series of 173 supraglottal horizontal laryngectomies carried out from 1970 to 1984 at the Gustave-Roussy Institute of Villejuif. After recalling of operability conditions and tackling characteristics of this series are quoted variants required from tumours localisations diversity and importance of extension. Then are treated clinical and anatomo-pathological classification as briefly surgeries results. This work is specially concerning in survival by analysing different topographical clinical and histological criteria. Survival of 173 patients, having gone through an supra-glottal laryngectomy is 45.5% at 3 years and 31.3% at 5 years. At least, reasons of failure are analysed. This kind of partial surgery caring of voice, should be proposed to patients affected by vallecula or epiglottis carcinomas as regards of good functional and improved carcinological results.

A direct approach to the plate and the bony and aponeurotic contents of the oval fossa made it possible to seal this off and this was checked surgically eighteen months later. On this occasion a general survey was carried out into the usual circumstances in which such a discovery is made, pathological concepts and treatment for these accidents. It would appear that unusual permeability may be found either in the internal auditory meatus or in the cochlear aqueduct. Therapeutic procedure depends on circumstances. Blocking the oval fossa is not always effective and should be reserved for cases in which abnormalities impair function. Choice between blocking the internal auditory meatus and eradication of the cochlear aqueduct depends to some extent on the information supplied ...

Olfactory esthesioneuroma is a rare malignant tumor arising in the olfactory epithelium. Forty ca... more Olfactory esthesioneuroma is a rare malignant tumor arising in the olfactory epithelium. Forty cases observed at the Institut Gustave‐Roussy from 1956 to 1987 are reported. This tumor usually grows slowly and is usually local, but it is important to be aware of the possibility of lymph node involvement (17%) and, particularly, of rapid development of distant metastases (25%), usually within 6 months. CT scan, and more recently, NMR have proved to be of value in choosing the surgical approach. In view of the usual point of departure, a combined neurosurgical and transfacial approach seems to be a satisfactory approach for obtaining oncological control of the lesion. The role of chemotherapy is discussed. The main prognostic factors seem to be the size of the lesion, the intracranial extension, and the lymph node involvement.
Between 1958 and 1973, 131 patients with carcinoma of the posterior pharyngeal wall were treated ... more Between 1958 and 1973, 131 patients with carcinoma of the posterior pharyngeal wall were treated at the Institute Gustave-Roussy (127 men, 4 women, mean age 61 years). Nine had a tumor of the oropharynx, 37 of the hypopharynx and 85 of the oro-hypopharynx. Nine had Tâ tumors, 19 Tâ, and 103 Tâ (TNM--UICC 1975). In all 122 patients were treated by irradiation: 83% were Tâ lesions, 81% were more than 4 cm in diameter, 80% were ulcerated, and 42% were staged Nâ. The survival rate of these patients was very poor (3% at five years). The nine patients who had been treated by surgical excision had a better survival rate and three are alive at 5 years, but the initial prognostic factors were better in this group. It seems that the indications for surgery should be widened to increase the survival rate in these poor prognosis cancers.
