Это взгляд на медицину «снаружи». Сделана попытка показать,
что все пси-дисциплины в силу специфики занимающиеся поведением человека, неизбежно втягиваются в политико-экономические манипулятивные процессы, становятся инструментами биовласти (М.Фуко). Эти процессы меняют границы психиатрии – предмет извечных споров психиатров – и формируют социальный облик эпохи.

ABSTRACT
The discussion on the methodological limitations of the quasi- measurement of human feelings and
sensations, started by the author in the journal Neurology bulletin, continues. There are a number of reasons for
accepting/rejecting and understanding the principles and limitations of clinical epidemiology/evidence-based
medicine (EBM) to the point of deliberate distortion. It is shown that the ideas of “freedom” and “creativity”
in medicine, are ways of avoiding control and the realization of biopower. A concrete example, the study of the
efficacy of antidepressants, shows how it is possible, either unintentionally or intentionally, to obtain a false
result without following the principles and tenets of EBM.

В этом качестве ДИС является инструментом перекладывания ответственности за выбор вмешательства с продавца медицинских услуг на потребителя и является контрактным договором, позволяющим избежать судебной ответственности и всевозможных потерь с обеих сторон. Комплекс проблем вокруг ДИС можно объяснить существенным различием ситуации эксперимента (для которой оно создавалось первоначально) и повседневной клинической практики. Закон о ДИС не имеет механизмов индивидуального применения, ибо не учитывает вопросов психологии принятия решений. Abstract: The Informed Consent (IC) procedure is considered as a legal construct, a product of liberal economics. As such, IC is a tool for shifting responsibility for the choice of intervention from the seller of health care services to the consumer and is a binding contract to avoid legal liability and all sorts of losses on both sides.
The set of problems surrounding the IC can be explained by the significant difference between an experimental procedure (for which it was originally created) and everyday clinical practice. The application of the IC law has no mechanisms for its individual application because it fails to take into account the psychology of decision-making.

What is it in fact «legalization» of euthanasia and its rapid spread across the world? How is it embedded in the economic and cultural development of society? What has it already led to and where can it lead to in the time to come?
How has medicine begun to evolve into its opposite, proposing killing as well as or instead of healing, forming an entire class of professional executioners as an integral part of the profession? How did the substitution of notions take place, forming an oxymoron: "involuntary good death"?
What is the role of the psychiatrist and is he in a position to judge the unbearability of sufferings of the dying, the presence of their "free will", their "awareness" of what they allegedly request, and even more so, not asking for? Why is euthanasia of the mentally ill fundamentally different from euthanasia in the case of body illnesses?

Представлено одно из объяснений разрыва традиционной медицинской связи диагностики и лечения. Предполагается, что это был закономерный процесс, вызванный к жизни результатами развития генетики и результатов применения клинической эпидемиологии (теории доказательной медицины), которые привели к началу гибели нозологической концепции, пока что de facto, а в перспективе – de jure. Медицина мучительно возвращается к целостному взгляду на больного.
Ключевые слова: нозологическая концепция; клиническая эпидемиология; кризис; патоморфоз; коморбидность; концепция болезни; индустриальное развитие медицины.
SUMMARY
One explanation for the breakdown of the traditional medical connection of diagnosis and treatment is presented. It is suggested that it was a natural process brought to life by the results of the development of genetics and the results of the application of clinical epidemiology (the theory of evidence-based medicine), which led to the beginning of the downfall of the nosological concept, so far de facto, and in the long term de jure. Medicine is painfully returning to a holistic view of the patient.

Цель этой работы-показать, что медицина вообще, и психиатрия в частности, представляют собой встр... more Цель этой работы-показать, что медицина вообще, и психиатрия в частности, представляют собой встроенную в Рынок отрасль производства (точнее-ремесла, местами переросшего в производство), а не «науку» или «искусство»-любимых сфер профессиональных фантазий и иллюзорного существования абсолютного большинства российских врачей. Этот крайне неохотно обсуждаемый вопрос, отнюдь не праздный, не является «делом вкуса». Отсутствие адекватного ИМЕНИ у предмета, приводят к тому что предмет просто не существует. А на его месте существует совершенно другой, выдуманный предмет. Неизбежный процесс «индустриализации» имеет свои достоинства и недостатки-неведомые тем, кто не хочет о них ничего знать. Незнание рождает неадекватные ожидания и претензии, как со стороны общества, так и самих врачей, в сущности не понимающих, где они, собственно, находятся и кто они есть. Между тем, такая модель непротиворечиво объясняет: • Конфликт между индивидуальностью и стандартизацией/унификацией, как причину кризиса клинической медицины, ориентированную на избыточность описаний. • "Дегуманизацию"медицины • Опасности, которые возникают при неизбежном внедрении индустриальной модели медицины в неготовое к этому сообщество (например, культ личности известных врачей, перетягивающих средства в «нужные» отрасли; поспешное внедрение непроверенных технологий (Зорин Н.А. 2009). • Конфликт антисциентизма и науки-неадекватное отношение к ней-мифы как о всемогуществе, так и о бессилии, с верой и больных и врачей в магическое. • Вопросы о научности ее методов (напр. неспособность отличить технологические классификации от научных-Природу от технологий). This work is a "literary response" to the publication by D. Trutneva and V. Mendelevich "Psychotherapy without a psychotherapist" within the overall subject "Apologies of the industrial model of medicine" which was raised by the author at the symposium "Stаtus praesens of psychiatry" in St. Petersburg on 29-30 November 2019. The body of the text is made up of two literary essays written way back in 1994. The essays have proven to be visionary in terms of the evolution of paramedical studies where I believe psychoanalysis belongs.

БЕХТЕРЕВА, 2018 c 19-22, 2018. Рассматриваются различия между клинической эпидемиологией и ее практической частью – доказательной медициной. Обсуждаются недостатки медицинских оценочных инструментов в их культурно-лингвистическом и смысловом аспектах. Сделана очередная попытка показать несостоятельность их применения для групповых обобщений.

Abstract
The discussion on methodological limitations of quasi-measuring human feelings and sensations continues, started by the author in the journal Neurological NEUROLOGICAL BULLETIN NAMED AFTER V.M. BEKHTEREV 2018, p 19-22, 2018. The differences between clinical epidemiology and its practical part – evidence-based medicine – are considered. The limitations of medical assessment tools in their cultural, linguistic and semantic aspects are discussed. Another attempt is made to show the inadequacy of their use for group generalizations.

В статье рассматривается проблема квазиизмерений чувств и ощущений, в частности, в свете ее приложения к доказательной медицине. Проверяется гипотеза о некорректности применяемых ныне квазиизмерений чувств и ощущений, и обобщения таких данных в метаанализах. На основе представлений структурной лингвистики рассматривается механизм опосредования выраженности психических феноменов, смыслами психических переживаний, имеющих семиотическую природу.

The paper deals with the problem of quasi-measurements of sentiments and feelings, in particular, in terms of evidence-based medicine. The examination is centered on the assumption of the inherent incorrectness of quasi-measurements of sentiments and feelings, and on the generalization of such data in meta-analyzes (MA). Structural linguistics concepts are applied to build a mechanism for mediating the severity of psychic phenomena through the meanings of psychic experiences of a semiotic nature.

А. Файнштейном как «коморбидность». Показано, что это по-
нятие не имеет однозначного толкования, что не способствует
продуктивному изучению того комплекса вопросов, который
оно породило. Рассматривается гипотеза, согласно которой
понимание коморбидности у автора названия и большей части
современного врачебного сообщества не совпадает и нуждает-
ся в прояснении.

The authors attempted to understand the causes of the confusion
that had arisen with the phenomenon which A. Feinstein
designated as “comorbidity” almost 50 years ago. It was shown
that this concept does not have a single interpretation, which is
not conducive to a productive study of the set of issues it gave
rise to. A hypothesis was considered that the understanding of
comorbidity by the author of the designation and most of the
modern medical community is not the same and needs to be
clarified.

ABSTRACT
The discussion on the methodological limitations of the quasi- measurement of human feelings and
sensations, started by the author in the journal Neurology bulletin, continues. There are a number of reasons for
accepting/rejecting and understanding the principles and limitations of clinical epidemiology/evidence-based
medicine (EBM) to the point of deliberate distortion. It is shown that the ideas of “freedom” and “creativity”
in medicine, are ways of avoiding control and the realization of biopower. A concrete example, the study of the
efficacy of antidepressants, shows how it is possible, either unintentionally or intentionally, to obtain a false
result without following the principles and tenets of EBM.

Abstract: The Informed Consent (IC) procedure is considered as a legal construct, a product of liberal economics. As such, IC is a tool for shifting responsibility for the choice of intervention from the seller of health care services to the consumer and is a binding contract to avoid legal liability and all sorts of losses on both sides.
The set of problems surrounding the IC can be explained by the significant difference between an experimental procedure (for which it was originally created) and everyday clinical practice. The application of the IC law has no mechanisms for its individual application because it fails to take into account the psychology of decision-making.

What is the role of the psychiatrist and is he in a position to judge the unbearability of sufferings of the dying, the presence of their "free will", their "awareness" of what they allegedly request, and even more so, not asking for? Why is euthanasia of the mentally ill fundamentally different from euthanasia in the case of body illnesses?

SUMMARY
One explanation for the breakdown of the traditional medical connection of diagnosis and treatment is presented. It is suggested that it was a natural process brought to life by the results of the development of genetics and the results of the application of clinical epidemiology (the theory of evidence-based medicine), which led to the beginning of the downfall of the nosological concept, so far de facto, and in the long term de jure. Medicine is painfully returning to a holistic view of the patient.

This work is a "literary response" to the publication by D. Trutneva and V. Mendelevich "Psychotherapy without a psychotherapist" within the overall subject "Apologies of the industrial model of medicine" which was raised by the author at the symposium "Stаtus praesens of psychiatry" in St. Petersburg on 29-30 November 2019. The body of the text is made up of two literary essays written way back in 1994. The essays have proven to be visionary in terms of the evolution of paramedical studies where I believe psychoanalysis belongs.

Abstract
The discussion on methodological limitations of quasi-measuring human feelings and sensations continues, started by the author in the journal Neurological NEUROLOGICAL BULLETIN NAMED AFTER V.M. BEKHTEREV 2018, p 19-22, 2018. The differences between clinical epidemiology and its practical part – evidence-based medicine – are considered. The limitations of medical assessment tools in their cultural, linguistic and semantic aspects are discussed. Another attempt is made to show the inadequacy of their use for group generalizations.

The paper deals with the problem of quasi-measurements of sentiments and feelings, in particular, in terms of evidence-based medicine. The examination is centered on the assumption of the inherent incorrectness of quasi-measurements of sentiments and feelings, and on the generalization of such data in meta-analyzes (MA). Structural linguistics concepts are applied to build a mechanism for mediating the severity of psychic phenomena through the meanings of psychic experiences of a semiotic nature.

The authors attempted to understand the causes of the confusion
that had arisen with the phenomenon which A. Feinstein
designated as “comorbidity” almost 50 years ago. It was shown
that this concept does not have a single interpretation, which is
not conducive to a productive study of the set of issues it gave
rise to. A hypothesis was considered that the understanding of
comorbidity by the author of the designation and most of the
modern medical community is not the same and needs to be
clarified.

Интересны и дальнейшие приключения этой статьи. В конце ее дан "первоисточник" - молодежный психиатрический сайт, на который ее поместили меня не спросив ( я посылал ее в частном порядке одному из адептов), ибо прошло много лет со дня ее написания и я хотел что-то подправить и пояснить. Мне в грубой форме было отказано. Попросил тогда убрать ее - отказано! Но когда я опубликовал две свои "Реплики" (см. в моем списке работ) где затрагивал научные (!) воззрения адептов этого молодежного сайта, то вместо дискуссии, на которую надеялся, получил...удаление своей работы с их сайта! Поэтому нажав на ссылку "первоисточника" вы статью там не найдете!
Все это -замечательные иллюстрации того, какими методами ведется борьба с неугодными публикациями и их неудобными авторами на так называемом "научном поле"...
