This article describes the distinctive function of the pediatric clinical research nurse (CRN) in... more This article describes the distinctive function of the pediatric clinical research nurse (CRN) in the anesthesia setting. The pediatric CRN in anesthesia acts as a liaison between families and the research team and is the major nexus between the principal investigator or anesthesiologist on a study, and the collaborating surgeons from many different departments. This is unique because the CRNs collaborate with physicians in specialties that can include plastics, urology, neurosurgery, orthopaedics, otolaryngology, cardiology, critical care, and many other departments. The profession requires a breath of knowledge ranging from clinical understanding of diseases, surgical procedures, and recovery to cognitive and developmental stages, to expertise in the research protocol process. Background: Our recent literature review (Pancreas 2015, in press) shows that both type 1 DM (T1D... more Background: Our recent literature review (Pancreas 2015, in press) shows that both type 1 DM (T1DM) and type 2 DM (T2DM) are associated with significant histopathologic changes in the exocrine pancreas in the absence of clinical symptoms (pain, pancreatitis or steatorrhea). We called this entity diabetic exocrine pancreatopathy (DEP). Aim: To determine if DEP is histo-pathologically distinct from CP. Methods: Under an IRB-approved protocol we identified from the Mayo Clinic Autopsy database subjects who had provided pre-mortem study consent and had well-preserved pancreata (minimal to no autolysis) at autopsy. For this study we selected subjects with i) long-standing T2DM (n=9) without history of pancreatic disease or of alcoholism who had moderate-to-severe fibrosis in the exocrine pancreas and ii) clinical diagnosis of CP (n=7) confirmed on autopsy. Five expert pancreas pathologists from around the world were made aware of the findings of the review on DEP (Pancreas 2015, in press). Unaware of clinical history or distribution of cases they independently reviewed Hematoxylin and Eosin and Trichrome stains on a digital pathology platform. They entered their observations on a predesigned score sheet and rated their confidence in their diagnosis; >75% confidence in a diagnosis was considered definitive, less as inconclusive. Overall concordance of each reviewer for the diagnosis was calculated using the diagnosis from our center as standard. Multi-rater kappa statistics were used to calculate inter-observer agreement. Results: All 16 cases were definitively classified as CP or DEP by all 5 pathologists with a diagnostic concordance of 81%, 94%, 94%, 100% and 100%, respectively. All 7 cases of CP were correctly diagnosed by all 5 reviewers. One case of DEP was incorrectly diagnosed as CP by 3 pathologists and 2 other DEP cases were misdiagnosed as CP by 1 and 2 pathologists each. The overall multi-rater kappa statistic was 0.83 (kappa>0.81 implies near perfect agreement). DEP and CP had both overlapping and distinguishing histopathologic features (Figure). Both DEP and CP showed inter-acinar fibrosis and varying degrees of acinar atrophy; however, unlike CP, DEP had minimal inter-lobular fibrosis, compete absence of stromal inflammation, minimal to no ductal changes and frequent presence of islet amyloid. Conclusion: DEP is a novel and distinct histopathologic entity that can be readily distinguished from CP with near-perfect inter-observer agreement. The pathophysiology and clinical implications of DEP need further study.

This descriptive study investigates the ways that students in an Ancient History class make meani... more This descriptive study investigates the ways that students in an Ancient History class make meaning of past events by relating them to their present lives and experiences. The study is grounded in theories of historical thinking, particularly focusing on the concepts of presentism and its usefulness for examining classroom teaching. The following two research questions guided the study: 1) How do students make connections between the past and present? 2) When given the opportunity in writing and discussion, in what ways are students engaging with distant events in the past? The research site was a middle school in a medium-sized city located in the Midwest. Data was collected in the Fall of 2010 over the course of eight weeks during two units of study (Ancient Civilizations Review and Ancient Greece). The following data sources were collected: five audio-recorded classroom discussions and other instruction, twelve interviews with students and student assignments and journal entries. The analysis revealed two categories of the students' thinking about the relevance of past events: (1) conflating the past with the present and the challenge of presentism, and (2) translating and adapting the past to the present. The data showed that some students viewed history from a presentist perspective that did not distinguish sufficiently between time periods. However, many students also applied lessons from the past to their own lives by associating historical ideas and themes to their individual experiences, which was especially evident in the discussions. Students tended to make sense of v history by assigning meaning to the concepts they were studying in a personal manner and by finding similarities between the past and present. vi TABLE OF CONTENTS

We also compared the prevalence of obesity and hypertension to reference standards for the adult population. Methods : Patient data were obtained from the electronic health records. All records identified as new patients during October 2013 were included. Variables of interest were clinic site, patient demographics, blood pressure, body mass index (BMI), chief complaint, and ICD-9 codes. Descriptive statistics were computed and compared to reference standards from previous reports. Results : During October 2013, there were 224 new patients that entered the clinics. Aedes (Ae.) aegypti is the primary vector for dengue viruses (serotypes1-4) and chikungunya virus... more Aedes (Ae.) aegypti is the primary vector for dengue viruses (serotypes1-4) and chikungunya virus. Homing endonucleases (HEs) are ancient selfish elements that catalyze double-stranded DNA breaks (DSB) in a highly specific manner. In this report, we show that the HEs Y2-I-AniI, I-CreI and I-SceI are all capable of catalyzing the excision of genomic segments from the Ae. aegypti genome in a heritable manner. Y2-I-AniI demonstrated the highest efficiency at two independent genomic targets, with 20-40% of Y2-I-AniI-treated individuals producing offspring that had lost the target transgene. HE-induced DSBs were found to be repaired via the single-strand annealing (SSA) and non-homologous end-joining (NHEJ) pathways in a manner dependent on the availability of direct repeat sequences in the transgene. These results support the development of HE-based gene editing and gene drive strategies in Ae. aegypti, and confirm the utility of HEs in the manipulation and modification of transgenes in this important vector.

Online-questionnaires can provide many capabilities not found in traditional paper-based questionnaires. Despite this, and the introduction of a plethora of tools to support online-questionnaire creation, current electronic survey design typically replicates the look-and-feel of paper-based questionnaires, thus failing to harness the full power of the electronic delivery medium. A recent environmental scan of online-questionnaire design tools found that little, if any, support is incorporated within these tools to guide questionnaire designers according to best-practice (Lumsden and Morgan, 2005). This paper briefly introduces a comprehensive set of guidelines for the design of online-questionnaires. Drawn from relevant disparate sources, all the guidelines incorporated within the set are proven in their own right; as an initial assessment of the value of the set of guidelines as a practical reference guide, we undertook an informal study to observe the effect of introducing the guidelines into the design process for a complex online-questionnaire. The paper discusses the qualitative findings-which are encouraging for the role of the guidelines in the 'bigger picture' of online survey delivery across many domains such as e-Government, e-Business, and e-Health-of this case study.

The study lists a series of core recommendations - identified by the 513 leaders involved in the study and validated by an additional 600 higher educational leaders from Australia and across the world - designed both to help address the leadership succession and capabilities crises, and to secure the Australian education system during the challenging times that lie ahead. Australia does not have a single school system. Under the federal political structure, education is the responsibility of the eight states and territories. While schooling across the country has many commonalities, there are a number of differences that affect school operations. The situation is made even more complex by the existence of a substantial and growing non-government school sector, which enrols 33 percent of all students and encompasses a wide variety of school types. However, in recent years there have been significant steps towards achieving greater national consistency across the eight states and territories. Nevertheless, caution is needed in generalising across the diversity of Australian schooling.

We sought to increase this repertoire by characterizing the ability of upstream sequences derived from the Ae. aegypti Ub L40 and polyubiquitin genes to drive the expression of marker proteins. Both genomic fragments were able to drive robust expression of luciferase in cultured mosquito cells. Following Mos1transformation, the Ub L40 promoter drove strong expression of a fluorescent marker in early larvae and in ovaries, while the polyubiquitin promoter drove robust EGFP expression in all stages of development including constitutive expression throughout the midgut. These promoter fragments provide two new expression profiles for future Ae. aegypti genetic experiments.
