We investigated the impact of paracetamol administration on lymphocyte DNA damage and oxidative s... more We investigated the impact of paracetamol administration on lymphocyte DNA damage and oxidative stress parameters for acute pain management in patients with acute traumarelated pain. Thirty-five adult patients with mild or moderate trauma and 33 eligible healthy volunteers as control subjects were enrolled. Blood samples were obtained from the patients before and at 2 and 12 h after IV administration of paracetamol. Serum lymphocyte DNA damage and total oxidant status (TOS), total antioxidant status (TAS) and oxidative stress index (OSI) were studied as oxidative stress parameters. Serum lymphocyte DNA damage levels were 14.97 ± 12.38 AU (arbitrary units) in Group 1 (before analgesia), 13.94 ± 9.28 AU in Group 2 (2 h after analgesia), and 10.57 ± 8.73 AU in Group 3 (12 h after analgesia). A significant difference was observed among the groups with respect to serum lymphocyte DNA damage (p < 0.01). Mean serum TOS and OSI values were lower in Groups 2 and 3 compared to that in Group 1. Furthermore, mean serum TAS values were higher in Group 3 compared to that in Goup 1. Our data suggest that the treatment of trauma-related acute pain with intravenous paracetamol resulted in decreased DNA damage in human T lymphocytes.

Bu çalışmanın amacı, erişkinlerde yüksek voltaja bağlı elektrik yaralanmalarında mortalite üzerin... more Bu çalışmanın amacı, erişkinlerde yüksek voltaja bağlı elektrik yaralanmalarında mortalite üzerine etki eden faktörlerin araştırılmasıdır. Gereç ve yöntem: Ocak 2009-Aralık 2011 tarihleri arasında yüksek gerilime maruz kalma nedeniyle acil servise başvuran ve yoğun bakıma yatırılmış olan olgular geriye dönük olarak incelendi. Olgular yaş, cinsiyet, mevsimsel dağılım, olay yeri, yanık derecesi, laboratuvar sonuçları, EKG değişiklikleri, yüksekten düşme, şok, yoğun bakımda yatış, sepsis gelişiminin mortalite ile ilişkisi bakımından değerlendirildi. Bulgular: Çalışmaya 18-66 yaş arasında 138'i (%90.8) erkek 14'ü (% 9.2) kadın olmak üzere 152 hasta dahil edildi. Yaş grubu, cinsiyet, elektrik yaralanmalarının yaz mevsiminde olması, sanayiide olması, 2. ve 3. derece yanık gelişimi, bilincin kapalı olması, yüksekten düşmenin eşlik etmesi, şok bulgusu olması, EKG değişikliği bulunması, yoğun bakıma yatış yapılması, kan kültüründe üreme olması, sepsis gelişimi gruplar arasında anlamlı bulundu. Fakat her biri tek başına mortaliteyi etkileyen bağımsız faktörler değildi. Olgularda 2.derece yanık olması, sepsis varlığı ve şok bulgularının olması mortalite bakımından prediktif bağımsız faktörler idi. Sonuç: Yüksek voltajlı elektrik yaralanmalarında erkek cinsiyet, yaşlı, başvuru esnasında bilinç kapalılığı, şok olması, 2. ve 3. derece yanık olması, sepsis gelişimi, yüksekten düşme olması, yoğun bakıma yatış yapılanlarda ölüm oranının daha fazla olduğu görülmektedir.

Background: This study evaluated the clinical characteristics of trauma patients in the southeast... more Background: This study evaluated the clinical characteristics of trauma patients in the southeastern coastal area of Turkey and investigated the factors influencing mortality. Methods: Patients admitted with trauma to the emergency departments of Harran and Gaziantep Medical Schools and to the emergency services of hospitals in Sanlıurfa between June 2008 and December 2008 were enrolled retrospectively in this study. All medical records and follow-up data were reviewed for each patient. Results: The study evaluated 15,120 trauma patients. The causes of trauma were motor vehicle accidents (38.7%), falls from heights (36.8%), burns (7.8%), knife wounds and gun shots (8.1%), homicides (6.5%), and workplace-related accidents (2.1%). The overall patient mortality rate was 3.8%. The mean patient ages were 47.8 ± 0.9 and 29.7 ± 0.4 among those who died and among those who survived, respectively (P < 0.01). The median times to arrival were 130 minutes and 42 minutes among those who died and among those who survived, respectively (P < 0.01). Whereas 79.9% of patients were discharged after treatment in the emergency departments, 16.3% were referred to various departments for hospitalization, and 3.8% were admitted to the intensive care unit (ICU). The mean score on the Glasgow Coma Scale was 7.5 ± 0.3 among who died and 12.8 ± 0.6 among those who survived (P < 0.05), and the mean Revised Trauma Scores were 8.7 ± 0.5 among those who died and 11.5 ± 0.7 among those who survived (P < 0.05). Intubation or cardiopulmonary resuscitation was initiated in 88% of those who died and 43.5% of those who survived (P < 0.05). Of those who died, 84% had cranial injuries and 43.5% had thoracic injuries. Conclusions: Frequent causes of trauma in our region are motor vehicle accidents and falls from heights. Type of trauma, rapid arrival at the hospital, hospital procedures and interventions, age, sex, and trauma scores were predictors of mortality in trauma patients.

The objective of this study was to investigate the rate of neural tube defects (NTD) and the clin... more The objective of this study was to investigate the rate of neural tube defects (NTD) and the clinical features of newborns of mothers who did not use folic acid (FA) in their pregnancies. Material and methods: The data of a total of 82 newborns, who were diagnosed with meningomyelocele were operated and examined, retrospectively. The newborns were divided into two groups depending on whether their mothers used FA during pregnancy or not. Results: The mothers of 37 (45.1%) newborns used FA during the antenatal period, whereas those of 45 (54.9%) newborns did not. The mean birth weight of the newborns whose mothers did not use FA were lower. Furthermore 9 (25%) newborns whose mothers did not use FA were delivered via cesarean section.The incidence of meningomyelocele was 80% for 45 newborns with NTDs whose mothers did not use FA. Conclusion: Our results revealed that the mean birth weight was lower while the incidence of meningomyelocele was significantly higher in newborn infants whose mothers did not use FA.

This is a case report of a patient with atrial fibrillation (AF) and transient ischemic attack (TIA) induced by vigrous snake bite that was suspected to be caused by the treatment of subcutaneous epinephrine, due to immediate hypersensitivity reaction following the administration of antivenom treatment.

Objective: To study the prognostic importance activity of paraoxonase and arylesterase, and the v... more Objective: To study the prognostic importance activity of paraoxonase and arylesterase, and the value of mean platelet volume in patients with acute ischaemic stroke. Methods: This case-control study was conducted at Harran University Hospital, Sanliurfa, Turkey, from January to June 2014, and comprised patients with symptoms of acute ischaemic stroke who presented to the emergency department. Paraoxonase activity, expressed in units per litre, or U/L, of serum, was evaluated in the absence of basal activity, and arylesterase activity was defined as micromoles, of phenol generated/min, and was expressed as U/L of serum. Mean platelet volume was measured as a routine parameter. SPSS 20 was used for data analysis. Results: Of the 94 participants, 48(51%) were patients with acute ischaemic stroke and 46(49%) were control subjects. Moreover, 27(56.3%) patients were females and 21(43.7%) were males. In the control group, 26(56.5%) were females and 20(43.5%) were males. The mean age of patients was 68.39±11.83 years compared to controls' 65±9.95 years. Decreased activity of prognostic importance and arylesterase were significant in patients than in the controls (p= 0.016 and p= 0.001, respectively). The median platelets of patients was significantly lower than that of the controls (p=0.004). However, the median mean platelet volume values were similar in the both groups (p=0.568). Binary logistic regression analyses showed that the paraoxonase and arylesterase were risk markers for the patients. Conclusions: Decreased paraoxonase and arylesterase activity and decreased platelet counts were observed probably due to increased oxidative stress in acute ischaemic stroke patients.

OBJECTIVES We investigated the state of inflammation, PTX3 level and other routine inflammatory m... more OBJECTIVES We investigated the state of inflammation, PTX3 level and other routine inflammatory markers (high sensitivity C-reactive protein [hsCRP], and white blood cells [WBC]), in patients who presented to the emergency department (ED) with migraine. We also investigated the relationship between the clinical presentation, PTX3 level, and other routine inflammatory markers in the emergency management of these patients. METHODS The study included 44 patients (group 1) who presented to the ED due to a migraine attack with aura and 44 controls (group 2) with similar demographic characteristics. RESULTS The WBC count was 8.82 ± 2.10 × 109/L in group 1 and 7.85 ± 2.04 × 109/L in group 2. The mean PTX3 level was 11.57 ± 3.99 ng/mL in patients who presented at the ED with a migraine attack, and 4.59 ± 1.28 ng/mL in controls. The differences values of WBC and PTX3 between the two groups were significant (respectively; P = 0.031, P < 0.001). ROC analyses indicated significant results for PTX3 as a marker for acute migraine attack. It had a sensitivity of 93% and specificity of 84% at a cut-off value of 5.80 ng/mL. CONCLUSION This is the first study to investigate plasma levels of PTX3 in patients with acute migraine. PTX3 as a biomarker may be used as an additional examination to the current subjective criteria to support the diagnosis of patients presenting to the ED with an acute migraine attack.

Posterior reversible encephalopathy syndrome (PRES) or leukoencephalopathy syndrome was introduce... more Posterior reversible encephalopathy syndrome (PRES) or leukoencephalopathy syndrome was introduced into clinical practice in 1996 by Hinchey et al., to describe unique syndrome, clinically expressed during hypertensive and uremic encephalopathy, eclampsia, and immunosuppressive therapy. Hyperperfusion with resultant disruption of the blood–brain barrier results in vasogenic edema, but not infarction, most commonly in the parieto-occipital regions. The severity of this clinical symptom varies. For example, the visual disturbance can manifest as blurred vision, homonymous hemianopsia, or even cortical blindness.Patients may be mildly confused or agitated but can become comatose.Other symptoms less commonly seen include nausea, vomiting, seizures, and brainstem deficits.Chronic kidney disease (CKD) and acute kidney injury are both commonly present in patients with PRES. We are presenting a rare case of neurogenic bladder who developed PRES after augmentation cystoplasty due to underlying CKD.

We report a very rare case of hemoptysis, whose cause has proven an infestation of the respiratory mucosa by a Leech. case report: An 80-year old male presented with a three-day history of hemoptysis. the patient was admitted to our intensive care unit for respiratory distress and hemodynamic instability. Endoscopic examination was performed following nasal packing under local anesthesia and oxygen. A worm was noticed adhered to the bronchial mucosa. this worm was removed under local anesthesia and identified as a leech. conclusion: the infestation by the leech remains relatively rare. the diagnosis should be considered in the presence of unexplained hemoptysis, in patients with a recent history of drinking stream water. Endoscopy is the cornerstone in the diagnosis and the treatment of this infestation.
