Los materiales de relleno inyectables ofrecen una alternativa de rejuvenecimiento facial, reducie... more Los materiales de relleno inyectables ofrecen una alternativa de rejuvenecimiento facial, reduciendo los riesgos de enfrentarse a una cirugía invasiva convencional. Es importante que el médico y el paciente estén enterados de los efectos adversos que pueden presentar los pacientes al colocarse estos materiales. OBJETIVO: determinar las características histopatológicas de granulomas a cuerpo extraño posterior a la colocación de implantes estéticos inyectables y compararlo con patrones de sustancias de relleno descritas en la literatura. MÉTODOS: Se revisaron 18 biopsias entre los años 2002 y 2004 correspondientes a 18 pacientes con el antecedente de haber recibido implantes. RESULTADOS: 95% de los pacientes estudiados eran del sexo femenino y 5% del sexo masculino, con edad promedio de 41 años. 89% de los pacientes tenía patrones granulomatosos de los cuales 56% tenía un patrón difuso y 33% patrón nodular, y un estroma fi broso circundante hasta en un 50% de las biopsias. El 50% de las biopsias tenía vacuolas pequeñas y 28% vacuolas pequeñas y grandes. 17% presentó birrefringencia. Los hallazgos histológicos predominantes fueron los que habían sido infi ltrados con silicona en un 33%, seguido de metilmetacrilato 28%, 17% de las biopsias tenía reacción infl amatoria secundaria a materiales inespecífi cos comprobados por su birrefringencia. CONCLUSIÓN: Los implantes cutáneos que con mayor frecuencia producen efectos adversos en nuestro medio son la silicona (33%) y polimetilmetacrilato (22%). En el examen histopatológico 28% de los pacientes presentaron granulomas supurativo asépticos. Los cuerpos birrefringentes fueron observados en materiales mixtos no específi cos, lo cual no ha sido descrito en la literatura. El examen histopatológico también puede orientarnos acerca de la etiología del material de relleno colocado.

En este caso, jamás pensé en la erradicación quirúrgica, (que podría haberse realizado con igual éxito), por razones que expondré más adelante.

Mucin 1 (MUC1) is a transmembrane glycoprotein that protects epithelial cells from injury caused ... more Mucin 1 (MUC1) is a transmembrane glycoprotein that protects epithelial cells from injury caused by external stimuli. In addition to this role, MUC1 is involved in cell-cell adhesion, proliferation, motility, invasion and survival. In epithelial cells, MUC1 expression is regulated by binding of TNFα to TNFR1 and activation of the NFκB pathway. In human skin, MUC1 is not expressed in normal epidermis but rather in pre-malignant and malignant conditions. Nevertheless, the expression of MUC1 and its implication in psoriasis vulgaris has not been considered. Here, we show that MUC1 was present in the epidermis of psoriatic plaques observed in 11 biopsies from patients diagnosed with psoriasis vulgaris which were compared with 5 normal human skin. Interestingly, MUC1 in addition to being localized at the apical surface of some suprabasal keratinocytes, was also localized over the entire cell surface of some of these cells and some basal keratinocytes. Mycobacterium cosmeticum sp. nov., a novel rapidly growing species isolated from a cosmetic infection and from a nail salon

Four isolates of a rapidly growing Mycobacterium species had a mycolic acid pattern similar to th... more Four isolates of a rapidly growing Mycobacterium species had a mycolic acid pattern similar to that of Mycobacterium smegmatis, as determined by HPLC analyses. Three of the isolates were from footbath drains and a sink at a nail salon located in Atlanta, GA, USA; the fourth was obtained from a granulomatous subdermal lesion of a female patient in Venezuela who was undergoing mesotherapy. By random amplified polymorphic DNA electrophoresis and PFGE of large restriction fragments, the three isolates from the nail salon were shown to be the same strain but different from the strain from the patient in Venezuela. Polymorphisms in regions of the rpoB, hsp65 and 16S rRNA genes that were shown to be useful for species identification matched for the two strains but were different from those of other Mycobacterium species. The 16S rRNA gene sequence placed the strains in a taxonomic group along with Mycobacterium frederiksbergense, Mycobacterium hodleri, Mycobacterium diernhoferi and Mycobacterium neoaurum. The strains produced a pale-yellow pigment when grown in the dark at the optimal temperature of 35 6C. Biochemical testing showed that the strains were positive for iron uptake, nitrate reduction and utilization of D-mannitol, D-xylose, iso-myo-inositol, L-arabinose, citrate and D-trehalose. The strains were negative for D-sorbitol utilization and production of niacin and 3-day arylsulfatase, although arylsulfatase activity was observed after 14 days. The isolates grew on MacConkey agar without crystal violet but not on media containing 5 % (w/v) NaCl or at 45 6C. They were susceptible to ciprofloxacin, amikacin, tobramycin, cefoxitin, clarithromycin, doxycycline, sulfamethoxazole and imipenem. Kikuchi-Fujimoto Necrotizing Lymphadenitis Associated With Cutaneous Lupus Erythematosus

Histiocytic necrotizing lymphadenitis or Kikuchi-Fujimoto disease is a rare entity, and even more... more Histiocytic necrotizing lymphadenitis or Kikuchi-Fujimoto disease is a rare entity, and even more rarely, it is associated with other diseases. In a few cases, the condition is associated with cutaneous lupus erythematosus (CLE). Histiocytic necrotizing lymphadenitis predominantly affects young women, who present with solitary or multiple cervical lymphadenopathy accompanied by symptoms such as fever, weight loss, sweating, or, in exceptional cases, hepatosplenomegaly. Laboratory examinations show normal or nonspecific results. The disease is of unknown origin, although a viral origin has been suggested, with the suspected agents including Epstein-Barr virus, herpesvirus type 6, and cytomegalovirus (CMV). Although the first and most of the more recent cases have been reported in Oriental patients, the disease has a wide geographic distribution. The clinical evolution is favorable, with spontaneous remission in less than 4 months in almost all cases. We present a case of a 37-year-old woman from Peru who presented with cervical adenopathies on two occasions. Biopsy of a lymph node revealed a histopathologic picture compatible with Kikuchi-Fujimoto histiocytic necrotizing lymphadenitis. The adenopathy disappeared in a few months. A year later, she presented with a maculopapular rash in the nasal and malar regions. The results of the skin biopsy and immunofluorescence examination were compatible with chronic CLE. The results of the serology testing for CMV were positive. Treatment with chloroquine was initiated, with almost complete recovery by 5 months. No manifestations of systemic lupus erythematosus have occurred since. The epidemiologic, clinical, and anatomopathologic aspects as well as the differential diagnosis of this entity are reviewed.
