940 male and female patients aged 15 to 77 years were enrolled in a multicentre noncomparative st... more 940 male and female patients aged 15 to 77 years were enrolled in a multicentre noncomparative study in order to assess the efficacy and tolerability of nimesulide in otorhinolaryngological inflammatory diseases. 309 patients were affected by otitis media and 631 by upper respiratory tract inflammation. All the patients were treated with orally administered granular nimesulide 100mg twice daily for a mean period of 10 days. Nimesulide significantly reduced the intensity of signs and symptoms, thus allowing functional recovery. The drug was well tolerated, and of the 75 patients who reported adverse effects, only 26 had to be withdrawn from treatment.

The aim of the present study is to characterise the cell kinetics of pleomorphic adenoma of the p... more The aim of the present study is to characterise the cell kinetics of pleomorphic adenoma of the parotid gland by assessing DNA content and proliferating cell nuclear antigen (PCNA) positivity. In 22 parotid adenomas, DNA content was measured by densitometry in histological serial sections stained with Feulgen's method and PCNA positivity was determined by immunohistochemistry with the monoclonal antibody PC10. To assess the proliferative activity, DNA index and PCNA index were evaluated. It was possible to distinguish two types of adenoma. In Group I there was a prevalence of diploid cells with a low PCNA index. Group II is represented by adenomas with a large percentage of triploid cells and a PCNA index significantly higher than that of Group I. Our findings suggest that the possibility of recurrence or malignant transformation depends on intrinsic biological properties of each adenoma.

In the field of reconstructive plastic surgery, grafts of autologous cartilage are sometimes used... more In the field of reconstructive plastic surgery, grafts of autologous cartilage are sometimes used to replace damaged or pathologic tissues, particularly in the nose, ear, and trachea. However, this procedure is difficult to apply, especially because of the scarcity of donor sites. In this study, we have cultured and characterized human chondrocytes from human nasal septal cartilage biopsies. The proliferative activity was evaluated by incorporation of 3H-thymidine into deoxyribonucleic acid (DNA) both under normal culture conditions and with different growth factors and serum concentrations. Identification of chondrocytes in culture was performed with immunohistochemistry and production of matrix with specific histochemical indicators. We observed a significant increase of cell kinetics of differentiated chondrocytes, embedded with intense metachromatic matrix, in the presence of transforming growth factor beta, and low concentrations of fetal calf serum. Therefore, in suitable conditions, human chondrocytes obtained even from small specimens can produce in vitro considerable quantities of pure autologous cartilaginous tissue within a few days. This newly formed cartilage can be used as a grafting material in reconstructive surgery, particularly in otolaryngology.

Retro-palatal obstruction is often involved in the pathogenesis of obstructive sleep apnea syndro... more Retro-palatal obstruction is often involved in the pathogenesis of obstructive sleep apnea syndrome (OSAS), which is currently treated by means of various surgical procedures. The aim of this paper is to describe our preliminary results using a new, minimally invasive and non-resective procedure called the "barbed Roman blinds technique" (BRBT), which was used to treat severe OSAS due to retro-palatal obstruction. The apnea-hypopnea index (AHI), time with <90 % O2 saturation, and Epworth Sleepiness Scale (ESS) of 32 consecutive adult outpatients (mean age 47.3 ± 8.6 years) were compared before and after surgery. The tolerability of the procedure was assessed using a 0-10 visual analog scale (VAS). All of the treated patients experienced a clinical and subjective improvement as documented by the significant differences (P < 0.001) in their pre- and post-BRBT AHI, time with <90 % O2 saturation, and ESS scores. There were no minor or major adverse events, and the good tolerability of the procedure was documented by the short mean hospital stay (1.2 ± 0.5 days) and the mean VAS value of 4.3 ± 0.7. The mean follow-up period was 12.2 ± 2.0 months. The preliminary results indicate that the BRBT is safe and effective, but long-term follow-up and comparative studies of larger case series are needed to confirm these encouraging findings.

The preoperative ultrasonography (US) and magnetic resonance imaging (MRI) diagnosis was confirmed by the results of a histopathological examination. US followed by MRI is the gold standard for the non-invasive diagnostic differentiation of dermoid cysts from other submental swellings and submental excision remains the best surgical option.

This is a modification of anterior palatoplasty, and its main features are the use of self-locking (barbed) threads and the possibility of stabilise the palatal suture by fixing it to anatomical bone and fibrous structures. The technique is described in detail and some preliminary results are presented.

It can be treated surgically and non-surgically, although resective procedures are associated with high postoperative morbidity and failure rate. We describe a new non-resective surgical procedure called the velo-uvulo-pharyngeal lift in which the soft palate is lifted, shortened, advanced and stiffened by means of permanent threads anchored to fibro-osseous attachments at the level of the posterior nasal spine and both pterygoid hamuli. Four adult patients (median age 44.5 years; range 42-65) affected by snoring and mild obstructive sleep apnoea-hypopnoea syndrome (apneoa-hypopnoea index, AHI < 20) requiring septal surgery under general anesthesia also underwent velo-uvulo-pharyngeal lift. In the era of the mini-invasive/conservative surgery, considering the increasing attention to the disregulation of the peripheral neuromuscular control of the upper airway contributing to pharyngeal collapse in OSAS, with the development of sophisticated treatments such as the neural stimulation of the upper-airway, which role should be reserved to a muscular resective procedure such as UPPP? Being aware of the uncertain results and the high postoperative morbidity of UPPP, we believe that we should re-evaluate the role of these procedures involving the resection of palatal/pharyngeal muscles and uvula.

Two special instruments specifically designed for this type of operation are illustrated. The technique finds a specific clinical application to cosmetic surgery of the nose when a functional problem exists.

The aim of this prospective study was to evaluate the effectiveness of fat injections in the trea... more The aim of this prospective study was to evaluate the effectiveness of fat injections in the treatment of velopharyngeal insufficiency (VPI). The study involved 10 patients (6 adults aged 19-48 years and 4 children aged 5-13 years) with mild/ moderate VPI who were injected with 3.5 to 8 mL of fat in the posterior, lateral pharyngeal walls and soft palate under general anaesthesia. A second fat-grafting procedure was performed in 2 patients to achieve further improvement. Nasoendoscopy revealed a reduction in the closure gap in all patients, and the perceptual evaluation demonstrated improved speech intelligibility and resonance and reduced nasal air leakage in all cases (P < .005). The aerodynamic assessment showed a significant reduction in nasal airflow during phonation (P < .05). Follow-up was 6 to 23 months. In conclusion, fat injections improved voice resonance and reduced nasal air escape in all treated cases and can be a promising alternative to major procedures, such as velopharyngoplasties, for the treatment of mild/moderate VPI.
