Background and Objective: Recently in Iran, the Health Transformation Plan (HTP) was formulated b... more Background and Objective: Recently in Iran, the Health Transformation Plan (HTP) was formulated by policy makers to reduce the amount of the out of pocket expenditures. The HTP caused different effects on medicines and medical supplies. The purpose of this paper was the identification of these effects. Methods: This study was a qualitative research performed in a 14-month interval in Iran during 2014-2015. A total of 17 semistructured interviews were used for data gathering. Participants were health authorities associated with the subject of the research and background in health executive management. They were aware of the HTP. Purposive sampling was done for data saturation. Criteria such as "Credibility", "Conformability", and "Dependability" were considered. Interviewees' anonymity was preserved. The data were transcribed, categorized, and then used the thematic analysis. Results: According to the thematic analysis, 6 themes and 21 sub-themes were found. Two main categories were identified. (I) The overt effects (3 themes) included optimizing the management of medicine and medical supplies, their cost controlling, and improving of supervision on distribution and consumption of them. (II) The covert effects (3 themes) included an increase in demand for hospitalization, a rise in total costs, and waste of hospital resources Conclusions: The HTP, besides positive and overt effects, had covert effects. The HTP increased not only in patients' inclination to stay more in hospitals, but also it increased total health expenditures and wasting resources. It could be suggested that, policy makers use this results to guarantee the success of the HTP.

Quality of life (QOL) is one of the health indexes for which many efforts have been made to defin... more Quality of life (QOL) is one of the health indexes for which many efforts have been made to define and measure during the last four decades of the 20(th) century in many countries. This paper is aimed at studying the QOL in relation to socioeconomic status of the general population of Isfahan in 1390. We applied a descriptive-analytical and sectional method. In this research, 385 women and men over 15 years of age from 14 regions of Isfahan's municipality were studied using multi-stage quota sampling. We examined QOL using the SF-36 standard questionnaire, along with two domains of mental and physical health and eight subscales within the validity domain of 65-90%. Social (81%) and economical (70%) status was also measured by the questionnaire instrument in both objective and subjective domains after confirming the validity and reliability of the instruments. Objectives: The aim of this study was to explore key informants' experiences about challenges and... more Objectives: The aim of this study was to explore key informants' experiences about challenges and recommendations of patient identification and tracking in past earthquakes in Iran. Methods: A qualitative survey of key informants' experiences and views was carried out on emergency physicians and nurses, first responders and managers of red crescent society, and emergency medical services (EMS) from March to September 2015. Semistructured interviews were performed with informants who had experience of response to past earthquakes in Iran by using a purposive sampling method. Interviews were audiotaped, transcribed, and entered into MAXQDA (version 10) for coding and content analysis. A thematic analysis of interview transcripts was undertaken. Results: 5 themes including the crowd's presence, lack of unified command and integrated management, deficiency of planning, shortage of managerial and information infrastructures, and human resources challenges with their subthemes were explored. Also, some solutions were explored as follows: community education, chain of command and integrated management in the disaster scene, and training the emergency medical personnel and first responders. Conclusions: There are many challenges that should be solved for improving patient tracking in future disasters in Iran. Planning for proper and effective communication between agencies that have responsibility for disaster management is very important. Also, community education is a vital measure to familiarize them with disaster and teach them how to save lives.

The objective of the present study was to investigate patient incentives for moral hazards using the experiences of experts of basic Iranian insurance organizations. This was a qualitative research. The study population included all experts of basic healthcare insurance agencies in the City of Isfahan, Iran, who were familiar with the topic of moral hazards. Aim: As we know, due to the undesirable structural and functional changes arise from aging, old p... more Aim: As we know, due to the undesirable structural and functional changes arise from aging, old people need more healthcare services than other groups in society. Many factors affect utilization of health services in this age group. Cultural factors can be named as the most important since they play a basic role in determining people's lifestyle and ultimately in the health protection behaviors. Therefore, this paper is aimed at investigation of cultural factors, affecting old people benefit of health services in Isfahan province in 2014. Methods: This study was descriptive analytic, conducted in later 2014. The research population consisted of people aged over 60 years who had been admitted in 12 hospitals affiliated to Isfahan University of Medical Sciences. Two hundred seventy-five people were selected and entered into the research using random sampling method. Data gathering tool was a researcher-made questionnaire in three scopes, and finally the data analyzed at two levels of descriptive level (including frequency, percentage, mean and standard deviation (SD) and inferential level (including statistical tests comparing independent group means, correlation coefficient test, and multivariable regression) in SPSS 22 software. Result: The results showed that the level of utilization for people who had routine exercises and jugging was 0.3 units of SD upper than others. Meanwhile, by increasing exercise frequency, level of religious beliefs and opinions and the having a proper and healthy diet, the utilization of healthcare services decreases by −0.198, −0.150 and −0.196, respectively. In fact, 5.5% of changes in health services utilization variable are explained by the research independent variables. Conclusion: The study showed that doctors' recommendations on physical activities, reasonable increase in exercise frequency, access to rich and healthy food, and religious beliefs can affect old people's utilization of health services; but they cannot explain all variances of it. Therefore, according to complexity of health related issues, other involved variables should be identified and investigated.

Most studies in this field agree that healthcare providers have the ability to influence the demand for their products. The purpose of this study is to evaluate the role of institutions in the phenomenon of induced demand. This study has been done using experts' experiences in Isfahan University of Medical Sciences. Materials and Methods: This is a qualitative study in which the semistructured interview was used for data generation. The participants were those informed in this regard and considered as experts. For data saturation, purposive sampling was done. In this study, 17 people were interviewed, and criteria such as reliability and stability were considered. The anonymity of the interviewees was guaranteed. The data were transcribed and categorized, and then analyzed using thematic analysis. Results: In the present study, thematic analysis was conducted and 30 subthemes and 2 main themes were extracted. The two main themes were healthcare insurance organizations and other health organizations. Each of the themes had its own sub-themes. Conclusion: The findings help health policy-makers have a better picture regarding the role of various institutions in the phenomenon of induced demand.
