Methods A clinical trial was carried out in the department of Dermatology and Venereology, Dhaka Medical College Hospital, Dhaka, Bangladesh from May 2013 to April 2014. Total sixty patients were enrolled and divided into group A and group B. Thirty of group A patients were treated with topical tacrolimus and thirty of group B patients were treated with topical clobetasol propionate. Results In group A, the duration of illness ranged from 2 months to 36 months and in group B, from 1 month to 24 months. Among the patients of group A and B, 25 (83.3%) and 27 (90%) patients improved, respectively. After 16th week of treatment, slight response in 6 (20%) and 4 (13.3%) and moderate response in 18 (59.4%) and 22 (63.2%) and marked response in 1 (3.3%) and 1 (3.3%) was seen in group A and group B, respectively. Patients and methods In this cross-sectional study, 120 patients with onychomycosis, attending outpatient department, were selected by purposive type of sampling technique. They were subjected to thorough clinical examination. Nail material was collected for mycological diagnosis by microscopy and culture on dermatophyte test medium. Results Microscopy was positive in 47 (39.2%) cases, whereas culture yielded Trichophyton rubrum in 53 (44.2%) and T. mentagrophytes in 2 (1.7%) patients. Common clinical nail changes in mycologically proven cases were thickening of nail plate (68.9%), subungual hyperkeratosis (63.9%), onycholysis (55.7%), roughening of nail plate (29.5%), yellowish discoloration (29.5%) and brownish-yellow discoloration (21.3%). Intracellular calcium plays an important part in the regulation of proliferation and differentiation of keratinocytes. This study tried to identify the correlation between severity of disease and serum calcium level in psoriasis patients. To assess the serum calcium level in patients of different types of psoriasis and to find out its relationship with disease severity according to Psoriasis Area Severity Index (PASI).This cross sectional study was conducted among purposively selected 45 patients in the Department of Dermatology and Venereology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka who had psoriasis on the basis of history, clinical findings and laboratory report. Regardless of gender, the age ranging from 18-65 years and having no calcium metabolism disorder or impaired renal function were the selection criteria of study participants. All patients were interviewed by Pre-structured questionnaire. Blood sample (5 ml) was collected from each subject and was preserved and analyzed for serum calcium level by the automated analyzer i.e. Beckman Coulter-AU680 in the Department of Biochemistry, BSMMU. Age range of the participants were 18 to 65 years (mean age=34.1±12.1 years). Maximum 26 (57.8%) patients had normal calcium level, 19 (42.2%) patients had hypocalcaemia and no patient was found with hypercalcaemia. Objective: to evaluate the malignant PNS mass by computed tomographic image and the findings of this modality were compared with histopathological result.Methods: It was a cross sectional type of study and carried out with suspected PNS mass having patients during January 2009 to October 2010.Results: The mean age of the patients was 35.95 ± 18.24 and common complaints of the patients were nasal obstruction (73.7%) and maximum 53.9% patients had PNS mass in maxillary sinuses. Thirty patients of pemphigus diagnosed clinically and confirmed by histopathology and immunology, were included in the study. Among the thirty patients, 18(60%) had pemphigus vulgaris. Among the patients of pemphigus foliaceus, 13.33% patients were in the age group of 30-45 years, among the patients of pemphigus erythematosus, 6.67% patients were in the age group of 30-45 years and among the patients of paraneoplastic pemphigus, 3.33% patients were in the age group of 46-60 years. Among the different varieties of pemphigus, highest 66.67% had the disease duration of 1 to 6 months. Male were more than female. Highest 76.50 % showed both mucosal and skin involvement as initial clinical presentation. In future, this study should be carried out on a large scale with prolong period, large sample size and in different hospitals in Bangladesh.

Two hundred thirty two cases who fulfilled the inclusion criteria were studied. This study aimed to assess the outcome of implementation of WHO modified partograph in labour management with special interest to evaluate the fetomaternal outcome. Among the 232 cases, engaged fetal head had more vaginal delivery, 78.8% than unengaged head. The rate of caesarean section was 26.9% in women with unengaged head vs 11.5% in women with engaged head in this study. This study showed that there was no LUCS when cases remained within alert line and outside the action line in partograph all were delivered by caesarean section. The rate of spontaneous delivery was more 58.6% who did not require any augmentation. On the other hand 80 percent cases of LUCS needed augmentation. Topical Calcipotriol can be used either monotherapy or combination therapy with betamethasone. This topical agent has "corticosteroid-sparing" benefit and reduction of side effects, duration of treatment and also relapse rates when used as combination therapy for the treatment of psoriasis. Objectives: The present study was conducted to compare the clinical efficacy between the topical Calcipotriol and combined formulation of topical Calcipotriol plus Betamethasone in the treatment of plaque type psoriasis. Materials and Methods: A clinical trial was carried out from January 2011 to Jan 2014 in outpatient department of Dermatology and Venereology, combined military hospital-Dhaka and Jessore Cantonment and Bangabandhu sheikh Mujib Medical University, Dhaka. Porokeratosis is a clonal disorder of epidermal keratinization, which is characterized by hyperke... more Porokeratosis is a clonal disorder of epidermal keratinization, which is characterized by hyperkeratotic papules or plaques that are surrounded by a thread-like elevated border. The histopathologic hallmark of porokeratosis is the cornoid lamella, which is a thin column of parakeratosis that overlies a thin or absent granular layer and that corresponds to the raised, hyperkeratotic border. Porokeratosis has five clinical types and malignant degeneration has been described in all forms of porokeratosis. We report a forty five year old farmer with a large plaque in chest for 30 years and multiple nodules within the large plaque for 2 years. A section of skin from margin of the plaque reveals histopathological features of porokeratosis and section from nodules reveals an invasive squamous cell carcinoma. To the best of our knowledge, this is the first reported case of porokeratosis transformed to squamous cell carcinoma on Bangladesh.

Hundred twenty patients with onychomycosis were selected by purposive type of nonprobability sampling technique. Majority of the patients 61(150.8%) were in the age group of 21-30 years old. Mean age of the patients were 32.8±14 years and most of the patients were house wives 36(30.0%). Disfigurement 117(97.5%) and discomfort 89(74.2%) were more common chief complaints of the patients. The mean duration of disease was 20.4±15.4 months and nail fold changes were associated with 37(30.8%) patients and more than a half 63(52.5%) of the patients had history of wet works. Intracellular calcium plays an important part in the regulation of proliferation and differentiation of keratinocytes. This study tried to identify the correlation between severity of disease and serum calcium level in psoriasis patients. To assess the serum calcium level in patients of different types of psoriasis and to find out its relationship with disease severity according to Psoriasis Area Severity Index (PASI).This cross sectional study was conducted among purposively selected 45 patients in the Department of Dermatology and Venereology, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka who had psoriasis on the basis of history, clinical findings and laboratory report. Regardless of gender, the age ranging from 18-65 years and having no calcium metabolism disorder or impaired renal function were the selection criteria of study participants. All patients were interviewed by Pre-structured questionnaire. Blood sample (5 ml) was collected from each subject and was preserved and analyzed for serum calcium level by the automated analyzer i.e. Beckman Coulter-AU680 in the Department of Biochemistry, BSMMU. Age range of the participants were 18 to 65 years (mean age=34.1±12.1 years). Maximum 26 (57.8%) patients had normal calcium level, 19 (42.2%) patients had hypocalcaemia and no patient was found with hypercalcaemia. Thirty patients of pemphigus diagnosed clinically and confirmed by histopathology and immunology, were included in the study. Among the thirty patients, 18(60%) had pemphigus vulgaris. Among the patients of pemphigus foliaceus, 13.33% patients were in the age group of 30-45 years, among the patients of pemphigus erythematosus, 6.67% patients were in the age group of 30-45 years and among the patients of paraneoplastic pemphigus, 3.33% patients were in the age group of 46-60 years. Among the different varieties of pemphigus, highest 66.67% had the disease duration of 1 to 6 months. Male were more than female. Highest 76.50 % showed both mucosal and skin involvement as initial clinical presentation. In future, this study should be carried out on a large scale with prolong period, large sample size and in different hospitals in Bangladesh.

Two hundred thirty two cases who fulfilled the inclusion criteria were studied. This study aimed to assess the outcome of implementation of WHO modified partograph in labour management with special interest to evaluate the fetomaternal outcome. Among the 232 cases, engaged fetal head had more vaginal delivery, 78.8% than unengaged head. The rate of caesarean section was 26.9% in women with unengaged head vs 11.5% in women with engaged head in this study. This study showed that there was no LUCS when cases remained within alert line and outside the action line in partograph all were delivered by caesarean section. The rate of spontaneous delivery was more 58.6% who did not require any augmentation. On the other hand 80 percent cases of LUCS needed augmentation. Objectives: The present study was conducted to compare the clinical efficacy between the topical Calcipotriol and combined formulation of topical Calcipotriol plus Betamethasone in the treatment of plaque type psoriasis. Porokeratosis is a clonal disorder of epidermal keratinization, which is characterized by hyperke... more Porokeratosis is a clonal disorder of epidermal keratinization, which is characterized by hyperkeratotic papules or plaques that are surrounded by a thread-like elevated border. The histopathologic hallmark of porokeratosis is the cornoid lamella, which is a thin column of parakeratosis that overlies a thin or absent granular layer and that corresponds to the raised, hyperkeratotic border. Porokeratosis has five clinical types and malignant degeneration has been described in all forms of porokeratosis. We report a forty five year old farmer with a large plaque in chest for 30 years and multiple nodules within the large plaque for 2 years. A section of skin from margin of the plaque reveals histopathological features of porokeratosis and section from nodules reveals an invasive squamous cell carcinoma. To the best of our knowledge, this is the first reported case of porokeratosis transformed to squamous cell carcinoma on Bangladesh.

Hundred twenty patients with onychomycosis were selected by purposive type of nonprobability sampling technique. Majority of the patients 61(150.8%) were in the age group of 21-30 years old. Mean age of the patients were 32.8±14 years and most of the patients were house wives 36(30.0%). Disfigurement 117(97.5%) and discomfort 89(74.2%) were more common chief complaints of the patients. The mean duration of disease was 20.4±15.4 months and nail fold changes were associated with 37(30.8%) patients and more than a half 63(52.5%) of the patients had history of wet works.
