Muir-Torre Syndrome (MTS) is a rare autosomal-dominant disorder characterized by the predispositi... more Muir-Torre Syndrome (MTS) is a rare autosomal-dominant disorder characterized by the predisposition to both sebaceous neoplasm and internal malignancies. MTS-associated sebaceous neoplasms reveal mutations in DNA mismatch repair (MMR) genes and microsatellite instability. A significant part of MTS patients represents a phenotypic variant, the hereditary nonpolyposis colorectal cancer (HNPCC). A strong correlation between microsatellite instability and immunostaining has been demonstrated. The early recognition of sebaceous neoplasm as part of MTS, and their differentiation from sporadic sebaceous neoplasm may have an important application in a clinical setting. The absence of MLH-1 or MSH-2 expression by immunostaining identifies tumors with mismatch repair deficiency. Our aim is to determine whether an immunohistochemical approach, targeting DNA repair proteins MSH-2 and MLH-1 in MTS-related sebaceous neoplasm and their sporadic counterparts, can be used for their identification. The objectives of this study were to identify the most common allergens found at the Patch Testing Clinic (PTC) of the Department of Dermatology, Medical Sciences Campus (MSC) University of Puerto Rico (UPR), to correlate patch testing results with demographics and clinical data, and to compare the results with similar studies in other institutions. A retrospective analysis of the records of patients who underwent patch testing for suspected acute allergic contact dermatitis at the Department of Dermatology of the Medical Sciences Campus of the UPR, from March 31, 2001 to May 31, 2005. Persistent facial swelling may occur as a rare complication of rosacea. This finding has been ref... more Persistent facial swelling may occur as a rare complication of rosacea. This finding has been referred to as lymphedematous rosacea, Morbihan's disease or persistent solid facial edema. A literature review for cases of lymphedematous rosacea revealed that the chronic inflammatory process that accompanies the disease contributes to an increase in the permeability of blood vessels. With time, the lymphatic drainage system becomes permanently impaired, leading to fluid accumulation in the affected skin. Herein, we report the case of a 58-year-old female with history of rosacea who developed bilateral periorbital swelling with associated erythema of the conjunctiva (ocular rosacea) over a 4 month period that only responded to oral corticosteroids. Biopsy revealed changes of lymphedematous rosacea.

The purpose of this study was to determine the incidence of skin cancer in Puerto Rico in a selected year (2005) and to compare these findings with those previously reported for Puerto Rico in 1974 and 1981 and with other countries. The data was collected from the pathology reports corresponding to the period of January to December 2005 of 21 participating Pathology Laboratories throughout Puerto Rico. The rate and distribution of the main types of skin cancer was calculated based on sex, age, anatomic location and laterality. The incidence of skin cancer in Puerto Rico for 2005 was 6,568 cases, which represent a rate of 167.9 per 100,000 inhabitants. The most common type of skin cancer was basal-cell carcinoma. Skin cancer was more common in males except for melanoma, which was more common in females. The incidence increases with age on all types of skin cancer. Due to the low incidence of cutaneous metastasis arising from adrenal cortical carcinoma, its diagnosis can be challenging based solely on histological analysis. Yet, the clinical history in combination with an immunohistochemical panel consisting of melan-A, alpha inhibin, D11, caretinin, neuron specific enolase, synatophysin, and chromogranin, can be useful in differentiating it from other tumors with similar cytomorphology.

To describe and identify those factors associated with remission on pemphigus vulgaris (PV) patie... more To describe and identify those factors associated with remission on pemphigus vulgaris (PV) patients in Puerto Rico. This retrospective cohort study evaluated PV patients followed at the University Puerto Rico (UPR) Bullous Diseases Clinic during the 2000-2010 period. Patients included in the study had clinical and pathologic findings consistent with PV and had a disease duration of at least 3 years. Variables including gender, date of birth, time of disease onset, and date of first partial or complete remission were collected from the medical chart for each study participant. The primary outcome was to determine the number of patients who achieved partial or complete remission. Other secondary outcomes were to identify if partial/complete remission were associated to gender, disease duration, and age at onset of disease. We describe an 18 year-old male patient who was anti-Ro/SS-A and anti-La/SS-B positive and presen... more We describe an 18 year-old male patient who was anti-Ro/SS-A and anti-La/SS-B positive and presented with recurrent annular plaques on the trunk, arms, face, and scalp with evidence of associated patchy alopecia. The skin biopsy revealed a superficial perivascular and periappendegeal lymphocytic infiltrate and focal areas of vacuolar alteration and smudging of the dermoepidermal junction. The patient also presented with a history of xerophthalmia. Skin lesions as well as sicca symptoms responded to antimalarial treatment with hydroxychloroquine. This case demonstrates a new case of annular erythema indistinguishable from those previously described in patients of Asian descent occurring in a Hispanic patient.

It may be a less-common entity encountered in dermatologic practice, but it is often a vexing one. Lesions occurring at this location present particular problems due to the unfamiliarity with clinical assessment, their relative inaccessibility, requiring more surgical finesse, and the lack of experience with histopathologic examination. Obtaining a specimen sufficient for interpretation is one of the main impediments to successful diagnosis in this setting. Most pigmented bands are benign, due to more common entities such as melanocytic activation, lentigo, and/or a nevus; however, deciding which ones are due to melanoma is of the utmost importance and can be difficult. This is a clinical study that attempts to determine the incidence and patterns of cutaneous lesio... more This is a clinical study that attempts to determine the incidence and patterns of cutaneous lesions in 109 pediatric patients with leukemia. Non-specific lesions, namely, adverse reactions to chemotherapy, complications of immunosuppression, and hemorrhagic diathesis were seen in 88.7% of those patients with acute lymphocytic leukemia and in 88.9% of patients with acute non-lymphocytic leukemia. Leukemia cutis was seen in three patients with acute non-lymphocytic leukemia, and in one patient with congenital leukemia. It seems that although skin complications are a frequent event in the course of childhood leukemia, leukemic infiltration of the skin a is rare event.

The purpose of this study was to evaluate the prevalence of dermatologic problems among patients ... more The purpose of this study was to evaluate the prevalence of dermatologic problems among patients witb chronic renal failure (CRF) undergoing hemodialysis or peritoneal dialysis. One-hundred and two patients witb CRF were examined for the presence of cutaneous alterations. All patients examined bad at least one cutaneous lesion. Tbe most prevalent findings were alterations in the cutaneous pigmentation. Of particular interest was an increased prevalence of hyperpigmented macules on the palms and soles. Otber manifestations, seen particularly in the hemodialysis group, included the balf-and-balf nail, pruritus, and keratotic pits of tbe palms and soles. Infectious processes were more prevalent in tbe group undergoing peritoneal dialysis. This study sbowed tbat all patients witb CRF have some type of cutaneous alteration and that the type of dialysis could have some influence upon the incidence of tbese changes.

Background The rate of human immunodeficiency virus (HIV) infection in women of reproductive age ... more Background The rate of human immunodeficiency virus (HIV) infection in women of reproductive age has increased dramatically in the last decade. The effects of pregnancy on the prevalence of cutaneous diseases in this group is unclear. Objectives To compare the prevalence of cutaneous diseases in healthy pregnant women, HIV-positive pregnant women, and HIV-positive nonpregnant women. Methods Fifty HIV-positive pregnant women and 52 HIV-positive nonpregnant women were compared with a control group of 50 HIV-negative pregnant women for the prevalence of cutaneous diseases. Results The prevalence of cutaneous diseases was higher (65%) in the HIV-positive nonpregnant group than in the HIV-positive pregnant group (42%) or the control group (28%). Infectious diseases were more frequent in the HIV-positive nonpregnant group. Conclusions Pregnancy does not seem to increase the prevalence of cutaneous diseases that occur during the course of HIV disease.

Background Solar lentigines are benign keratinocytic proliferations resulting from prolonged and ... more Background Solar lentigines are benign keratinocytic proliferations resulting from prolonged and cumulative sun exposure. The newer photoselective lasers have become the mainstay of treatment. However, cryosurgery and trichloroacetic acid (TCA) solution are cost-effective alternatives in the treatment of solar lentigines. Methods Twenty-five patients with multiple solar lentigines over the dorsa of their hands were included. Each hand was treated randomly with either 30% TCA solution or liquid nitrogen spray. The efficacy of treatment was evaluated at 8 weeks. Results Cryosurgery was more likely to produce significant lightening of the lentigines than 30% TCA solution (P < 0.05) but was more painful and took longer to heal. Conclusion Cryosurgery was found to be superior to TCA 30% solution in the treatment of solar lentigines. This study demonstrates that old-fashioned treatments for solar lentigines are still excellent and cost-effective therapeutic choices.
