Background: This study compares long term results of two femoral bone tunnels in anterior cruciat... more Background: This study compares long term results of two femoral bone tunnels in anterior cruciate ligament (ACL) reconstruction. The femoral tunnel can be drilled transtibially (TT) or through an anteromedial portal (AM). Methods: 300 patients with ACL reconstructions were chosen for this study. They were divided into two groups: 150 patients with anteromedial drilling (AM) and 150 with transtibial (TT) drilling. In the AM group the reconstructions were performed using semitendinosus graft with Tape Locking Screw (TLS™) technique (n=87) or Retrobutton™ femoral and BioScrew™ tibial fixation with a semitendinosus-gracilis graft (n=63). In the TT group the fixation method used was Rigidfix™ femoral and Intrafix tibial fixation. The evaluation methods were clinical examination, knee scores (Lysholm, Tegner, IKDC) and instrumented laxity measurements (KT-2000™). The inflammatory involvement of cervical spine ligaments in patients with rheumatoid arthritis (R... more The inflammatory involvement of cervical spine ligaments in patients with rheumatoid arthritis (RA) with atlantoaxial subluxation was studied. Biopsy specimens were obtained from the ligament structures between the posterior arch of the atlas and the spinous process of C2 during atlantoaxis stabilizing operations. Inflammatory cells in cryostat sections were stained in situ using monoclonal hybridoma antibodies applied in a sensitive avidin-biotin-peroxidase complex (ABC) method. In 4 of 8 ligament specimens, focal inflammatory cell infiltrates consisting mainly of T11 + T lymphocytes (77 +/- 9%) and M1 + monocytes (21 +/- 6%) were found. The occurrence of lymphocyte activation markers Ia, 4F2 and T9, 46 +/- 8%, 38 +/- 10% and 14 +/- 5%, respectively, suggests active involvement of the lymphocytes in the local immune inflammation. Our findings suggest a mechanism eventually leading to slackening of the cervical spine ligamentous apparatus and atlantoaxial subluxation in RA.

This fracture is unique and can present diagnostic problems. Lying in the coronar plane makes the atlas difficult to visualize radiographically. Computer-guided tomography is a helpful diagnostic technique. The brittleness of the cervical spine in long-term rheumatoid patients should also be kept in mind when performing endotracheal intubation.

The purpose of our study was to evaluate retrospectively the mid-term results of the Miller-Galan... more The purpose of our study was to evaluate retrospectively the mid-term results of the Miller-Galante II (Zimmer, Warsaw, USA) unicondylar knee arthoplasty (UKA). The study included 46 patients with medial UKAs. Mean follow-up time was 7.0 years (range 2.7-13.1 years). Survival rate of the prosthesis was 86.6% (95% CI 73.7-99.6) at 7 years. The mean clinical and functional Knee Society Scores had increased from 51 and 62 points preoperatively to 76 and 93 points (P < 0.001) postoperatively. Five of the 46 knees were revised because of excessive wear of the polyethylene liner, and three due to progression of the osteoarthritis in the lateral compartment of the knee. Survival of this fixed-bearing UKA was not as good as previously reported and polyethylene wear seems to be a more common problem than previously assumed.

Of 96 adult patients with a distal intercondylar, intraarticular fracture of the humerus treated ... more Of 96 adult patients with a distal intercondylar, intraarticular fracture of the humerus treated operatively during a 10-year period, 27 patients suffered a local complication, while the total number of individual complications were 34. There were six postoperative infections, 12 neural injuries, and 16 fixation failures. Because of the complication, a reoperation was performed in 12 of the 27 patients. The final outcome was assessed at a reexamination after a mean follow-up period of 6 (2-10) years. Three patients had a permanent dysfunction of the ulnar nerve. In another 3 patients, failure of the fixation had resulted in an established nonunion of the distal humerus. It appeared that the possibilities of internal fixation in several cases had been overestimated.

Of 54 patients with chronic (n = 49) or acute (n = 5) anterior cruciate ligament instability, 52 ... more Of 54 patients with chronic (n = 49) or acute (n = 5) anterior cruciate ligament instability, 52 were evaluated after mean 16 months' follow-up interval after open (n = 18) or arthroscopic assisted (n = 36) bone-tendon-bone patellar ligament reconstruction. Preoperative Knee Signature System side-to-side difference in anterior displacement decreased from 7.9 mm to 3.5 min at follow-up evaluation (p < 0.0001). An objectively satisfactory limit of 5 mm in side-to-side difference was achieved in 73% of the patients. At follow-up evaluation, there was a positive pivot shift sign in eight knees (one definite and seven trace). Average Lysholm knee scores improved from 69 to 83. The only significant difference between the arthroscopic assisted and open groups was smaller side-to-side anterior displacement difference in the arthroscopic group (2.2 mm versus 4.8 mm, p = 0.002). Results suggest that more accurate and isometric placement of the graft is possible with the arthroscopic-assisted technique.

Cementless fixation was used in 35 femoral and in 28 tibial components. There were indications for revision in 11 cases. Conversion to a total knee had been performed in 4 cases, and 1 tibial as well as 1 femoral component had been exchanged, mainly because of polyethylene wear with increasing deformity. 2 femoral components were loose and 1 had fractured.

The popularity of different sports activities has increased during the last three decades, a deve... more The popularity of different sports activities has increased during the last three decades, a development that has been considered beneficial in many respects for society as well as the individual. One of the deleterious effects of this development, however, is an increased number of sports injuries. The exact incidence of sports injuries is difficult to calculate due to many potential biasing circumstances like data collection, definition of sports injuries and the lack of suitable control subjects. In a large study where the acute accidents in Finland were examined the incidence of sports injuries was 670/10,000 inhabitants in 1987. Seven per cent of the total number of injuries seen at a large casualty department were sports injuries. The main purpose of studying sports injuries is to identify the sportsmen at high risk of injury and the main risk factors. Only in this way can preventive measures be undertaken in an effort to make participation in sports safe, regardless of the level or type of sports practiced.

Bone-patellar tendon-bone graft has been the most commonly used graft material in anterior crucia... more Bone-patellar tendon-bone graft has been the most commonly used graft material in anterior cruciate reconstructions, but there has been increasing use of hamstring tendon grafts. However, no existing clinical studies show adequate support for the choice of one graft over the other. Hamstring tendons are equally as good as patellar tendon in anterior cruciate ligament reconstructions. Prospective randomized clinical trial. Ninety-nine patients with laxity caused by a torn anterior cruciate ligament underwent arthroscopically assisted reconstruction with graft randomization according to their birth year. Grafts were either bone-patellar tendon-bone with metal interference screw fixation or double-looped hamstring tendons with metal plate fixation. There were no significant differences between the two groups preoperatively or at operation. Standard rehabilitation included immediate postoperative mobilization without a knee brace, protected weightbearing for 2 weeks, and return to full activity at 6 to 12 months. Forty-three patients in the patellar tendon group and 46 patients in the hamstring tendon group were available for clinical evaluation at a minimum of 21 months after surgery. No statistically significant differences were seen with respect to clinical and instrumented laxity testing, International Knee Documentation Committee Score ratings, isokinetic muscle torque measurements, and Kujala patellofemoral, Lysholm, and Tegner scores. Equal results were seen for patellar and hamstring tendon autograft anterior cruciate ligament reconstructions at 2 years after surgery. Both techniques seem to improve patients' performance.

Two thirds of the patients die of the disease. In retrospective studies the histological grading of the sarcomas has been shown to be the best prognostic sign. Combination of the effective treatment modalities: surgery, radiotherapy and chemotherapy in the primary phase of the treatment as well as the concentration of all the cases in selected care units is recommended.
