The Ninth annual meeting of the European Venous Forum (EVF) was held from 26 to 28 June 2008, in ... more The Ninth annual meeting of the European Venous Forum (EVF) was held from 26 to 28 June 2008, in Barcelona under the presidency of Professor Marc Cairols. It was awarded 17 credit hours by the European Board of Vascular Surgery. The meeting was a great success, attended by nearly 400 delegates from the UK, Europe, USA and Australia. The aim of the EVF meetings is to address many issues with respect to the management of patients with venous disease. There were plenary sessions consisting of guest lectures, two didactic sessions and abstract presentations. As is customary, 30 abstracts were accepted for presentation from over 100 submitted for consideration. They were presented over two and a half days, interspersed over the other sessions and invited lectures. Each abstract was presented over 10 minutes and was followed by a further 10 minutes discussion time – a format that has been successful since the formation of the EVF in 2000. An innovation this year was the live presentation of interesting cases with duplex examination and active audience participation. Following the EVF invited lecture delivered by Professor Jawed Fareed, Professor M Cairols honoured Professor AN Nicolaides with a plaque for his lifelong contribution to the promotion of venous science. Each year the EVF awards three prizes for the best presentations according to originality and quality. The first was awarded to G Mosti (Paper 1.3), the second to JL Gillet (Paper 5.3) and the third to AP Gasparis (Paper 2.3). First and second place winners will receive a scholarship that will enable them to travel to the next meeting of the American Venous Forum (AVF), where they will present their work. The opening ceremony was held on Thursday at the congress venue, Fira Palace Hotel, followed by a cocktail reception. The EVF was founded in 2000 with the aim of presenting selected papers on venous disease of high scientific quality. It currently has over 200 members. The president of each EVF, along with his team, undertakes to organize the congress locally. However, the registration, selection of papers and printing of the scientific programme is done centrally from the organizing committee, whose president is Professor A N Nicolaides. The advantages of being a member of the EVF (annual membership E55 [£40]) are: reduced registration rate for the annual meeting and a reduced subscription rate to Phlebology. Introduction Les veines perforantes (VP) du pied ont un mode de fonctionnement unique : contraire... more Introduction Les veines perforantes (VP) du pied ont un mode de fonctionnement unique : contrairement a toutes les autres VP du membre inferieur, elles drainent le sang des veines profondes (veines plantaires) vers le reseau superficiel. Objectif Decrire l’anatomie des VP du pied qui sont des voies de drainage vers le reseau saphene lors de la systole plantaire. Materiel et methodes Quatre cents membres de 200 sujets non embaumes ont ete injectes au latex neoprene apres lavage a l’eau savonneuse. Une dissection anatomique a ete realisee puis une segmentation coloree. Cent phleboscanners ont egalement ete analyses apres reconstruction 3D par VRT. Resultats Les veines plantaires se drainent directement dans les veines tibiales posterieures a partir du confluent calcaneen, mais aussi dans les veines tibiales anterieures par l’intermediaire de la VP dorsale du pied. Mais les connexions superficielles sont les plus interessantes car les plus mal connues : – avec la veine marginale mediale, origine de la grande saphene a la cheville par 4 perforantes : sous-malleolaire, naviculaire, cuneenne et du premier espace inter-metatarsien. Elles constituent en quelque sorte avec les veines plantaires mediales un complexe fonctionnel medial ; – avec la petite saphene et sa branche achileenne : ce sont la perforante calcaneenne et les PV externes (sous et inter-tendineuses des fibulaires lateraux) qui sont une voie de sortie importante des grosses veines plantaires laterales. Ceci constitue un second complexe fonctionnel lateral. Conclusion Ces donnees anatomiques expliquent le flux ascendant enregistre au Doppler dans la partie basse des 2 troncs saphenes lors de la systole plantaire. La responsabilite des VP du pied doit etre prise en compte dans les varices du pied. L’exploration des veines plantaires et des PV du pied devrait faire partie du bilan cartographique et hemodynamique de tout patient insuffisant veineux.
Morphological studies on the effect of compression on the leg veins are rare and mostly performed... more Morphological studies on the effect of compression on the leg veins are rare and mostly performed in the supine position. The aim of this study was to investigate the influence of compression applied with different pressures on the venous calibre in the standing position. Standing magnetic resonance imaging was used to measure the venous diameters of superficial and deep leg veins in a patient with massive varicose veins without and with different levels of compression and realistic 3D vectorial models were built. In the standing position compression stockings with a pressure of 22 mmHg were able to reduce the calibre of deep calf veins, but not of superficial varices. These were compressed only by bandages exerting pressures between 51 and 83 mmHg. Compression stockings may reduce the diameter of deep calf veins in the standing position. To empty a varicose vein after venous ablation much higher pressures are required.

Objective: Clinical classifications attempt to summarize a large amount of information in a few i... more Objective: Clinical classifications attempt to summarize a large amount of information in a few indices. CEAP is the most comprehensive and widely used classification of chronic venous disorders. The objective was to evaluate, in a routine clinical setting, the information associated with each CEAP clinical class and their ascending severity and additivity. Methods: This work was a multicenter evaluation of newly designed software dedicated to the management of venous diseases. Forty-nine angiologists from nine European countries entered a total of 872 full records of unselected patients. The data were analyzed to evaluate the informational value of each of the clinical classes and to test their ascending severity and additivity, with monovariate and multivariate statistical techniques with SPSS/PC software on the database of the 872 right lower limbs. Results: The series consisted of 700 women (80.3%) and 172 men, aged 18 to 100 years (median, 53 years). The ascending severity of the classes was shown with the statistical association of higher severity C classes with the age of the patient, a history of previous deep vein thrombosis, the diameter class of the most dilated varicose vein, venous symptoms, and the presence of a corona phlebectatica. The additivity, as measured with the Cronbach ␣ coefficient analysis, was satisfactory in highest classes but poorer within the first three classes, and factor analysis of correspondences showed the heterogeneity of the variables that make the classification. Conclusion: The information summarized with the CEAP clinical classes shows a good ascending severity but a poorer additivity. These limitations seem to be related to the heterogeneity of the information content, which suggests some refinements of this basic tool for clinical research in the field of chronic venous disorders.

In addition, incidence and progression of venous symptoms were documented. Methods: From May 2007 to September 2008, we contacted all participants of BVS I and invited them for a reinvestigation. The participants answered a standardized questionnaire and were examined by clinical means and by duplex ultrasound in the same way as in BVS I. Results: The response at follow up after 6.6 years was 84.6%. We reinvestigated 1978 participants. The prevalence for varicose veins (VV) rose from 22.7% to 25.1% and for CVD from 14.5% to 16%. Participants with C-Class C2 as a maximum at BVS I increased to higher C-classes in 19.8% (nonsaphenous VV) and in 31.8% (saphenous VV). In a multivariate analysis, the main risk factors for were age, obesity, and arterial hypertension. Conclusions: These results show a high incidence of progression of CVD to higher C-classes.

Matériel et méthodes : Notre étude a été réalisée sur 45 moulages artériels et 48 angiographies rénales. Nous avons effectué des mesures de la longueur et du calibre de l'artère polaire inférieure, en utilisant un programme computérisé d'analyse des images. Nous avons prélevé des sections de la paroi de l'artère polaire inférieure, qui ont été analysées histologiquement en utilisant les colorations Van Gieson et Masson. Résultats : La longueur et le calibre de l'artère polaire inférieure dépendent de l'origine et du niveau où se fait l'abord du parenchyme rénal. L'artère polaire inférieure a une longueur plus grande (18,9 mm) lorsqu'elle naît de l'aorte (sa longueur pouvant être comparée avec celle de l'artère rénale principale) et une longueur plus petite (10,5-11 mm) si son origine se situe au niveau des artères prépyéliques ou rétropyéliques. Les corrélations concernant le calibre de cette artère sont similaires : son calibre est important lorsqu'elle naît de l'aorte alors qu'il est faible si elle provient des rameaux des artères rénales. Par rapport au niveau de la pénétration dans le rein de l'artère polaire inférieure, sa longueur est plus petite lorsque l'artère aborde le parenchyme rénal dans la partie inférieure du sinus rénal et plus grande si l'artère pénètre le rein par l'extrémité médiale ou la face antérieure du pôle inférieur rénal. La structure des parois des artères polaires inférieures est élastique, avec une tunique moyenne bien représentée. Conclusion : Les paramètres morphologiques et les particularités structurales de l'artère polaire inférieure assurent une pression efficace et un débit circulatoire adéquat au niveau du pôle inférieur rénal.
