Exclusion criteria were: policystic ovary syndrome, severe endometriosis or uterine disease. Patients were randomized using a computer-generated randomization into 2 groups: Hp-Hmg group (n ¼ 32) receiving 75 IU/d, and r-FSH þ r-LH group (n ¼ 35), receiving 75 IU/d of r-FSH þ 75 IU/day of r-LH. Primary outcome was ovarian response in terms of oestradiol (E 2 ) increase (day 1 to day 6 of stimulation and Hcg day) and number of follicles recruited; Secondary outcomes were: duration of stimulation, total gonadotrophin dose, number of dominant follicles, clinical pregnancy rate (PR), ongoing PR, multiple pregnancy, miscarriage and cancellation rate due to risk of ovarian hyperstimulation syndrome (OHSS).

Oocyte maturation, characterized by germinal vesicle (GV) breakdown, formation of first meiotic spindle, extrusion of the first polar body, and arrest in metaphase of the second meiotic division. However, numerous anomalies in the nuclear and cytoplasmic maturation were induced during in vitro maturation. This study was conducted to analyze spindle and chromosome configurations of in vitro maturation failure metaphase I (MI) arrested oocytes collected from PCO patients.

Within every 30 min checking time-frame, those which freshly extruded PBs were regarded as telophase I stage (some were confirmed with immunostaining) and then cryopreserved by the vitrification minimal volume method (Group A). The control oocytes were collected after 14 h of hCG and vitrified at metaphase II stage (Group B). After warming of oocytes from both groups, there was another 6 h (group A) and 2 h of culture (group B) before oocytes were subjected to activation (Ca 2+ -free CZB medium with 10 mM strontium chloride and 5 mg/ml Cytochalasin B for 6 h). To examine the total cell number, the blastocysts were fixed at 100 h post activation and stained with DAPI by epi-florescence microscopy. Results were analyzed by the Chi-square (P<0.05) statistical test.

RESULTS: A total of 5 men (ave 36.8 AE 10yrs) donated their specimens with a mean concentration o... more RESULTS: A total of 5 men (ave 36.8 AE 10yrs) donated their specimens with a mean concentration of 57.5 AE 43x10 6 /ml, motility of 43.0 AE 22%, and 6.4 AE 4% normal morphology. The presence of intact centrosomes in the infertile men was found in 79.0% (226/286) which was lower in comparison to the 92.0% identified in the controls (887/964, P¼0.0001). Interestingly, 2% (5/286) of the spermatozoa from the study group had only 1 signal. The structural integrity assay involving the centrosomal angle was measured consistently among infertile and controls between 30-35 . The assessment in human ciliated fibroblasts indicated that this angle is unique to the spermatozoon. After fixation and staining of 50 spermatozoa, only 38 head+tail pairs were identified. We identified intact centrosomes in 31.6% in isolated tails.

Most endometriosis patients have fertility complaints and, in spite of the improvement of assiste... more Most endometriosis patients have fertility complaints and, in spite of the improvement of assisted reproduction procedures, outcomes remain unsatisfactory. This suggests that there are unknown ethiopathogenic influences that adversely affect fertility. Laparoscopic treatment of endometriosis is of questionable efficiency with regard to achieving better fertility results, with controversies mainly surrounding ovarian residual reserve. However, the laparoscopic approach follows good practice principles and is considered a minimally invasive procedure, with the advantage of being diagnostic and therapeutic. Decision to excise endometriomas must be taken cautiously, considering factors such as patient's age, previous ovarian reserve, previous pelvic surgery, presence of pain or malignancy suspicion, disease extension and the mean diameter of the lesions.
